In Emergency medical services (EMS), missions are increasing and the proportion of missions where there is no need for ambulance transportation (non-transport mission) is large. The resources to treat critically ill patient are slim and treatment in time-critical. In an optimal situation, critically ill patients are recognised by EMS and those not in need of emergency care are referred to other health care resources or treated on scene, avoiding overcrowding of the emergency department (ED). A critically ill patient may also have to wait at the ED if the patient is not recognised immediately. This can be very harmful and can delay important or even lifesaving treatment. The general aim of this thesis was to evaluate pre-hospital usage and emergency department care and treatment delays in critically ill patients and to recognise patient-related risk factors for delayed care.
The thesis consists of four retrospective studies. In the first study, the effect of income of the residential area on the EMS use was examined. Residents living in lowest income areas had more age-adjusted EMS missions than residents in higher income areas and there were more age-adjusted non-transport missions and more age-adjusted high-frequency user dispatches person-years.
The second study examined factors associated with time delay from first medical contact (FMC) to coronary angiography with ST-elevation myocardial infarction (STEMI) patients. Multivariate analysis showed that absence of chest pain, dyspnoea as the main symptom, not following the treatment protocol, treatment initiated by General Practitioner (GP) and distance to hospital over 100 km were risk factors for delay.
The third and fourth study focused on patients admitted straight from ED to intensive care unit (ICU) and to operating room (OR). In multivariate analysis, delayed emergency department length of stay (EDLOS) before ICU admission was associated with radiological examination, Glasgow Coma Scale score above 9, absence of pre-arrival notification and thrombocyte level less than 100 x 109/l. Prolonged EDLOS prior to emergency operation was associated with first measurement of vital parametres, time to first examination by physician and gastrointestinal diagnosis.
EMS usage is higher in lower income areas. EMS delays with STEMI patients are associated with diagnostic challenges, long distances and protocol violations. ED delays are also mainly associated with diagnostic challenges.
Ensihoitopalvelun tehtävämäärät ovat nousseet merkittävästi ja usein tehtävät eivät johda sairaalakuljetukseen. Resurssit hätätilapotilaiden hoitoon ovat niukat ja hoito on aikakriittistä. Optimaalisessa tilanteessa potilaat tunnistetaan ja tehokas hoito aloitetaan sairaalan ulkopuolella ja vastaavasti potilaat, jotka eivät tarvitse sairaalahoitoa hoidetaan paikan päällä tai ohjataan muihin terveydenhuollon palveluihin. Vastaavasti, kriittisesti sairas potilas voi joutua odottamaan myös päivystyspoliklinikalla, jos potilasta ei tunnisteta muiden potilaiden joukosta. Tämä voi olla hyvin haitallista ja viivästyttää tärkeää tai jopa henkeä pelastavaa hoitoa.
Väitöskirjan tavoite oli tutkia ensihoidon käyttöastetta ja päivystyspoliklinikalla kriittisesti sairaiden potilaiden hoitopolkuja ja viiveitä ja tunnistaa hoidon viivästymiseen vaikuttavia tekijöitä.
Väitöskirja koostuu neljästä retrospektiivisestä tutkimuksesta. Ensimmäisessä osatyössä tutkittiin asuinalueen tulotason vaikutusta ensihoidon käyttöasteeseen. Matalamman tulotason alueella oli enemmän ikävakioituja ensihoidon tehtäviä, ikävakioituja tehtäviä, jotka eivät johtaneet kuljetukseen sekä ikävakioituja ensihoidon suurkuluttajia.
Toisessa osatyössä tutkittiin ST-nousuinfarktipotilaiden hoitoketjua ensihoidosta sepelvaltimoiden varjoainekuvaukseen. Pääsy varjoainekuvaukseen myöhästyi todennäköisemmin, jos potilas oli ensihoidon kohdatessa rintakivuton tai pääoireena oli hengenahdistus. Myös yli 100 km etäisyys sairaalasta, hoitoprotokollan noudattamatta jättäminen tai yleislääkärin aloittama hoito olivat yhteydessä myöhästyneeseen varjoainekuvaukseen.
Kolmas ja neljäs osatyö koostuivat päivystyksestä teho-osastolle ja leikkaussaliin otetuista potilaista. Monimuuttujamallissa tehohoitoa tarvitsevien potilaiden päivystyskäynti viivästyi, jos tehtiin radiologinen tutkimus, Glasgow Coma Scale Score oli yli 9, ennakkoilmoitusta ei tehty tai trombosyyttitaso oli alle 100 x 109/l. Päivystysleikkausta tarvitsevilla potilailla päivystysleikkauksen viivästyminen oli yhteydessä vitaaliparametrien mittaamisen odotusaikaan, lääkärin ensimmäisen tutkimisen odotusaikaan ja mahasuolikanavan diagnoosiin.
Ensihoidon käyttöaste on suurempi matalan tulotason asuinalueilla verrattuna korkeamman tulotason alueisiin. Ensihoidon viiveet STEMI potilailla liittyvät diagnostisiin haasteisiin, pitkiin välimatkoihin ja protokollasta poikkeamiseen. Päivystyspoliklinikan viiveet liittyvät myös pääasiassa diagnostisiin haasteisiin.
