Tiivistelmä
Syöpälääkehoidot ovat keskeinen osa nykypäivän yksilöllisesti räätälöityä syövän hoitoprosessia, minkä turvallinen toteutuminen vaatii korkealaatuista potilasohjausta. Laadukkaan potilasohjauksen varmistamiseksi tulisi käyttää standardisoituja toimintatapoja. Tarkistuslista on yksi tapa toteuttaa ohjausprosessi systemaattisesti samalla tavalla kerrasta toiseen riippumatta siitä, kuka hoitajista ohjauksen antaa. Ohjauskäytänteitä yhtenäistämällä voidaan edistää potilasturvallisuutta. Opinnäytetyön tavoitteena oli jatkokehittää systemaattista toimintatapaa osana syöpälääkehoidon potilasohjauksen laadunvarmistusprosessia. Tarkoituksena oli tutkimusnäyttöön perustuen vahvistaa aiemmin laaditun ja pilottikäytössä olevan syöpälääkehoidon potilasohjauksen tarkistuslistan teoreettista pohjaa. Työn toimeksiantajana oli Pirkanmaan sairaanhoitopiiri.
Opinnäytetyö toteutettiin integroituna kirjallisuuskatsauksena sekä tarkistuslistan pilottiversion käyttökelpoisuutta arvioivana kyselynä (n=30). Kirjallisuuskatsauksen aineisto analysoitiin vertailevalla sisällönanalyysillä ja käyttäjäkyselyn tulokset kuvailevia tilastollisia analyysejä käyttäen. Tulokset osoittivat, että aiemmin pilotoitu tarkistuslista sisälsi niitä keskeisiä osa-alueita, joita syöpäpotilaan ohjaukseen pitää kuulua. Tarkistuslistaa käyttäneet hoitajat suhtautuivat yleisesti ottaen positiivisemmin listan käytön hyödyllisyyteen kuin vielä listaa käyttämättömät hoitajat. Tilastollisesti merkittävää eroa ryhmien välillä oli väittämissä tarkistuslista toimii muistin tukena (p=0,027) sekä tekee potilasohjauksesta systemaattisempaa (p=0,046), joiden osalta listaa käyttäneet hoitajat arvioivat käytön selkeästi hyödyllisemmäksi.
Opinnäytetyö tuotti lisätietoa tarkistuslistan käytöstä syöpälääkehoidon potilasohjauksessa. Aineistojen analysointi selkeytti tarkistuslistan asiakokonaisuuksien hahmottamista ja uudelleen jäsentämistä. Työn tuotoksena laadittiin päivitetty ja teoreettisesti vahvistettu tarkistuslista sairaanhoitajien apuvälineeksi syöpälääkehoidon potilasohjaustilanteisiin. Opinnäytetyön tuloksia voidaan tulevaisuudessa hyödyntää alueellisissa syöpäkeskuksissa tavoiteltaessa hoitokäytänteiden yhtenäistämistä ja kehittämistä niin Suomessa kuin kansainvälisestikin. Parhaimmillaan tarkistuslistaa voi käyttää potilasohjauksen laadunvarmistuksen työkaluna syövänhoidon yksiköissä. Opinnäytetyö toimii pohjana jatkotutkimukselle ja mahdollistaa vahvistetun tarkistuslistan käytön vaikuttavuuden tutkimisen syöpälääkehoidon potilasohjauksen kontekstissa.
Abstract: Cancer drug therapies with antineoplastic agents are a key part of today's individually tailored cancer treatment process, which requires high quality patient education. To ensure this, standardized practices should be used. The checklist is one way to provide the patient education process systematically in the same way from time to another regardless who of the nurses performs the patient education. Patient safety can be increased by uniting patient education practices. The aim of the thesis was to further develop a systematic approach as part of the cancer drug therapy patient education quality assurance process. With evidence-based methods, the purpose was to strengthen the theoretical basis of the previously formed and piloted cancer drug therapy patient education checklist. The commissioner of the thesis was Pirkanmaa Hospital District.
The thesis was carried out as an integrated literature review and a survey (n=30) of the usefulness of the pilot version of the checklist. The material in the literature review was analyzed using constant comparison method and the results of the survey were presented using descriptive statistical analyzes. The results showed that the previously piloted checklist contained the key contents that should be included in the chemotherapy patient education. Nurses who have used the checklist, reacted generally more positive about the utility of the list than those who have not used the list yet. In this study, between the respondent groups, the difference concerning utility of the checklist was statistically significant for claims checklist serves as a memory support (p=0.027) and makes the patient education more systematic (p=0.046), the nurses who have used the list evaluated the use of the checklist to be more useful.
The thesis produced research data on the use of the checklists in cancer drug therapy patient education. The analysis of the both data clarified the conceptualization and restructuring of the contents in the checklist. As a result, an updated and theoretically strengthened checklist was formed as nurses' tool for cancer drug therapy patient education. In future, the results can be utilized by seeking to harmonize and develop clinical practices in regional cancer centers in Finland but also internationally. At its best, the checklist serves as a quality assurance tool for patient education in cancer treatment units. The thesis serves as a basis for further research and enables to study the effectiveness of using the strengthened checklist in the context of cancer drug therapy patient education.
