Tiivistelmä
Lasten ylipaino on sekä Suomessa että kansainvälisesti merkittävä ja kasvava kansanterveydellinen ongelma, joka lisää monien sairauksien riskiä ja heikentää terveyteen liittyvää elämänlaatua. Vanhempien voimavarat ja perheen yksilöllinen tilanne vaikuttavat perheen mahdollisuuksiin ja motivaatioon toteuttaa ylipainoa ehkäiseviä elintapoja. Työssäkäyvien lapsiperheen vanhempien on jaettava voimavaransa ja aikansa perheen ja työelämän vaatimusten kesken. Se miten tämä onnistuu, vaikuttaa koko perheen hyvinvointiin.
Tämä pro gradu -tutkielma on kaksiosainen. Ensimmäisen osan muodostaa systemaattinen kirjallisuuskatsaus, jonka tarkoituksena oli kuvata, miten lapsiperheen vanhemman työ on yhteydessä lapsen ylipainoon tai ylipainoriskiin. Katsauksen tavoitteena oli tuottaa tietoa, jonka avulla voidaan paremmin tunnistaa ja ohjata perheitä, joissa lapsilla on suurentunut riski ylipainoisuuteen. Kirjallisuuskatsauksen tiedonhaku tehtiin Nursing & Allied Health Database, Social Science Database, Cinahl ja PsycInfo-tietokannoista. Kirjallisuushaun lopullinen tulos oli 167 artikkelia. Sisäänottokriteerien perusteella katsaukseen valittiin 38 alkuperäistutkimusta, jotka käsittelivät vanhemman työn ja lapsen ylipainon välisiä yhteyksiä.
Tutkielman toisen osan muodostaa julkaisuharkintaan lähetettävä tutkimusartikkeli: Työn ja perheen yhteensovittamisen yhteys 4-vuotiaan lapsen ylipainoon ja ylipainoisuutta ehkäiseviin ravitsemuksellisiin elintapoihin. Tutkimuksen tarkoituksena oli selvittää miten vanhemman kokemus työn ja perheen yhteensovittamisesta sekä taustatekijät ovat yhteydessä 4-vuotiaan lapsen ylipainoisuuteen ja ylipainoisuutta ehkäiseviin elintapoihin. Elintavoista tarkasteltiin lapsen kasvisten, hedelmien ja marjojen käyttöä sekä perheen yhteisiä aterioita. Lisäksi tarkasteltiin neuvolan terveydenhoitajan tuen tarpeen arviota lapsen elintapoihin liittyen. Tutkimuksen tavoitteena oli tuottaa tietoa, jonka avulla voidaan tunnistaa, tukea ja ohjata paremmin perheitä, joissa vanhemmat kokevat työn ja perheen yhteensovittamisen hankaluutta ja joiden lapsilla on ylipainoa tai riski ylipainoisuuteen sekä pohtia, miten lapsiperhepalveluja tulisi kehittää tähän tarpeeseen vastaamiseksi.
Kirjallisuuskatsauksen tulosten mukaan vanhempien työhön käyttämä aika, työn epäsäännöllisyys, työelämän muutosten ajoitus, työssäkäynti verrattuna kotivanhemmuuteen sekä stressi työn ja perheen yhteensovittamisesta olivat yhteydessä lapsen ylipainoriskiin. Työ on erityisesti vanhemman ajankäyttöön liittyen yhteydessä lapsen ylipainoriskiin ja ylipainoriskiä lisääviin elintapoihin, kuten pikaruuan kulutukseen ja yhteisten aterioiden vähenemiseen. Lasten ylipainoisuuden ehkäisyssä ja hoidossa tulee ottaa huomioon perheen yksilölliset voimavarat myös työn ja perheen yhteensovittamisen haasteiden näkökulmasta. Paljon työskentelevien vanhempien lapsilla saattaa olla suurempi riski ylipainoisuuteen.
Tutkielman tilastollisen osan aineistona käytettiin THL:n vuonna 2018 keräämää FinLapset -aineistoa, joka analysoitiin kvantitatiivisin menetelmin käyttäen SPSS-ohjelmistoa. Tutkimuksessa käytettävä aineisto muodostui lopulta niistä 2145 lapsesta, joilla oli lastenneuvolassa mukana olleen vanhemman vastaus käytettävissä, Avohilmon rekisteritieto ISO-BMIstä sekä vastannut vanhempi oli työelämässä vähintään keskimäärin 16 h viikossa. Aineiston kuvailuun käytettiin frekvenssejä ja prosenttiosuuksia. Ristiintaulukoinnin, khiin neliötestin ja logistisen regression avulla tarkasteltiin eri muuttujien yhteyksiä.
Tulosten mukaan ylipainoisuus oli yleisempää pojilla ja matalammin koulutettujen vanhempien lapsilla. Lapset söivät useammin kasviksia, hedelmiä ja marjoja perheissä, joissa vanhemmalla oli korkeampi koulutus, säännöllinen työ tai vanhempi koki työmääränsä liian suureksi. Perheen yhteinen ateria syötiin päivittäin useammin niissä perheissä, joissa oli kaksi aikuista, vanhemmalla oli korkeampi koulutus tai säännöllinen työ. Kun vanhempi koki, ettei hänen työmääränsä ollut liian suuri, eikä hän kokenut laiminlyövänsä kotiasioita työnsä vuoksi, perheessä syötiin useammin yhteisiä aterioita. Terveydenhoitaja oli arvioinut useammin elintapoihin liittyvää tuen tarvetta perheillä, joilla oli ylipainoinen lapsi mutta työn ja perheen yhteensovittamista kuvaavat tekijät eivät olleet yhteydessä terveydenhoitajan tekemään arvioon lisätuen tarpeesta. Vanhempien kokema työn ja perheen yhteensovittamisen hankaluus ei näytä lisäävän lasten ylipainoisuutta, mutta voi vaikuttaa perheen ylipainoa ehkäisevien ravitsemuksellisten elintapojen toteutumiseen. Children's overweight, both in Finland and internationally, is a significant and growing public health problem that increases the risks of many diseases and reduces the quality of life related to health. Parent’s resources and the individual situation of the family affect the family’s opportunities and motivation to carry out a lifestyle that prevents overweight. Working parents must share their resources and time between the demands of work and family life as much as possible. The success of this affects the well-being of the whole family.
This master's thesis has been divided into two parts. The second part of this master's thesis consists of a research article sent for publication consideration: Reconciling work and family and the risk of obesity in a 4-year-old child. The purpose of the study was to find out how the background factors and the parent's experience of reconciling work and family are related to a 4-year-old child's overweight and lifestyle that prevents overweight. Lifestyle choices included the child's use of vegetables, fruits, and berries, as well as having family meals. The purpose was also to provide information whether the counselor nurse had identified the need for additional support for these families. The aim of the study was to provide information on how to better identify, support and guide families in which parents find it difficult to reconcile work and family and whose children are overweight or at risk for obesity, and to consider how family services should be developed to meet this need.
According to the results of the literature review the risk of overweight in children is associated with the time parents spent at work, the irregularity of work, the timing of changes in working life, working compared to home parenting, and the stress of reconciling work and family life. Especially when it comes to the parent’s use of time, work plays a significant role in the risk of child being overweight and in the lifestyle that increase the risk of being overweight, such as consuming fast food and having less family meals. The prevention and treatment of childhood obesity must also take into account the individual resources of the family from the perspective of the challenges of reconciling work and family. Children of parents who work a lot may be at a higher risk for being overweight.
The material used in the quantitative part of the thesis was FinLapset data collected by the Finnish institute for health and welfare in 2018, which was analyzed by quantitative methods using SPSS software. The material used in the study eventually consisted of 2145 children after combining the Avohilmo registry data on ISO-BMI with the answers of parents involved in the child health care visit who worked an average of at least 16 hours per week. Frequencies and percentages were used to describe the data. Cross-tabulation, chi-square test, and logistic regression were used to examine the relationships between different variables.
The results showed that overweight was more common in children of parents with low levels of education and boys. Children ate vegetables, fruits, and berries more often in families where the parent had a higher level of education, regular work, or the parent felt their workload was too large. A daily family meal was eaten more often in families with two adults or the parent having higher education or regular work. Family meals were eaten more often in families where the parent felt that his or her workload was not too great and he or she did not feel that the household was neglected because of work. The nurse had more often assessed lifestyle-related need for support in families with an overweight child, but factors describing work-family balance were not related to the nurse’s assessment of the need for additional support. The difficulty experienced by parents in reconciling work and family does not appear to increase childhood obesity but may affect the implementation of nutritional lifestyles that prevent family obesity.
