Tiivistelmä
Potilasturvallisuuden varmistaminen on keskeinen osa hoitotyötä ja terveydenhuollon viestinnällä on suuri merkitys potilasturvallisuuden toteutumiselle. Puutteellinen viestintä voi aiheuttaa huomattavaa haittaa potilaalle sekä hoidon laadulle ja jatkuvuudelle etenkin suullisen raportoinnin yhteydessä. Tutkimuksien mukaan tiedonkulun ongelmat ovat myötävaikuttamassa jopa 70 prosentissa terveydenhuollon haittatapahtumissa. Tiedonkulkuun liittyvien virheiden todennäköisyyttä on mahdollista vähentää hyödyntämällä yksinkertaisia, systemaattisia ja strukturoituja raportointimenetelmiä.
ISBAR-raportointimenetelmä on tutkituin suullisen raportoinnin ja konsultoinnin työkalu. ISBAR-menetelmän käytön tavoitteena on lisätä potilasturvallisuutta, vähentää haittatapahtumia, yhdenmukaistaa toimintaa sekä parantaa hoidon laatua. ISBAR:in avulla oleellinen tieto potilaan tilasta ja tilanteesta siirtyy eteenpäin johdonmukaisessa, tiiviissä ja selkeässä muodossa. Menetelmän käytön opetus ei ole tällä hetkellä kuitenkaan riittävää terveydenhuollon perusopetuksessa.
Tämän toiminnallisen opinnäytetyön tarkoituksena oli lisätä sairaanhoitajaopiskelijoiden tietoutta ISBAR-raportoinnin käytöstä, havainnollistaa ISBAR:in merkitystä potilasturvallisuutta lisäävänä tekijänä sekä kannustaa menetelmän käyttöönottoon hoitotyössä. Toiminnallisen opinnäytetyön tehtävä oli suunnitella ja tuottaa laadukas heräteopetusvideo Karelia-ammattikorkeakoulun uudelle Syventävä potilasturvallisuus -kurssille ja sen kohderyhmänä olivat syventävän vaiheen sairaanhoitajaopiskelijat. Videolla kuvataan sekä huonon että esimerkillisen raportoinnin merkitystä potilasturvallisuudelle. Jatkokehitysideana olisi lisätä ISBAR:in käytön opetusta sairaanhoitajaopinnoissa ja mitata opetuksen lisäyksen vaikuttavuutta työelämässä. Effective communication is an essential component of safe patient care since it reduces the occurrence of errors and improves the continuity of care. It has been discovered that 70 percent of adverse events are caused by deficiency in communication. Structured communication tools have been demonstrated to enhance efficient communication, and thus, patient safety.
The ISBAR reporting method is the most studied reporting tool in healthcare. Its main objective is to increase patient safety and the quality of care by improving the communication of essential information during patient handovers and consultations. ISBAR consists of a standardized structure that ensures assertive and effective communication and reduces the likelihood of errors. However, the training and teaching of ISBAR are not yet sufficient in Finnish healthcare education.
The purpose of this practice-based thesis was to increase nursing students’ knowledge of ISBAR reporting and to demonstrate the importance of effective communication for patient safety. The aim was to increase students’ competence to use the ISBAR method in practice in different healthcare environments. The main objective of the thesis was to create a stimulating educational video about the ISBAR reporting method to be used as a part of advanced patient safety nursing studies at Karelia University of Applied Sciences. The video illustrates the difference and consequence between incomplete and adequate reporting for patient safety. A further development idea would be to increase ISBAR training in Finnish nursing education and measure its effectiveness for the patient safety and quality of care in a certain healthcare environment.
