Tiivistelmä
Äidinmaidon terveyshyödyt vauvalle ovat kiistattomat. Erityisesti äidinmaidosta on hyötyä keskosille, joita kuitenkin imetetään vähemmän kuin täysiaikaisina syntyneitä vauvoja. Imetystä tulisi pitää ensisijaisena ja normaalina ravitsemuksen lähteenä vastasyntyneelle ja ponnistella sen onnistumiseksi. Keskosten kohdalla imetyksessä voi olla monia haasteita. Rintamaidon lypsäminen on tärkeä, mutta vaativa keino rintaruokinnan saavuttamiseksi. Äidit tarvitsevat näyttöön perustuvaa ja sensitiivistä imetysohjausta rintamaidon lypsämisen ja imetyksen onnistumiseksi. Selviytyminen rintamaidon lypsämisessä nähdään jatkuvaksi prosessiksi, jossa ongelmallisen tilanteen hallinnan, muuttamisen tai tunteiden säätelyn avulla etsitään ratkaisua tilanteeseen.
Tämä pro gradu -tutkielma koostuu kahdesta osasta. Ensimmäinen osa on julkaisuharkintaan lähetettävä artikkelikäsikirjoitus "Coping Strategies With Breast Milk Expression Among Mothers of Premature Infants: Differences between Mothers of Premature Infants in Hospital and Post-discharge" (Ikonen, Örtengren, Kaunonen). Toisena osana on täydentävä lisäosa.
Tämän tutkimuksen tarkoituksena oli kuvata sairaalahoidossa olevien keskosten äitien (n=129) ja kotiutuneiden keskosten äitien (n=45) rintamaidon lypsämisessä käyttämiä selviytymismenetelmiä ja niissä ilmeneviä eroja. Aineisto oli kerätty vuosina 2015?2016 ja aineisto analysoitiin tilastollisin menetelmin SPSS for Windows (Version 23) -ohjelmalla. Tutkimuksen tulosten mukaan sairaalassa olevien keskosten äidit käyttivät kaikkia viittä eri selviytymismenetelmää enemmän tai saman verran kuin kotiutuneiden keskosten äidit. Tilastollisesti merkitsevä ero löytyi Välttelemättömyys- ja Hallinta-selviytymismenetelmien käytössä sekä selviytymismenetelmien kokonaiskäytössä CMEPI-mittarilla (Coping with Milk Expressing for Preterm Infant) mitattuna. Selviytymismenetelmät, joissa ei ollut tilastollisesti merkitsevää eroa, olivat Aktiivisuus ja opettelu, Mahdollistaminen ja rajoittaminen sekä Positiivinen ajattelu. Lisäosassa on keskosten imetykseen ja rintamaidon lypsämiseen liittyvää kuvausta aiempien tutkimusten pohjalta. Keskostenkin kohdalla on mahdollista saavuttaa haasteista huolimatta hyvät imetystavoitteet. Pohja imetyksen onnistumiselle luodaan jo sairaalassa. Rintamaidon lypsäminen on tärkeässä roolissa maidontulon aloittamisessa ja ylläpidossa. Kotiutumisvaihe on sekä haaste että mahdollisuus imetyksen kannalta. Imetystä on mahdollista tukea joka vaiheessa raskausajasta kotiutumisen jälkeiseen aikaan asti.
Tämä tutkimus on tärkeä avaus rintamaidon lypsämiseen ja erityisesti siihen liittyvien selviytymismenetelmien käyttöön liittyen. Keskosten äideille annettava imetysohjaus kaipaa lisätutkimusta ja hoidon kehittämistä Suomessa. Tutkimuksen tulosten perusteella tulee keskosten äideille tarjota mahdollisuuksia hyödyntää yksilölle tyypillisiä selviytymismenetelmiä ja erityisesti kotiutumisen jälkeen herätellä äidin vähemmän käyttämien selviytymismenetelmien aktiivisempaa käyttöä rintamaidon lypsämisen ylläpitämiseksi. Hoitotyöntekijöitä varten tulisi laatia kotiutumisen jälkeinen prosessikuvaus, joka auttaisi ammattilaisia kartoittamaan äidin selviytymismenetelmiä ja vahvistamaan niitä. Lisäksi tulisi järjestää puitteet, joissa selviytymismenetelmien monipuolinen käyttö on mahdollista.
ABSTRACT
Coping strategies in milk expression of mothers of premature infants in hospital and post-discharge
The health benefits of breast milk for a baby are undeniable. Breast milk is particularly beneficial for premature babies, yet they are less often breastfed than term babies. Breastfeeding should be considered as the primary and normal source of nutrition for a newborn and it is essential to make efforts towards that goal. When it comes to premature infants, breastfeeding can pose many challenges. Expressing breast milk is an important but demanding way to achieve breastfeeding. Mothers need to get evidence-based and sensitive breastfeeding counselling to manage breast milk expression and breastfeeding. Coping with expressing breast milk is seen as a continuous process, which involves finding a solution to a problem by managing or altering the situation, or by emotional regulation.
This Master's thesis consists of two parts. The first is a manuscript for a review entitled "Coping Strategies With Breast Milk Expression Among Mothers of Premature Infants: Differences between Mothers of Premature Infants in Hospital and Post-discharge" (Ikonen, Örtengren, Kaunonen) which will be sent for publication. The secondary part takes the form of a thesis supplement.
The purpose of the study was to describe the coping strategies in breast milk expression, and the differences in coping strategies between the mothers of premature infants in hospital (N = 129) and the mothers of discharged premature infants (N = 45). The data was collected in 2015?2016 and the data was analyzed using statistical methods and program of SPSS for Windows (Version 23). According to the results, the mothers of the premature infants in hospital used all the five different coping strategies more or equally much compared to the mothers of premature discharged infants. A statistically significant difference was found in the use of Non-Avoidance and Mastering, as well as in the overall use of coping strategies using the CMEPI (Coping with Milk Expressing for Preterm Infant) scale. Coping strategies with no statistically significant differences were Activity and Learning, Facilitation and Delimitation, and Positive thinking. The thesis supplement contains a description of breastfeeding and breast milk expression, drawn from previous studies. With premature infants it is possible to achieve good breastfeeding goals despite the challenges involved. The basis for successful breastfeeding is already laid out in the hospital, and breast milk expression plays an important role in the initiation and maintenance of milk supply. The period of discharge is both a challenge and an opportunity for breastfeeding. It is possible to support breastfeeding during all of these periods, starting from pregnancy up to and after the time of discharge.
This study forms an important step in relation to breast milk expression, and in particular to the use of coping strategies related to breast milk expression. The breastfeeding counselling given to mothers of premature infants needs further research and development in Finland. Based on the results of the study, the mothers of premature infants have to be able to utilize typically individual coping strategies, and especially after discharge be made aware of lesser-used coping strategies to maintain breast milk expression. A protocol of post-discharge breastfeeding support should be drawn up for nurses that would help them to determine the coping strategies used by mothers, and to strengthen their use. It is also important to enable the use of diverse coping strategies to promote and maintain breastmilk expression in mothers of premature infants.
