Tiivistelmä
Suomessa menehtyy joka vuosi noin 300 alle 19-vuotiasta lasta ja nuorta, mutta vain kahdella sairaanhoitopiirillä on alueellinen saattohoitosuunnitelma, jossa on mukana kattava lasten saattohoidon osio. Kuitenkin lasten ja nuorten saattohoitosuunnitelma voisi edistää hoidon yhdenmukaisuutta sekä sen saatavuutta valtakunnallisesti.
Tämän kehittämisprojektina toteutetun opinnäytetyön tavoitteena on selkiyttää ja yhtenäistää Turun yliopistollisen keskussairaalan lasten ja nuorten klinikan toimintaperiaatteita kuolevan lapsen ja nuoren hoidossa, sekä parantaa sairaanhoitajien valmiuksia kohdata palliatiivisessa hoidossa tai saattohoidossa oleva lapsi ja nuori sekä hänen perheensä. Projekti linkittyi osaksi Palliatiivisen koulutuksen kehittämisen -hanketta (EduPal). Tämä opinnäytetyö tuotti tietoa työssä lasten ja nuorten palliatiivisen- ja saattohoidon osaamisen kehittämiseen.
Kehittämisprojektin tarkoituksena oli laatia eettisten periaatteiden, sosiaali- ja terveysministeriön laatukriteereiden, Turun yliopistollisen sairaalan lasten ja nuorten klinikan tarpeiden, ja vertailukehittämisprosessin kautta todettujen hyvien käytäntöjen mukainen, alueellinen lasten ja nuorten saattohoitosuunnitelma. Kehittämismenetelmänä käytettiin vertailukehittämistä, jossa tarkasteltiin toisten yliopistosairaaloiden lasten ja nuorten saattohoitosuunnitelmia, sekä toteutettiin kuvaileva kirjallisuuskatsaus. Kehittämisprojektin tutkimuksellinen osio tuotti tietoa saattohoitosuunnitelman laatimista varten sairaanhoitajien kokemuksista palliatiivisessa- tai saattohoidossa olevan lapsen, nuoren ja perheen kohtaamisesta.
Tutkimuksellisen osan aineistonkeruu toteutettiin puolistrukturoituna verkkokyselynä lasten ja nuoren klinikan sairaanhoitajille (n=37, vastausprosentti 18,5 %). Aineisto analysoitiin kuvailevia tilastollisia menetelmiä apuna käyttäen. Tutkimustulosten mukaan sairaanhoitajat kohtasivat työssään harvoin palliatiivisessa hoidossa tai saattohoidossa olevaa lasta tai nuorta. He kokivat vaikeimpina asioina omat tunteensa, surevien vanhempien kohtaamisen ja tilanteet, joissa vanhemmat eivät halunneet kertoa lapselleen totuutta kuolemasta. Tulosten mukaan lisäkoulutus ja tuki sairaanhoitajille oli tarpeen; etenkin perheen kohtaamiseen, oirehoitoon sekä psykososiaaliseen tukeen liittyen.
Lasten ja nuorten saattohoitosuunnitelma perustui palliatiivista hoitoa ja saattohoitoa määrääviin säädöksiin ja hoitosuosituksiin sekä vertailukehittämisessä tunnistettuihin hyviin käytänteisiin. Suunnitelmaan sisällytettiin suuntaviivat lasten ja nuorten hyvään elämän loppuvaiheen hoitoon sekä toiminta-ohjeita hoitohenkilökunnalle palliatiivisen hoidon ja saattohoidon eri vaiheisiin. Kehittämisprojektin tuloksia voidaan hyödyntää koulutuksen kohdentamisessa ja materiaalina. Each year approximately 300 children and adolescents die before the age of 19 in Finland. Only two of the health care districts have end-of-life care plans that thoroughly include children's needs. End-of-life care plans for children and adolescents could increase the availability of end-of-life care nationally and make the care more uniform.
The aim of the thesis, that was carried out as a development project, is to unify the practices in the Children’s and Adolescents clinic of Turku university hospital when providing care for a dying child. The aim is also to increase the preparedness of nursing staff for meeting families with a child or adolescent in palliative or end-of-life care. The development project was linked to the Palliative education development project (EduPal). This thesis produced information on the development of palliative and end-of-life care for children and adolescents.
The purpose of the development project was to create a palliative and end-of-life care plan for pediatric and adolescent patients in accordance with ethical principles, the quality criteria published by the Ministry of Social Affairs and Health in 2019, and the needs of the Children’s and Adolescents clinic of Turku university hospital. The development method used was benchmarking, which compared end of life care plans for children and adolescents of other university hospitals in Finland. A descriptive literature review was also conducted. The research phase produced information from nurse’s experiences about encountering the families with a child or adolescent in palliative or end-of-life care for developing the end-of-life care plan.
The research part of the development project was carried out using a quantitative research method, a semi-structured online survey. A link to the online survey was sent to the nurses of the Children and Adolescents Clinic and the results were analysed using statistical methods. 37 nurses responded to the survey. According to the results of the survey, nurses rarely encounter children or adolescents in palliative care or end-of-life care at work. They perceive that the most difficult things were their own feelings, facing the suffering of the parents and parents’ refusal to tell the child the truth about death. The results suggest that further training for nurses was needed; especially for encountering the family, for symptomatic treatment and psychosocial support.
The end-of-life care plan was based on legislation and recommendations governing palliative care and end-of-life care and proven good practices found in the benchmarking. The plan includes guidelines for good end-of-life care for children and adolescents. It also includes guidelines for nursing staff during the different stages of palliative and end-of-life care. The results of the development project can be used in allocating training and as material.
