Asthma subtypes formed by epidemiological and clinical methods

Asthma is a complex condition with multiple subtypes. The application of asthma subtypes in clinical work remains difficult, since the formed subtypes have been a mixture of etiological-, clinical-, and risk factors as well as personal characteristics. Additionally, only a few studies have been able to form risk factor profiles for asthma subtypes.
We formed clinically meaningful asthma subtypes in three large population-based epidemiological studies: the Espoo Cohort Study (ECS), the Northern Finnish Asthma Study (NoFAS), and the Finnish Environment and Asthma Study (FEAS). In the ECS we divided the population into persistent asthma, transient asthma, early onset persistent asthma, and late onset persistent asthma, and studied the effects of different hereditary categories (parents and/or siblings with asthma or allergic diseases). In the NoFAS and FEAS we used Latent Class Analysis (LCA) to form subtypes of asthma. In the NoFAS we formed subtypes of asthma control and severity and in the FEAS we formed subtypes of adult-onset asthma at the onset of asthma.
In the NoFAS we identified four subtypes among women and three subtypes among men. For women the subtypes were: 1) controlled, mild asthma (41% of women), 2) partly controlled, moderate asthma (24%), 3) uncontrolled asthma, unknown severity (26%), and 4) uncontrolled, severe asthma (9%). For men the subtypes were: 1) controlled, mild asthma (31% of men), 2) uncontrolled asthma, unknown severity (53%), and 3) partly controlled, severe asthma (17%). In the FEAS we identified five subtypes among women and four among men. For women the subtypes were: 1) moderate asthma (16% of women), 2) cough-variant asthma (31%), 3) eosinophilic asthma (16%), 4) allergic asthma (20%), and 5) difficult asthma (16%). For men the subtypes were: 1) mild asthma (33% of men), 2) moderate asthma (35%), 3) allergic asthma (22%), and 4) difficult asthma (10%). In the ECS we studied the effects of heredity on asthma subtypes and found that parental asthma was mostly a risk factor for more persistent types of asthma and that the effect of paternal asthma continues into adulthood, while the effect of maternal asthma diminishes after childhood.
The asthma subtypes can and should be formed according to the study design and study questions. Their connection with asthma endotypes remains to be studied further, but additional knowledge on asthma subtypes is required to enable the individual treatment of asthma.
Astma on monimuotoinen sairaus ja sillä on useita alatyyppejä. Näiden alatyyppien hyödyntäminen kliinisessä työssä on kuitenkin osoittautunut vaikeaksi, koska ne on useissa tutkimuksissa muodostettu käyttämällä sekä riskitekijöitä että kliinisten tutkimusten tuloksia ja potilaiden henkilökohtaisia ominaisuuksia. Samasta syystä näille alatyypeille altistavia tekijöitä ei ole pystytty tutkimaan tarkemmin.
Muodostimme astman alatyyppejä kolmessa isossa väestöpohjaisessa tutkimuksessa: Espoon kohorttitutkimus (ECS), Astman omahoito -tutkimus (NoFAS), ja Ympäristö ja astma -tutkimus (FEAS). ECS- tutkimuksessa astmaa sairastavat jaettiin pysyvään astmaan, ohimenevään astmaan, varhain alkavaan pysyvään astmaan ja myöhään alkavaan pysyvään astmaan. Tutkimme perinnöllisyyden (vanhemmilla ja/tai sisaruksilla astma tai allergisia sairauksia) vaikutusta näiden alatyyppien puhkeamiseen. NoFAS- ja FEAS-tutkimuksissa käytimme alatyyppien luomiseen latentti luokka -analyysiä (LCA). NoFAS:ssa muodostimme astman hoitotasapainoa ja vaikeusastetta kuvaavia alatyyppejä, ja FEAS:ssa aikuisiällä alkavan astman alatyyppejä diagnoosivaiheessa.
NoFAS:ssa tunnistettiin neljä alatyyppiä naisilla ja kolme miehillä. Naiset: 1) Hoitotasapainossa, lievä astma (41 % naisista), 2) Osittain tasapainossa, keskivaikea (24 %), 3) Ei tasapainossa, vaikeusaste ei tiedossa (26 %), ja 4) Ei tasapainossa, vaikea (9 %). Miehet: 1) Hoitotasapainossa, lievä astma (31 % miehistä), 2) Ei tasapainossa, vaikeusaste ei tiedossa (53 %), ja 3) Osittain tasapainossa, vaikea astma (17 %). FEAS:ssa tunnistettiin viisi alatyyppiä naisilla ja neljä miehillä. Naiset: 1) Keskivaikea astma (16 % naisista), 2) Yskä-variantti (31 %), 3) Eosinofiilinen (16 %), 4) Allerginen (20 %), ja 5) Vaikea astma (16 %). Miehet: 1) Lievä (33 % miehistä), 2) Keskivaikea (35 %), 3) Allerginen (22 %), ja 4) Vaikea (10 %). ECS:ssa havaitsimme, että vanhempien ja sisaruksien astma altistaa pysyvämmälle astmalle ja että isän astman vaikutus astman puhkeamiseen jatkuu aikuisuuteen, kun taas äidin astman vaikutus loppuu nuoruudessa.
Astman alatyyppejä olisi syytä muodostaa sekä tutkimusväestön ominaisuuksien että tutkimuskysymysten perusteella. Alatyyppien yhteyttä astman endotyyppeihin olisi hyvä selvittää lisää tulevaisuudessa. Kaikki lisätieto astman alatyypeistä todennäköisesti parantaa astman yksilöllisen hoidon kehittämistä.
