Tiivistelmä
Sähköiset terveyspalvelut (eHealth) monipuolistavat potilaiden mahdollisuuksia osallistua omaan hoitoonsa. eHealthin on todettu lisäävän potilaan kokemaa turvallisuutta, parantavan kommunikointia potilaan ja ammattilaisten välillä sekä tukevan potilaiden voimaantumista. Motivoituminen ja sitoutuminen eHealth -palvelujen käyttöön on kuitenkin osin haasteellista. Tutkimusten mukaan potilaat ovat pääosin kiinnostuneita eHealth palveluista, mutta sitoutumiseen vaikuttavat erityisesti neljä pääteemaa: potilaan kokemat hyödyt/haitat palvelusta, potilaan saama ohjaus, potilaiden asenteet sähköisiä terveyspalveluita kohtaan sekä palveluiden käytettävyys. My MS -palvelu on näistä yksi esimerkki: MS -potilaat voivat raportoida voinnistaan ja oireistaan ammattilaisille palvelun avulla.
Tässä opinnäytetyönä tehdyssä kehittämisprojektissa tarkasteltiin MS-tautia sairastavien potilaiden kokemuksia My MS -palvelusta. Työssä selvitettiin, mitkä tekijät edistävät ja mitkä estävät motivoitumista palvelun käyttöön sekä mitä kehitettävää palvelussa mahdollisesti on. Kehittämisprojektiin sisältyvä tutkimus toteutettiin teemahaastatteluina, joihin osallistui kaikkiaan 11 My MS -palvelua käyttänyttä tai käyttävää potilasta.
Tulosten mukaan potilaiden motivoitumista My MS -palvelun käyttöön edistävät ammattilaisilta saatu riittävä tuki ja ohjeistus, palvelusta koetut hyödyt, kuten oman sairauden ymmärryksen lisääntyminen sekä yleinen myönteinen asenne sähköisiä palveluja kohtaan. Motivoitumista estävät erityisesti palvelusta koetut harmit, esimerkiksi haasteet saada kuvattua vointiaan oirekyselyiden avulla, puutteelliset palvelun käyttötaidot sekä MS-taudin kognitiiviset häiriöt, kuten väsymys ja muistiongelmat.
MS-tautia sairastavien potilaiden tarpeet My MS -palvelua kohtaan ovat vaihtelevia. Ammattilaisten rooli palvelun ohjaamisessa on tärkeä; suunnitelmalliseen, yksilölliseen ja monikanavaiseen ohjaukseen kannattaa panostaa. Itse palvelun kiinnostavuuteen ja hyödyllisyyteen omahoitoa tukevana palveluna voidaan vaikuttaa muun muassa lisäämällä palveluun vuorovaikutuksellisuutta, potilaan tarpeisiin räätälöityvää sisältöä sekä tukemalla kommunikaatiota palvelun välityksellä. Electronic health care services (eHealth) enable patients for more diverse participation of their own treatment. Studies show that eHealth can increase patient experienced security, improve patient-clinician communication and support patient empowerment. Patients are interested in eHealth in general, but the motivation to use and to commit to eHealth can be a challenge. There are four major themes which can affect to the commitment: experienced benefits regarding the service, patient education, the attitude towards eHealth and the usability of the services. eHealth service My MS enables patients with MS to report their health status and symptoms to health care professionals.
The research of this development project was implemented with theme interviews. Interviews were conducted with 11 MS patients who use or have used the My MS service.
The aim of this research was to explore and study the experiences that patients with MS had about the My MS service. The research questions were: What reasons promote or impede the motivation to use the service and what kind of improvement needs the service has? The research was conducted with theme interviews for 11 MS patients who had experience of using the My MS service.
The results show that the motivation to use the My MS service improved through user support and education from the health care professionals. Experienced benefits, such as the increased understanding of their own disease and overall positive attitudes towards electric services, promoted the motivational factors. Difficulties to give adequate answers about their health, lack of ability to use the service and cognitive challenges, such as fatigue or memory issues, were the main reasons for impeded motivation.
The patient’s needs for the My MS service vary. The clinicians have an important role in guiding the patients and organized, personalized and multi-formed user education of the service is necessary. Adding more interaction, tailored contents and supporting communication between the patient and the clinician through the service promote both self-care and the use of My MS service.
