Tiivistelmä
Kehittämistyön tarkoituksena oli luoda Helsingin kaupungin palvelukeskustoimintaan ikääntyneen asiakkaan liikuntapalvelun prosessikuvaus ja toimintamalli. Kehittämistyö toteutettiin tutkimuksellisena kehittämistyönä, jonka tavoitteena oli ikääntyneiden toimintakyvyn ylläpysyminen ja kotona asumisen tukeminen.
Kehittämistyön tarkoitukseen pyrittiin asettamalla kolme kehittämistehtävää. Ensimmäisinä kehittämistehtävinä olivat palvelukeskustoiminnan henkilöstölle suunnattu kehittämistyöpaja ja prosessianalyysi, joiden tarkoituksena oli selvittää liikuntapalveluprosessin haasteet ja luoda niihin ratkaisut. Toisena kehittämistehtävänä oli ikääntyneiden asiakkaiden ryhmähaastattelu, jolla pyrittiin liikuntapalvelun käyttäjäkokemuksen selvittämiseen ja asiakasymmärryksen syventämiseen toimintamallia kehitettäessä. Kolmannessa kehittämistehtävässä hyvät käytänteet ja näyttöön perustuvat toimintatavat koottiin toimintamalliksi hyödyntäen kehittämistyön aineiston ja tietoperustan vuoropuhelua.
Kehittämistyön tuloksena syntyneet prosessikuvaus ja toimintamalli luovat raamit toimintakykyyn positiivisesti vaikuttavan liikuntapalvelun toteuttamiselle heikentyneen toimintakyvyn omaavien ikääntyneiden kohderyhmässä. Kehittämistyössä tuotettu prosessikuvaus ja toimintamalli toimivat palvelukeskusten liikuntatoiminnasta vastaavan henkilöstön työtä ohjaavana mallina ja kirkastavat palvelukeskusten roolia ikääntyneiden liikunta- ja kuntoutuspalvelujen ketjussa. Kehittämistyö nosti esiin kehittämistarpeet sisäisen ja ulkoisen yhteistyön, tiedonkulun ja liikuntapalvelun laadun kehittämiselle. Toimintamallia voidaan hyödyntää osana sosiaali- ja terveyspalvelujen yhteistyön kehittämistä ikääntyneiden toimintakyvyn ja kotona asumisen tukemiseksi. The purpose of this development work was to create a process description and model of the exercise group service for elderly customers in Helsinki´s service centers. The development work was conducted as research-based development, with the aim of maintaining the functional ability of the elderly and supporting their ability to live at home.
To achieve the development works’ objectives, three development tasks were established. The first development task included a development workshop for the service center staff and process analysis. These tasks aimed to identify challenges within the exercise group service process and devise solutions to address them. The second development task entailed conducting a group interview with elderly customers, aimed at exploring their user experience with the exercise group service and enhancing customer comprehension during the model development process. In the third development task, best practices and evidence-based approaches were compiled into a model by leveraging the interchange between the project´s data and knowledge base.
The resulting process description and model provide a framework for the execution of exercise group services, which have a positive impact on the functional ability of elderly individuals with reduced physical or cognitive capabilities. The process description and model produced in the development work serve as a guiding model for the staff responsible for the exercise group services in service centers and clarify the role of service centers in the continuum of physical exercise and rehabilitation services for the elderly. The development work highlighted the necessity for improvements in internal and external collaboration, information dissemination, and the quality of exercise group services. The model can be integrated into the collaborative efforts in social and health care services to support the functional ability and living at home of the elderly.
