Tiivistelmä
Pro gradu -tutkielma koostuu Hengityskonepotilaan hoitotyö -mittarin kehittämistä kuvaavasta kirjallisesta osasta ja tieteellisen aikakauslehden julkaisuharkintaan otetusta tutkimusartikkelista (Soini Johanna, Koivula Meeri ja Joronen Katja: Hengityskonehoidossa olevan potilaan hoitotyön toteutuminen teho-osastolla sairaanhoitajien näkökulmasta). Pro gradu -tutkielman aineisto kerättiin neljän teho-osaston sairaanhoitajilta (n=138) kyselylomakkeella marras-joulukuussa 2013. Aineisto analysoitiin tilastollisin menetelmin.
Tutkielman kirjallinen osa kuvaa Hengityskonepotilaan hoitotyö -mittarin kehittämistä ja luotettavuuden arviointia. Työssä kuvataan systemaattisen kirjallisuuskatsauksen pohjalta Hengityskonepotilaan hoitotyö -mittarin teoreettinen rakenne, joka jakautuu kahteen osaan mitaten välitöntä ja välillistä hoitotyötä. Välitön hoitotyö sisältää kolme osiota, jotka mittaavat 1) Potilaan tarkkailua ja hoitotyön toimintoja, 2) Hengitysteiden hoitamista ja potilaan suojaamista ja 3) Hengityskoneesta vieroitusta. Välillinen hoitotyö sisältää myös kolme osiota: 1) Yhteistyö ja kommunikaation toteutuminen, 2) Perheen osallistuminen ja 3) Hoitotyön täydennys koulutus ja laadun arviointi.
Artikkelissa kuvattiin hengityskonehoidossa olevan potilaan välillisen ja välittömän hoitotyön toteutumista ja tutkittiin miten taustatekijät ovat yhteydessä hoitotyön toteutumiseen. Tutkimustulosten mukaan hengityskonehoidossa olevan potilaan hoitotyö toteutuu Suomessa melko hyvin näyttöön perustuvien suositusten mukaisesti sairaanhoitajien arvioimana. Parhaiten toteutuivat välillisen hoitotyön osa-alueista yhteistyö ja kommunikaatio sekä hoitotyön koulutus ja laadun arviointi. Heikoimmin toteutuivat välittömän hoitotyön osa-alueista hengitysteiden hoitaminen ja potilaan suojaaminen sekä välillisen hoitotyön osa-alueista perheen osallistuminen. Ohjeita lääkehoidon toteuttamisesta, hengitysteiden hoitamisesta ja VAP:n (hengityskonehoidosta aiheutuneen keuhkokuumeen) ehkäisystä käytettiin epäsäännöllisesti ja työntekijäkohtaisesti. Yliopistollisten - ja keskussairaaloiden välillä esiintyi eroja hengityskonepotilaan hoitotyön toteutuksessa.
Tutkimustuloksia voidaan hyödyntää hengityskoneessa olevien potilaiden hoitotyön kehittämisessä. Tutkimustulosten mukaan hengityskonehoidossa olevan potilaan näyttöön perustuvat hoitosuositukset tulisi ottaa käyttöön systemaattisemmin teho-osastoilla ja henkilökunnalle tulisi tarjota täydennyskoulutusta hoitosuositusten implementointiin. Myös perhehoitotyön kehittämisestä teho-osasto-olosuhteissa tarvitaan sekä koulutusta että arvo- ja asennekeskustelua. Lisää tutkimustietoa tarvitaan hengityskoneessa olevan potilaan näyttöön perustuvan hoitotyön toteutumisesta ja sen vaikutuksista potilaiden toipumiseen hengityskonehoitojaksosta.

This master´s thesis consists of a report of constructing Nursing care of ventilated patients´ - instrument and a scientific article that has been submitted for a review in a scientific journal (Soini Johanna, Koivula Meeri & Joronen Katja: Nursing care of patients on mechanical ventilation in intensive care unit from nurses´ perspective). Data was collected by a questionnaire from nurses from four intensive care units (ICU) (n=138) in November to December 2013. The data was analyzed statistically.
The report describes the development and the preliminary psychometrics of Nursing care of ventilated patients´ -instrument. The report describes the theoretical structure of the instrument, which was developed on the basis of a literature review. The instrument is divided into two parts, measuring direct and indirect patient care. Direct patient care consists of three areas measuring 1) Patient observation and nursing processes, 2) Airway management with patient covering and 3) Weaning from ventilator. Indirect patient care also consists of three areas measuring 2) Collaboration and communication, 2) Family participation and 3) Nursing education and quality control.
The scientific article described the nursing practice of mechanically ventilated patients´ direct and indirect nursing care, and we examined how the demographics were related to nursing practice. Study showed that nursing care for mechanically ventilated patients´ was realized quite well according to guidelines, in intensive care units in Finland from nurses´ perspective. Nursing care for mechanically ventilated patients´ was realized well in these indirect care areas: Collaboration and communication, and Nursing education and quality control. In terms of indirect care area, Family participation, and direct care area, Airway management with patient covering, were evaluated to actualize the weakest. Practice for medication-, airway management- and VAP (ventilator associated pneumonia) -prevention protocols were irregular and depended on individual nurses. There were differences in nursing practice of ventilated patients in university and central hospitals.
The results of this study can be utilized when developing the nursing care for mechanically ventilated patients in ICU. Study results showed that evidence-based practice for mechanically ventilated patients should be implemented more systematically in ICU´s, and units should provide education to nursing staff for implementing evidence-based practice. Developing family nursing in intensive care units needs both education and discussion of values and attitudes. Moreover, there is a need to study more evidence-based nursing practice for mechanically ventilated patients´, and how that relates to the patients´ recovery from ventilator care period.
