Tiivistelmä
Ensihoitotyö on henkisesti haastavaa sekä luonteeltaan hyvin vaihtelevaa. Tehtävän kriittisyys, ympäristö sekä ajallinen kesto eivät aina ole ennakoitavissa, lisäksi työhön liittyy pelko omasta turvallisuudesta. Erityisen kuormittavat työtilanteet, kuten onnettomuudet, menehtyneet lapsipotilaat, loukkaantumiset ja uhka- ja väkivaltatilanteet lisäävät riskiä traumaperäiselle stressihäiriölle. Ensihoitajan perustaitoihin kuuluu työssäjaksamisesta huolehtiminen. Stressin vastustuskykyä voidaan kasvattaa altistumalla sopivalle määrälle stressiä, esimerkiksi oppimisprosessin kautta.
Opinnäytetyön tavoite oli koota tietoa Kaakkois-Suomen ammattikorkeakoululle ensihoitajien koulutusohjelman sisällön kehittämistarpeen arviointia varten. Opinnäytetyön tarkoitus oli tutkia, kouluttaako Kaakkois-Suomen ammattikorkeakoulu ensihoitajaopiskelijoita kohtaamaan ensihoitotyön henkistä kuormittavuutta, stressaavuutta ja korkeaa posttraumaattisen stressihäiriön riskiä. Tämän lisäksi selvitettiin, kokevatko ensihoitajaopiskelijat tarpeelliseksi koulutuksen edellä mainituista ilmiöistä.
Opinnäytetyö toteutettiin määrällisenä kyselytutkimuksena ja sitä pohjustavalla kirjallisuushaulla. Kysely kohdennettiin Kaakkois-Suomen ammattikorkeakoulun ensihoitolinjan loppuvaiheen opiskelijoille. Kyselytutkimus lähetettiin opiskelijoiden Learn-alustalle. Alusta tavoitti 80 kyselytutkimuksen kriteerit täyttävää opiskelijaa, joista 37 vastasi kyselytutkimukseen. Tällä saavutettiin 46 % vastausprosentti.
Saman ensihoito-opetuksen läpi käyneet opiskelijat olivat eri mieltä siitä, onko ensihoitajan työn henkisestä kuormituksesta ollut opetusta. Vastaajat toivat kuitenkin selvästi esiin puutteet opetuksessa. Valtaosa vastaajista kokee aloittavansa ensihoidon harjoittelut ilman riittäviä valmiuksia kohdata ja käsitellä ensihoitotyön stressitekijöitä. Vastaajista suurin osa koki tarpeelliseksi niin ensihoitotyön henkisen kuormituksen, stressaavuuden, posttraumaattisen stressioireiston että jälkipurkumenetelmien koulutuksen opintojen aikana. Emergency responders' work is mentally challenging and highly varying. Critical situations, environments and the duration of a call are unpredictable. Paramedics must also worry about their own safety during calls. Especially burdening scenes like serious car accidents, deceased children, and the threat of violence against oneself will increase an emergency responder’s possibility of having a stress-related traumatic reaction. The paramedics' basic skills should include taking care of their own well-being. Due to the nature of emergency work, emergency responders have a great risk of developing a mental disorder, and the risk of PTSD is roughly 40% higher than that of ordinary citizens. Then again, one’s tolerance for stress and mental problems can be increased with training for these stressful situations and better education in understanding stress and resilience.
The goal of this thesis was to gather information for the South-Eastern Finland University of Applied Sciences (XAMK) to evaluate the need for improvement in the paramedics' study program. The purpose was to investigate whether XAMK educates future paramedics to face work-related stress, mental burden, the high risk of developing PTSD and the effects of these on an individual. And do students wish to have training of those phenomena.
The research was performed by conducting a quantitative survey with a basis in literature. The survey was targeted at paramedic students of XAMK who had reached the final stages of their studies. The survey was distributed online through the Learn platform. It reached 80 students that met the criteria, and 37 of them answered the survey giving it a response rate of 46%.
Students who had gone through the same education process disagreed with each other about whether there had been training regarding work-related stress and burden or not. Nevertheless, they clearly pointed out the inadequate education in these phenomena. Most of the respondents felt that they must begin their first practical training in emergency care without proper tools and skills to face and to handle the mentally challenging aspect of a paramedic’s work. Education and training to withstand work-related stress, the mental burden and the high risk of developing PTSD, as well as coping methods after crises and their effects on an individual paramedic are important topics. Nearly all agreed that these phenomena should be addressed more and be a more coherent part of the study program.
