Evaluation of cognitive complaints and neuropsychological performance in early-onset cognitive impairment and dementia

Alzheimer’s disease (AD) and frontotemporal dementia (FTD) are common forms of early-onset (≤ 65 years) dementia (EOD), but they account for a minority of all causes of early-onset cognitive impairment. The neuropsychological presentations in EOD and late-onset dementia have distinct features, but the differences between EOD and the more common aetiologies (e.g., depression) of early-onset cognitive impairment are not well understood. Assessment methods specifically designed for this population may help in timely and accurate EOD diagnosis.
This study evaluated the utility of the Cognitive Function at Work Questionnaire (CFWQ) in the assessment of subjective cognitive symptoms, compared the INECO Frontal Screening (IFS) and the Frontal Assessment Battery (FAB) tests in the evaluation of executive dysfunction, and delineated differences in the neuropsychological impairment profiles between EOD, mild cognitive impairment (MCI) and subjective cognitive decline (SCD) and between AD, FTD and depression. Data consisted of memory clinic patients (n = 210) from Oulu and Kuopio University Hospitals and media service company (Yleisradio) employees (n = 418).
Patients scored markedly higher than employees on the CFWQ. Higher scores were linked to more severe depressive symptoms and accumulation of strain factors (employee data) and cognitive impairment levels (patient data). EOD and MCI patients with neurological causes performed worse on the IFS and FAB than did patients with nondegenerative MCI or SCD. The IFS had better accuracy in detecting executive dysfunction, and it correlated slightly better with the cognitive impairment level than FAB. The cognitive impairment profiles of EOD patients were dominated by difficulties in memory and executive functions, and differences in other domains and symptom awareness were also evident relative to patients with MCI or SCD. Compared to patients with FTD, AD patients had worse memory retention ability but milder behavioural and affective symptoms. The AD and FTD patients had more impaired symptom awareness, immediate memory and general cognition than the patients with depression.
CFWQ and IFS are promising methods for the initial assessment of patients with early-onset cognitive impairment. The neuropsychological impairment profile is useful in identifying EOD patients and in differential diagnostics of AD, FTD and depression.
Alzheimerin tauti (AT) ja otsa-ohimolohkorappeuma (frontotemporal dementia, FTD) ovat yleisimmät alle 65-vuotiaana alkavat muistisairaudet (early-onset dementia, EOD), mutta valtaosa työikäisten kognitiivisista oireista liittyy masennukseen ja muihin kuormitustekijöihin. Koska EOD:n ja vanhempana alkavien muistisairauksien neuropsykologisissa oirekuvissa on eroja, työikäisille suunnattujen arviointimenetelmien kehittäminen voi auttaa EOD:n varhaisessa tunnistamisessa. Tietoa tarvitaan EOD:n ja muiden oireita aiheuttavien syiden välisistä eroista kognitiivisissa oireprofiileissa.
Tutkimuksessa arvioitiin Työssä muistaminen -kysymyssarjan soveltuvuutta subjektiivisten oireiden tunnistamiseen ja verrattiin toiminnanohjauksen seulontatestien (IFS ja FAB) toimivuutta työikäisten arvioinnissa. Lisäksi tutkittiin eroja neuropsykologisissa oireissa EOD:n, lievän kognitiivisen heikentymän (mild cognitive impairment, MCI) ja subjektiivisen kognitiivisen heikentymän (subjective cognitive decline, SCD) sekä AT:n, FTD:n ja masennuksen välillä. Aineisto koostui Oulun ja Kuopion yliopistosairaaloiden muistipoliklinikoiden potilaista (n = 210) ja Ylen työntekijöistä (n = 418).
Potilaat saivat selvästi korkeampia pistemääriä kysymyssarjassa kuin Ylen työntekijät. Korkeat pistemäärät olivat työntekijöillä yhteydessä vaikeampaan masennusoireiluun ja kuormitustekijöiden kasautumiseen, ja potilailla kognitiivisen heikentymän tasoon. IFS ja FAB testeissä EOD- ja MCI-potilaat, joilla epäiltiin muistisairautta, suoriutuvat heikommin kuin muut MCI- ja SCD-potilaat. IFS erotteli paremmin toiminnanohjauksen häiriöistä kärsivät muista potilaista ja se korreloi hieman korkeammin kognitiiviseen heikentymään kuin FAB. EOD-potilaiden neuropsykologissa oireissa painottuivat muistin ja toiminnanohjauksen vaikeudet, mutta eroja MCI ja SCD potilaisiin oli usealla kognition osa-alueella ja oiretiedostuksessa. AT-potilailla tieto säilyi heikommin muistissa kuin FTD-potilailla, mutta heillä oli vähemmän käytös- ja mielialaoireita. Masennuspotilaiden vaikeudet oiretiedostuksessa, välittömässä muistissa ja yleisessä kognitiossa olivat lievempiä kuin AT- ja FTD-potilailla.
Työssä muistaminen -kysymyssarja ja IFS ovat lupaavia työikäisen muistipotilaan arviointimenetelmiä. Neuropsykologiset oireprofiilit auttavat erottamaan EOD:n muista syistä oireilevista, sekä AT-, FTD- ja masennuspotilaat toisistaan.
