Tiivistelmä
Syöpään sairastuu joka vuosi yhä enemmän ihmisiä, ilmiö on maailmanlaajuinen. Syöpään sairastuminen aiheuttaa monenlaisia psykososiaalisia tukitarpeita, joihin terveydenhuoltojärjestelmämme ei kykene yksin vastaamaan. Myös kolmannen sektorin toimijat, kuten syöpäjärjestöt ja -yhdistykset mahdollistavat sairastuneille erilaisia tukipalveluja, joiden tarkoituksena on parantaa heidän elämänlaatuaan ja auttaa heitä selviytymään elämästään syöpäsairauden kanssa.
Tutkimuksen tarkoituksena oli kuvata syöpäpotilaan psykososiaalista tukea ja siihen liittyviä haasteita syövänhoidon sairaanhoitajien ja syöpäjärjestön neuvontahoitajien määrittelemänä. Tavoitteena oli tuottaa tietoa, jota voidaan hyödyntää kehitettäessä syöpäpotilaan palveluketjussa toimivien sairaanhoitajien antamaa psykososiaalista tukea. Tutkimus oli kvalitatiivinen. Aineisto kerättiin teemahaastatteluin 12/17–9/18 välisenä aikana. Haastatteluteemat muodostettiin kirjallisuuteen perustuen. Haastattelut toteutettiin sairaanhoitajien yksilöhaastatteluina (n=17). Aineisto analysoitiin induktiivisella sisällönanalyysilla.
Sairaanhoitajien kuvaamana syöpäpotilaan kokonaisvaltainen ja yksilöllisiin tarpeisiin vastaava psykososiaalinen tuki oli informatiivista ja psyykkistä tukea, sekä asiantuntemusta syövästä ja sen hoidosta. Se oli myös saatavilla olemista, sairauden ja hoitojen eri vaiheissa annettavaa pitkäaikaista tukea, elämäntilanteen ja arkielämän tarpeiden ymmärtämistä, sekä erilaisten verkostojen ja yhteistyön kautta mahdollistettua tukea. Psykososiaalisen tuen haasteet liittyivät informatiiviseen ja psyykkiseen tukeen, taustatekijöiden moninaisuuteen, sairaalan käytänteisiin ja tiloihin, sekä syöpäpotilaan palveluketjuun ja verkostotyöhön.
Tutkimuksen tulosten perusteella voidaan todeta, että syöpäpotilailla on moninaisia ja yksilöllisiä psykososiaalisen tuen tarpeita ja tuen saamiseen liittyviä haasteita. Tämän tutkimuksen tulokset vahvistivat aikaisempaa tutkittua tietoa aiheesta. Uutena löydöksenä tässä tutkimuksessa korostui syövänhoidossa ja syöpäjärjestöissä työskentelevien sairaanhoitajien tekemän yhteistyön merkitys syöpäpotilaan yksilöllistä ja kokonaisvaltaista tuen saamista mahdollistavana tekijänä.
Tulosten mukaan syöpäpotilaiden tuen saaminen mahdollistuisi paremmin, jos syöpäpotilaan palveluketjussa toimivat tahot olisivat tietoisempia toistensa tarjoamasta tuesta ja tekisivät enemmän yhteistyötä, tukitoimet ja niiden vastuualueet olisi määritelty tarkemmin, tukitarpeita kartoitettaisiin syövänhoidossa systemaattisemmin, syöpäpotilaan tukemiseen olisi syövänhoidossa riittävästi resursseja, esimerkiksi akuutin kriisin hoitoon, vähemmistöjen, sekä maaseudulla ja haja-asutusalueilla asuvien tukipalveluiden saatavuuteen kiinnitettäisiin nykyistä enemmän huomioita, ja vertaistuki kyettäisiin mahdollistamaan kaikille syöpäpotilaille sairauden ja hoitojen eri vaiheissa. Tuloksia voidaan hyödyntää kehitettäessä syöpäpotilaan palveluketjussa toimivien sairaanhoitajien antamaa psykososiaalista tukea. Kehitystyössä tulee huomioida muun muassa hoitajien ammatillisen osaamisen kehittämisen tarve, esimerkiksi psyykkisen tuen antamiseen, sekä syövänhoidon hoitajille kohdennettu lisäkoulutus kolmannen sektorin tarjoamasta tuesta.
ABSTRACT
Cancer patients’ psychosocial support from nursing perspective
The number of people getting cancer diagnosis increases every year. This is a worldwide phenomenon. Beside actual cancer treatments, cancer patients need different kind of psychosocial support which our health care system alone is unable to provide. Therefore, the third sector actors such as different cancer institutes and organisations play an important role in offering and arranging cancer patients and their families different support services, the aim of which is to improve the quality of life for those struggling with cancer and to help them to cope with the illness in their everyday life.
The purpose of this study was to describe cancer patients’ psychosocial support and the challenges related to it from cancer treatment nurses’ and advice service oncology nurses’ perspective. The aim was to generate information which can be useful in developing the psychosocial support nurses provide to cancer patients in different phases and areas of the illness. The study design was qualitative. The data was gathered using theme interviews during 12/17 and 9/18. The themes were formed based on the relevant literature. The interviews were conducted as individual interviews with nurses (n=17). The data was analysed using inductive content analysis.
As described by the nurses, the overall psychosocial support with individual needs was informative and emotional, including information about the cancer and the treatment. Furthermore, it included being available when needed, longterm support in the different phases and treatments of the illness, and understanding the everyday needs and life situation of the patient. Support arranged by different networks and cooperations was also mentioned. The challenges the nurses met when offering psychosocial support were related to giving informative and emotional support itself, the diversity of background factors, hospitals’ policy and premises as well as the service chain and network of the cancer patient.
The results indicate that cancer patients not only have different individual psychosocial needs but also face different challenges in receiving the support services. The results are in line with other studies conducted on the subject. A new finding was the relevance of the cooperation between cancer treatment nurses and nurses acting in different cancer organisations in guaranteeing a cancer patient’s overall support with special needs.
According to the results, the support services would be more easily available for patients if the actors in the service chain were more aware of the support services each actor in the chain offers and they cooperated more, the services themselves and the areas of responsibility were defined more specifically, a patient’s needs for different support were examined more systematically and there were enough resources to meet these needs, for example in an acute crisis, the needs of patients belonging to different minority groups or living in rural areas were paid a special attention to, and if it was possible to arrange peer support for each cancer patient in each phase of the illness and treatment. The results are beneficial in developing the psychosocial support nurses give to cancer patients. In the development process, the nurses’ need for further education for achieving vocational competence should be emphasized, especially regarding education in emotional support given to the patient as well as information about different support services offered by the third sector.
