Tiivistelmä
Vertaisarviointia on käytetty Suomessa sairaalan sisäisessä työyhteisössä jonkin verran, mutta ensihoidossa sen käyttöä on tuotu tietoisuuteen vasta muutaman vuoden ajan. Vertaisarviointi pohjautuu Learning from excellence -ideologiaan, joka on saanut alkunsa Iso-Britanniasta. Sen tavoite on parantaa potilasturvallisuutta ja hoidon laatua. Vertaisarviointi on hyvien suoritteiden löytämisen lisäksi myös kollegan työn arvostamista ja esiin tuomista. Vertaisarvioinnin kautta on mahdollista oppia toisten tekemästä työstä ja jakaa osaamista kollegiaalisesti. Tämän opinnäytetyön tarkoituksena oli selvittää Pohjois-Savon pelastuslaitoksen ensihoitohenkilöstön mielipiteitä suunnittelemamme vertaisarviointilomakkeen toimivuudesta ja kehittämiskohteista sekä vertaisarvioinnin soveltuvuudesta ensihoitoon. Opinnäytetyön tavoitteena oli suunnitella sähköinen vertaisarviointilomake Pohjois-Savon pelastuslaitokselle sekä antaa ensihoidon henkilöstölle käyttökokemus vertaisarvioinnista. Ensihoitohenkilöstöltä saatava palaute tuotti tietoa kuinka vertaisarviointia voidaan tukea ja millä tavoin vertaisarviointilomaketta tulee kehittää tulevaisuudessa. Vertaisarviointilomakkeen tuottaminen oli tutkimusmenetelmältään kehittämistyö, mutta palautteen keräämiseen sekä analysointiin oli käytetty määrällisen tutkimuksen menetelmiä. Palautetta kerättiin sekä sähköisellä että paperisella lomakkeella Pohjois-Savon pelastuslaitoksen Neulamäen, Petosen, Varkauden ja Suonenjoen yksiköissä. Sähköinen lomake lähetettiin yhteyshenkilölle sähköpostitse. Sähköposti sisälsi vertaisarviointi- ja kyselylomakkeet sekä saatekirjeen, jossa kerrottiin työn tarkoituksesta. Paperinen lomake ja saatekirje olivat nähtävissä pelastuslaitoksen ilmoitustaululla. Aineiston analyysissa hyödynnettiin ei-parametristä Mann-Whitneyn U-testiä. Vertaisarviointiin osallistui vastaajista vain viisi henkilöä (N=52), joten todelliset tulokset lomakkeen käytöstä jäivät vähäisiksi. Vertaisarviointi havaittiin kuitenkin hyödylliseksi ja sen koettiin lisäävän oppimista ja työssä kehittymistä. Motivaation lisääntyminen palautteen antamisen suhteen jakoi vastaajien mielipiteitä. Tämä oli huomattavissa myös selvitettäessä vertaisarvioinnin sopivuutta ensihoitoon. Näihin vaikuttivat luultavasti vastaajien puutteellinen motivaatio ja perehtyminen aiheeseen. Tuloksista oli huomattavissa, että henkilöt, jotka olivat osallistuneet vertaisarviointiin, olivat sitä mieltä, että sitä tulisi jatkaa myös tulevaisuudessa. Vastaajat, joiden mielestä vertaisarviointia ei tulisi jatkaa, eivät olleet osallistuneet vertaisarviointiin. Vertaisarvioinnin anonymiteetistä tulokset eivät tuoneet esille selkeää mielipidettä puolesta tai vastaan. Vertaisarvioinnilla koettiin myös olevan mahdollista antaa rakentavaa palautetta. Esimiehen tulee antaa palautetta vertaisarvioinnista mahdollisimman pian suoritteen jälkeen. Vertaisarviointia tulee jatkaa ympäri vuoden, jotta sen käyttö ei jäisi kertaluonteiseksi ja se saataisiin vakioitua jatkuvaan käyttöön. Pidemmällä aikavälillä saaduista palautteista on mahdollisuus kerätä kooste ja tuottaa tietoa toimivista käytännöistä ensihoidossa esimerkiksi erilaisten helposti omaksuttavissa olevien tietoiskujen muodossa.
The peer review concept is not new in Finnish hospital organizations, but in the context of first aid it has only been used for a few years. Peer review is based on the Learning from excellence ideology, originated in the United Kingdom. A peer review aims at improving patient safety and quality of management. In addition, the value of peer review is in showing both good practices and appreciation of colleague’s work. Throughout the process there are opportunities for sharing knowledge and learning from the work of others. The purpose of this thesis was to investigate the opinions of the first aid personnel of Northern Savo Rescue Department on the functioning and development of the peer evaluation form that we have designed and appropriateness of the peer review in the first aid. The aim of the thesis was to plan an electronic peer review form for the Northern Savo Rescue Department and to provide first aid personnel with a peer evaluation experience. The feedback from the first aid personnel will provide support for the development of peer review and peer evaluation form in the future. Generating the peer review form can be chacterized as a developmental research method. In collecting and analyzing the feedback process quantitative study methods were used. Feedback was collected through electronic and paper questionnaires from the personnel in the Northern Savo Rescue Department units of Neulamäki, Petonen, Varkaus and Suonenjoki. The contact person was sent an e-mail with the following attachments: a peer review form, a feedback questionnaire and a cover letter explaining the purpose of the study. The paper version of the questionnaire and the cover letter were posted on the bulletin board of each unit. In the analysis a non-parametric Mann-Whitney U test was utilized. There were only five respondents of the total of 52 who had completed the peer review process. This has to been taken into account when interpreting the results. However, the peer reviews were considered useful and increased learning and development. There was no consensus regarding whether the peer review increased motivation to give feedback or whether the peer review was suitable of in the context of first aid. These results may have been affected by low motivation to participate in the study and a lack of knowledge of the topic. Those who participated in the peer review process agree on that it should be continued in the future. Those who did not participate were more inclined that the peer review should not continue. The anonymity of the peer review did not raise opinions either for or against, but suggestions including constructive feedback were presented. In the future, feedback from peer reviews should be given as soon as possible by the manager. To increase continuity of this project, peer review shoud become routine in organizations. As a part of long-term development work, the evaluations from peer reviews could provide detailed information on the best practices to be encouraged in organizations.
