The study aimed to describe and assess the preparation of preschool children and their parents for day surgery. The study consisted of three sub-studies: I, a systematic literature review, II, a randomized controlled trial (RCT), and III, a multi-method study.
In the first sub-study, interventions from scientific articles were analyzed to determine their effectiveness on children's pain and fear and parents' anxiety and stress in preparation for day surgery. The review included 15 studies that used narrative synthesis and nine that used meta-analysis. The interventions were: functional (n=1), informational (n=10), and a combination of both (n=4). Familiarization visits, written materials and moving images were among the interventions that significantly reduced parents' anxiety and stress. Two of these interventions also reduced children's pain. The results of the meta-analysis showed that these interventions had a statistically significant effect on reducing the parents' anxiety.
The second sub-study evaluated the effectiveness of a mobile application on children's fear and pain and parents' anxiety and stress. Participants were randomly assigned to either a control group (n=35) or an intervention group (n=36). Although the mobile app did not reduce parents' anxiety or children's pain, it did result in a decrease in children's fear in the intervention group. Before the operation, parents in the intervention group experienced significantly more stress than those in the control group. However, after the operation, stress levels significantly decreased for the parents in the intervention group when they were at home. In the third sub-study, parents' views on preparing their children for day surgery were collected using a questionnaire (n=41) and a semi-structured interview (n=15). Parents used cognitive and sensory information to prepare their children for day surgery. They were satisfied with the instructions and valued the preparation methods' usability, content, timing and family orientation.
Preparing families with children for day surgery can involve traditional and digital preparation methods, the starting point of which should be the individual needs of each family and family-centered care.
Tutkimuksen tarkoituksena oli kuvata ja arvioida leikki-ikäisten lasten ja heidän vanhempiensa päiväkirurgiaan valmistamista. Tutkimus koostui kolmesta osatutkimuksesta: I järjestelmällinen kirjallisuuskatsaus, II satunnaistettu kontrolloitu tutkimus (RCT) ja III monimenetelmätutkimus.
Osatutkimuksessa I analysoitiin tieteellisissä artikkeleissa esiintyneitä interventioita ja niiden vaikuttavuutta lasten kipuun ja pelkoon sekä vanhempien ahdistukseen ja stressiin päiväkirurgiaan valmistamisessa käyttäen narratiivista synteesiä (n=15) ja meta-analyysiä (n=9). Katsauksen interventiotyypit olivat toiminnallinen (n=1), informatiivinen (n=10) ja näiden yhdistelmät (n=4). Kuusi interventiota sisälsi liikkuvaa kuvaa, tutustumiskäyntejä ja kirjallista materiaalia, jotka vähensivät tilastollisesti merkitsevästi vanhempien ahdistusta ja kaksi näistä lasten kipua. Meta-analyysin mukaan interventiot vähensivät tilastollisesti merkitsevästi vanhempien ahdistusta. Osatutkimuksessa II arvioitiin mobiilisovelluksen vaikuttavuutta lapsen pelkoon ja kipuun sekä vanhemman ahdistukseen ja stressiin satunnaistamalla heidät kontrolliryhmään (n=35) ja interventioryhmään (n=36). Mobiilisovellus ei vähentänyt vanhempien ahdistusta tai lasten kipua. Interventioryhmän vanhemmat kokivat ennen leikkausta sairaalassa tilastollisesti merkitsevästi enemmän stressiä kuin kontrolliryhmän vanhemmat, mutta stressi väheni tilastollisesti merkitsevästi leikkauksen jälkeen kotona. Lisäksi interventioryhmässä lasten pelko väheni. Osatutkimuksessa III kuvattiin lasten vanhempien näkemyksiä päiväkirurgiaan valmistamisesta keräämällä aineisto kyselylomakkeella (n=41) ja puolistrukturoidulla haastattelulla (n=15). Vanhemmat käyttivät kognitiivista ja sensorista tietoa lapsen valmistamisessa, olivat tyytyväisiä ohjeisiin ja arvostivat valmistamismenetelmien käytettävyyttä, sisältöä, ajoitusta ja perhelähtöisyyttä.
Lapsiperheiden päiväkirurgiaan valmistamisessa voidaan käyttää perinteisiä ja digitaalisia valmistamismenetelmiä, joiden lähtökohtana tulee olla lapsiperheiden yksilölliset tarpeet ja perhelähtöinen hoitotyö.
