Tiivistelmä
Tämä pro gradu -tutkielma koostuu kirjallisuuskatsauksesta ja tieteellisen aikakauslehden julkaisuharkintaan lähetetystä artikkelikäsikirjoituksesta (Leino O, Kaunonen M, Koivula T & Aho AL: Erityislasten vanhempien kokemuksia palvelujärjestelmästä). Artikkelissa esitellyn tutkimuksen tarkoituksena oli kuvata erityislasten vanhempien kokemuksia palvelujärjestelmästä. Aineisto kerättiin erityislasten vanhemmilta ryhmähaastatteluilla (n=9). Aineisto analysoitiin induktiivisella sisällönanalyysillä.
Vanhemmat kokivat saamansa kohtelun vaihdellen perhelähtöisestä epäempaattiseen. Perhelähtöisyys koettiin vastavuoroisuutena, kokonaisvaltaisena kohtaamisena ja tarpeenmukaisten palveluiden toteutumisena. Epäempaattisena kohteluna koettiin epäasiallinen käytös, yhteistyöongelmat, kaltoinkohtelu, erilaisuuden ymmärtämättömyys ja välinpitämättömyys.
Kirjallisuuskatsauksen tarkoituksena oli kuvata erityislasten vanhempien kokemuksia lapsen terveydenhuollon palveluista ja tunnistaa tyytyväisyyteen yhteydessä olevia tekijöitä. Tavoitteena on tuottaa tietoa vanhempien kokemuksista terveydenhuollon palveluiden parantamiseksi. Aineisto koostui 17 tieteellisestä artikkelista vuosilta 2003—2013. Kirjallisuushaku tehtiin Medic-, Medline- ja Cinahl -tietokannoista ja hakua täydennettiin manuaalisesti. Aineisto analysoitiin induktiivisella sisällönanalyysillä.
Kirjallisuuskatsauksen mukaan erityislasten vanhemmat kokivat terveydenhuollon palvelut perhekeskeisinä, vanhemmuutta tukevina, perheen selviytymistä tukevina ja perheen voimavaroja kuormittavina. Vanhempien tyytyväisyys terveydenhuollon palveluista muodostui organisaation toimintaan ja inhimillisiin tekijöihin liittyvistä osa-alueista. Organisaation toimintaan sisältyivät terveyspalveluiden toteutukseen ja terveyspalveluiden järjestämiseen liittyvät asiat. Inhimillisiin tekijöihin kuuluivat perheen ominaispiirteet ja henkilökunnan ominaispiirteet.
Parents of children with special health care needs and their experiences with the Finnish service system
This Master’s thesis consists of a literature review and a manuscript submitted for publication (Leino O, Kaunonen M, Koivula T & Aho AL: Experiences of the parents’ with special needs children about the service system.) The mission of paper was to describe the experiences of the parents of children with special health care needs about the social service system in Finland. The source material was collected by conducting group interviews with the parents in question (n=9). The data was then analyzed by using inductive content analysis.
The reported experiences varied from receiving positive family centered service to enduring unempathetic treatment. The family centered treatment was perceived as experiencing reciprocity, overall encounter, and materialization of the required services. Lack of empathy, on the contrary, was experienced in terms of inappropriate behavior, problems with cooperation, mistreat of children, ignorance of abnormality, and insensitivity.
The purpose of the review was to describe the experiences of the parents of children with special health care needs about the public health care services for their child and to identify factors in connection to their satisfaction. The objective is to produce information about the parents’ experiences in order to improve the services of the public health care system. The research material consisted of 17 scientific articles from 2003—2013. The literature search was made from Medic, Medline and Cinahl databases and supplemented manually. The data was analyzed by using inductive content analysis.
According to the literature review, the parents of children with special health care needs experienced the services of the public health care system as being both family-centered, supporting their parenthood and empowering the family as a whole, as well burdening their resources as a family. Their satisfaction with the services of the public health care system consisted of sectors related to the organization’s operative and human factors. The operative factors included sectors related to the realization and arrangement of health services. The human factors included characteristics of both the family and the health care personnel.
