Tiivistelmä
Hyväksi muodostuva synnytyskokemus on naisen elämän kannalta merkityksellinen asia, sillä se vaikuttaa paitsi hänen ihmissuhteisiinsa ja vuorovaikutukseen vastasyntyneen kanssa myös hänen haluunsa tulla uudelleen äidiksi. Vaikean synnytyskokemuksen aiheuttama synnytyspelko, sen vaikutukset naisen mielenterveyteen ja perheen hyvinvointiin, nostavat synnytyskokemuksen käsittelyn tarpeelliseksi.
Tämän tutkimuksen tarkoituksena on kuvata naisten kokemuksia synnytyksestä sekä heidän kokemuksiaan Relaxbirth® synnytystuesta. Tutkimuksen tavoitteena on tuottaa tietoa, jonka avulla voidaan lisätä ymmärrystä niistä tekijöistä, jotka vaikuttavat tyytyväisyyteen tai tyytymättömyyteen synnytyksessä. Tutkimuksen tavoitteena on tuottaa myös tietoa, jota voidaan hyödyntää synnytysyksiköissä, joissa suunnitellaan Relaxbirth® synnytystuen käyttöönottoa. Saavutetun tiedon avulla voidaan kehittää synnytyksen hoitoa synnyttäjälähtöisemmäksi. Tutkimuksen lähestymistapa on laadullinen ja aineisto kerättiin kyselylomakkeen avoimen kysymyksen vastauksista vastikään synnyttäneiltä äideiltä. Tutkimukseen analysoitiin 250 Relaxbirth® synnytystuki tutkimukseen osallistuneen naisen vastaukset. Tutkimusaineiston analyysimenetelmänä käytettiin induktiivista sisällönanalyysiä.
Naisten kokemat syyt tyytyväisyyteen synnytyksessä muodostuivat fyysisesti helposta kokemuksesta, taidokkaasta ja positiivisesta henkilökunnasta, osallistavasta kanssakäymisestä, hyvinvoinnista huolehtimisesta, oman suoriutumisen onnistumisesta ja perhettä yhdistävästä kokemuksesta. Naisten kokemat syyt tyytymättömyyteen synnytyksessä muodostuivat fyysisen koskemattomuuden järkkymisestä, suunnitellusta poikenneesta synnytyksen kulusta, puutteista tiedollisessa ja taidollisessa tuessa, puutteista henkisessä ja tasavertaisessa kohtaamisessa, omien taitojen ja voimavarojen riittämättömyydestä sekä terveen lapsen syntymän ja perhettä yhdistävän kokemuksen vaarantumisesta. Naisten tyytyväisyyden kokemukset Relaxbirth® synnytystuesta liittyivät aktiivisen synnytyksen mahdollistumiseen ja positiivisen synnytyskokemuksen vahvistumiseen. Naisten tyytymättömyyden kokemukset Relaxbirth® synnytystuesta liittyivät puolestaan puutteisiin käytettävyydessä sekä odotettua vähäisempään hyötyyn.
Tulokset vahvistivat aiempia tutkimustuloksia ja osoittivat, että synnytykseen osallistuvan kätilön toiminta on avainasemassa vahvistamaan synnyttämässä olevan naisen positiivista kokemusta, sillä hyvä vuorovaikutussuhde sekä emotionaalinen, fyysinen ja tiedollinen tuki lisäävät naisen hallinnan tunnetta synnytyksessä. Myös oman kehon käytön mahdollistuminen ja kokemus oman suoriutumisen onnistumisesta vahvistavat naisen kokemusta synnytyksestä selviämisestä. Tietoa voidaan hyödyntää suunnitellessa odottavan ja synnyttävän perheen voimavaroja tukevia käytänteitä.
Women's experiences with childbirth and the Relaxbirth® birthing support
A good childbirth experience is very meaningful in a woman's life, as it will affect not only her personal relationships and interaction with her newborn child, but also her desire to have another child. The fear of childbirth that stems from a difficult delivery as well as its impact on the mental health of the woman and well-being of her family make examination of the childbirth experience a necessity.
The purpose of this study is to describe women's experiences with childbirth and the Relaxbirth® birthing support device. The goal of the study is to produce information which could be used to increase understanding of the factors affecting a woman's satisfaction or dissatisfaction with childbirth. It is also to produce information which could be used in delivery wards planning to use the Relaxbirth® birthing support. The information produced can be used to make maternity care more mother-oriented. A qualitative research approach was used for the study, and data was collected from open-answer survey responses given by mothers who had just given birth. The responses of 250 women participating in the Relaxbirth® birthing support study were analysed. Inductive content analysis was used as the method for analysing research data.
The reasons given by women for a satisfying childbirth experience include a physically easy delivery, skilled and positive staff, participative interaction, holistic care, a sense of accomplishment and an event that brings the family closer together. The reasons given by women for an unsatisfying childbirth experience include a sense of breach of physical integrity, the delivery taking a different course from the one planned, a lack of support in information and skills, shortcomings in emotional and equal treatment, a feeling of inadequacy in one's own skills and resources, and endangerment of the health of the child and an event that brings the family together. Women's satisfactory experiences with the Relaxbirth® birthing support involved making an active delivery possible and reinforcement of a positive childbirth experience. Women's unsatisfactory experiences with the Relaxbirth® birthing support involved shortcomings in utility and benefits that fell short of expectations.
The survey results confirmed previous research findings, demonstrating that the actions of the midwife participating in the delivery played a key role in reinforcing a positive experience for the mother, as a good interactive relationship and emotional, physical and informational support enhanced the mother's feeling of control in the childbirth process. In addition, making it possible for the mother to use her own body and a sense of personal accomplishment reinforce the mother's experience of coping with the demands of childbirth. This information can be used in planning practices that support the resources for an entire family expecting or giving birth to a child.
