Tiivistelmä
Perhetyön perusteet -koulutusta on annettu vuodesta 2013 erään sairaanhoitopiirin alueella psykiatrista hoitotyötä tekevälle henkilökunnalle. Tämän opinnäytetyön tarkoitus oli selvittää, miten hoitohenkilökunta on pystynyt työssään hyödyntämään saamaansa koulutusta. Opinnäytetyö kohdentui aikuispsykiatriaan sairaanhoitopiirin psykiatrian osastolle sekä polikliinisiin yksiköihin. Opinnäytetyön avulla koulutuksen järjestäjät saavat tietoa hoitohenkilökunnan kokemuksista koulutuksen hyödynnettävyydestä työssään sekä ehdotuksia koulutuksen jatkokehittämiseen.
Tietoperustassa perehdyttiin mielenterveystyöhön, perhetyön menetelmiin ja perhetyön koulutukseen. Aineiston keruumenetelmänä käytettiin ryhmämuotoista puolistrukturoitua haastattelua. Tämä valittiin, jotta hoitohenkilökunnan kokemukset koulutuksen hyödynnettävyydestä ja toiveet koulutuksen kehittämisestä tulisivat kuulluksi. Mahdollisuus osallistua haastatteluun tarjottiin jokaisen yksikön perhetyön perusteet -koulutuksen käyneelle henkilökunnalle. Aineisto kerättiin kahdella eri haastattelukerralla sekä mahdollisuutena vastata tutkimuskysymyksiin kirjallisesti. Se analysoitiin aineistolähtöisellä sisällönanalyysillä, jolloin tärkeä tieto säilytettiin ja epäolennainen tieto jätettiin pois. Pelkistämisen, ryhmittelyn ja pohdinnan kautta pääkategorioiksi saatiin puitteiden huomioiminen, menetelmien käyttäminen sekä potilaan ja läheisen kohtaaminen.
Haastattelutulosten mukaan koulutuksen hyödynnetyin osio hoitotyössä oli psykoedukaation antaminen sekä tietoa keräävät haastattelut. Elämänkaaren käyttäminen hoitotyön työkaluna sekä potilaan ja läheisen kohtaamisesta opitut asiat toteutuivat hyvin käytännön hoitotyössä. Joidenkin menetelmien, kuten sukupuun, käyttö koettiin vaativana ja aikaa vievänä. Perhetyö miellettiin vahvasti perhetapaamisten järjestämisenä, toisaalta omien näkemysten perhetyöstä koettiin laajentuneen koulutuksen myötä. Haastateltavien mukaan koulutuksen ajankohdaksi toivottiin aamupäivää iltapäivän sijaan. Kirjallista materiaalia, kuten koulutuskansiota, kaivattiin työyksiköihin. Toivottiin myös uusintatapaamista koulutuksessa opittujen asioiden kertaamiseksi. Työparitoiminnan vähäisyys nousi esille osassa vastauksia.
Jatkossa voitaisiin selvittää, miksi koulutuksen hyödynnettävyys käytännön työssä on jäänyt vähäiseksi ja kuinka työparityöskentelyä voitaisiin paremmin toteuttaa. Toinen jatkotutkimusaihe voisi olla potilaiden ja omaisten kokemukset perhetyön toteutumisesta.
Training in the foundations of family work and consequent use in mental health nursing
In 2013 education on the foundations of family work was provided for personnel working within psychiatric care in the region of Lohja. The purpose of this thesis is to determine how personnel have been able to utilize the acquired education in their practice. This study focused on adult psychiatric wards and outpatient departments at Lohja hospital. The course providers will gain knowledge about how personnel was able to incorporate the education into their practice as well as suggestions for further development of the training program.
Mental health work, family work, and family work education formed the background theory. A grounded theory approach using semi-structured, group interviews were used. This approach enables the exploration of experiences and opinions regarding the application and further development of education. All personnel that had taken part in the family work education were invited to take part in the study. Data were collected during two rounds of interviews and through written responses. A content analysis of the data was carried out, useful information was saved and irrelevant was excluded. Through reduction (simplification), group coding and discussion the following main categories were established, environment, utilization of methods and contact with patient and relations.
According to interview results, the beneficial aspects of the training were psychoeducational as well as information generating interview techniques. Life mapping as a tool in care work as well as skills connected to interactions with patients and relatives were well incorporated into practice. The adoption of other methods, such as a family tree, was more demanding and time-consuming. Family work was strongly conceived as meetings with the family, however, the perception of family work was broadened through the training. According to the participants, mornings rather than the afternoons, was a more desirable time for training to take place. Written material, such as a training manual, would be appreciated by the units that took part. Participants also expressed a desire for a follow-up training session.
Future research could explore why the training was incorporated into practice at such low levels and how pair work could be more successfully achieved in the future. Another topic for further exploration could be patient’s and relatives’ experiences of family work.
