Tiivistelmä
Terveydenhuolto henkilökunnan kokemat uhka- ja vaaratilanteet ovat lisääntyneet. Erityisesti vakavampien väkivaltatilanteiden määrä on kasvussa ja tavallisesti kyseessä on ensihoito- tai ensiaputilanne. Suomessa ratkaisua ongelmaan haetaan jo eduskunnan tasolla heidän vireille laittamallaan lakialoitteella koskien ensihoitotehtävällä toimivan henkilön väkivaltaista vastustamista.
Tämän opinnäytetyön tarkoituksena oli kartoittaa Länsi-Uudenmaan pelastuslaitoksen ensihoitohenkilöstön vuosilta 2017–2019 raportoimia uhka- ja vaaratilanteita suhteessa muihin raportoituihin työtapaturmiin, uhka- ja vaaratilanteiden laatua sekä selvittää millaisia kehitysehdotuksia ensihoitohenkilöstöllä oli tilanteiden ennaltaehkäisemiseksi. Opinnäytetyö toteutettiin kvantitatiivisena tutkimuksena, analysointimenetelmänä käytettiin sisällön erittelyä. Tavoitteena oli auttaa toimeksiantajaa kehittämään uhka- ja vaaratilanteiden ennaltaehkäisyä antamalla määrällistä tietoa henkilöstön raportoimista uhka- ja vaaratilanteista sekä kehitysehdotuksista. Toimeksiantajana toimi Länsi-Uudenmaan pelastuslaitos.
Tuloksista ilmeni raportoitujen uhka- ja vaaratilanteiden selkeä nousu vuodesta 2017 vuoteen 2019. Lähes puolessa raportoiduista tapauksista esiintyi aggressiivista tai uhkaavaa käytöstä tai fyysistä väkivaltaa. Ennaltaehkäisynä ensihoitohenkilöstön puolelta tuotiin esiin ennakointi, lisäavun pyyntö sekä tilanteen ja potilaan taustatietojen selvitys. Moni kehitysehdotus oli kytköksissä toisiinsa; esimerkiksi ajantasaisella koulutuksella voitaisiin mahdollisesti parantaa ensihoitohenkilöstön ennakointikykyä uhkaavissa tilanteissa sekä yhteneväisillä toimintamalleilla viranomaistahojen kesken saataisiin lisättyä ensihoitohenkilöstön työturvallisuutta.
Raportoiduista tilanteista esille nousivat usein päihteet sekä niiden osallisuus tilanteisiin. Jatkossa olisikin hyvä tutkia tarkemmin päihteiden osuutta uhka- ja vaaratilanteisiin, tapahtuma-ajankohdan merkitystä sekä osapuolten sukupuolijakaumaa. Suomessa on yhteensä 22 pelastuslaitosta, joista 21 käyttävät samaa järjestelmää uhka- ja vaaratilanteiden raportointiin, jolloin jatkotutkimukset voitaisiin nostaa valtakunnalliselle tasolle. Situations of threat and danger experienced by health care personnel have increased. In particular, the number of more serious incidents of violence are increasing and these are usually a first aid or emergency care situations. In Finland, a solution to this problem is already being sought at the parliamentary level with a bill they have initiated concerning violent opposition to an emergency care personnel.
The purpose of this Bachelor´s thesis was to find out the situations of threat and danger reported by the emergency care personnel of the Länsi-Uusimaa Rescue Department in 2017-2019 in relation to other reported occupational injuries at work. There was also a meaning to find out what kind of situations of threat and danger were reported and development proposals the emergency care personnel have suggested to prevent the situations. The thesis was carried out as a quantitative research and the content analysis was used as a method of analysis. The aim was to help the client to develop the prevention of the situations of threat and danger by providing quantitative information about the situations of threat and danger and development proposals reported by the staff. The client was the Länsi-Uusimaa Rescue Department.
There was a clear increase in reported threat and danger situations from 2017 to 2019. Almost half of the reported cases involve aggressive or threatening behavior or physical violence. Anticipation, a request for additional assistance and a clarification of the situation and the background information of the patient were suggested as a development proposal on the part of the emergency care personnel. Many development proposals were interconnected; for example, up-to-date training could potentially improve the anticipation in threatening situations and common procedures between authorities could increase the safety of emergency personnel.
Intoxicants and their involvement in the situations were often emerged in the reported situations. Further studies should investigate the role of intoxicants in the threat and danger situations, the time of the incidents and the gender of the participants. There are total of 22 rescue departments in Finland, 21 of which use the same system for reporting threat and danger situations, which could raise further research to the national level.
