Tiivistelmä
Sairausvakuutuslain muutoksen myötä Kansaneläkelaitos (Kela) ei ole korvannut 1.1.2023 alkaen lääkärin lähetteeseen perustuvan yksityisen fysioterapian kustannuksia. Opinnäytetyön tarkoituksena oli kuvata työikäisen tuki- ja liikuntaelinkuntoutujan (tule-kuntoutujan) yksityisen sektorin fysioterapiaan ohjanneita tekijöitä, sekä selvittää, millainen merkitys fysioterapialähetteellä ja Kela-korvauksella on ollut fysioterapiaan ohjautumisessa.
Opinnäytetyö toteutettiin kvantitatiivisena tutkimuksena. Tutkimusaineisto kerättiin anonyymisti Webropol-verkkokyselyllä toukokuussa 2023, jolloin kyselyyn vastasi 99 yksityisen sektorin fysioterapiapalveluja käyttänyttä työikäistä tule-kuntoutujaa. Kyselytutkimuksen analysoinnissa käytettiin tilastollisia menetelmiä, ja tutkimustuloksia verrattiin aiempaan tutkimustietoon.
Fysioterapeutin ammattitaito ja nopea pääsy fysioterapiaan olivat kyselytutkimuksen mukaan keskeisimmät tekijät ohjautumisessa yksityisen sektorin fysioterapiaan. Yleisimpiä fysioterapiaan ohjanneita tule-vaivoja olivat niska-hartia, olkapää-, polvi- ja akuutit alaselkävaivat. Lääkärin vastaanotolta fysioterapiaan ohjautuminen oli todennäköisempää fysioterapialähetteellä kuin sanallisella suosituksella, ja puolet ohjautui fysioterapiaan ensisijaisesti lähetteellä. Jos lääkäri ei ohjaa tule-kuntoutujaa fysioterapiaan, katkeaa hoitopolku todennäköisemmin ja tule-vaiva voi jäädä kuntouttamatta. Kela-korvauksen poiston myötä noin neljäsosa kyselyyn vastanneista oli vähentänyt käyntejä yksityisen sektorin fysioterapiassa, ja lähes puolet pitäisi Kela-korvauksen palauttamista melko merkityksellisenä tai erittäin merkityksellisenä omalla kohdalla. Aiempi tutkimustieto tuki kyselytutkimuksen tuloksia.
Kehitysehdotuksena esitetään, että yksityisen sektorin tule-fysioterapiaan ohjauduttaisiin suoraan varaamalla aika itse, tai terveydenhuollon ammattilaisen hoidontarpeen arvioinnin kautta. Lääkärin vastaanotolle ohjautuville työikäisille tule-kuntoutujille tulisi tarjota fysioterapialähete, mikä edistää fysioterapiaan ohjautumista. Kela-korvauksen palauttaminen yksityisen sektorin fysioterapiaan, sekä fysioterapian, siihen liittyvien palveluiden ja fysioterapeuttien ammatillisen osaamisen markkinointi, tukisi ohjautumista fysioterapiaan. Tutkimuksesta saatujen tulosten myötä pystytään kehittämään työikäisten tule-kuntoutujien fysioterapiaan ohjautumiseen vaikuttavia tekijöitä. As a result of the amendment to the Health Insurance Act, Social Insurance Institution of Finland (KELA) has not reimbursed the costs of private physiotherapy treatment based on a doctor's referral since 1 January 2023. The purpose of the thesis was to describe the factors that led a working-age person with musculoskeletal disorders (MSDs) to seek physiotherapy in the private sector, and to find out what role the physiotherapy referral and Kela reimbursement played in the referral to physiotherapy.
The thesis was conducted as a quantitative study. The data was collected anonymously through a Webropol online survey in May 2023 and answered by 99 working-age people with MSDs who had used private sector physiotherapy services. Statistical methods were used to analyze the survey and the results were compared with earlier research data.
According to the survey, the professionalism of the physiotherapist and quick access are the key factors in accessing physiotherapy in the private sector. The most common MSDs leading to physiotherapy are neck and shoulder, knee, and acute lower back pain. Patients are more likely to be referred to physiotherapy from a doctor's appointment with a physiotherapy referral than with a verbal recommendation, and half of the people are referred to physiotherapy primarily with a referral. If a doctor does not refer the patient to physiotherapy at all, the treatment pathway is more likely to be interrupted and the condition is more likely to remain untreated. With the abolition of the Kela reimbursement, around a quarter of the respondents have reduced their visits to private sector physiotherapy, and almost half would consider the reinstatement of the Kela reimbursement to be fairly or very relevant for their own case. Previous research data supported the results of the survey.
A development proposal is that referrals to private sector physiotherapy should be made directly by self-booking or through a healthcare professional. A physiotherapy referral should be offered to working-aged people with MSDs who are referred to a doctor. The reinstatement of Kela reimbursement for private sector physiotherapy, as well as the marketing of physiotherapy services and the professional skills of physiotherapists, would support referral to physiotherapy. The results of the study will help to develop the factors influencing the referral to physiotherapy of working-aged people with MSDs.
