Tiivistelmä
Opinnäyteyön tarkoituksena oli tarkastella sairaanhoitajavastaanottomallin soveltuvuutta yhteispäivystykseen ennen kaikkea lääkäriltä sairaanhoitajalle siirrettävien tehtävien osalta. Tavoitteena oli laatia esiselvitystyö, joka voisi toimia suunnittelun tukena Keski-Suomen sairaanhoitopiirin NOVA 'hankkeessa. Kerätyn aineiston pohjalta laadittiin kaksi potilastapausta, joiden hoidon kulku yhteispäivystyksessä prosessoitiin kaaviomuotoon. Sairaanhoitajan ja lääkärin välisiä tehtävänsiirtoja on toteutettu runsaasti ennen kaikkea perusterveydenhuollon vastaanotoilla mutta yhdenmukaiset, valtakunnalliset säännöt ja ohjeistukset ovat puuttuneet. Erikoissairaanhoidon osalta vastaanottotoiminta on edelleen hyvin lääkärivetoista, tehtävänsiirtoja on toteutettu melko vähän ja sairaanhoitaja on ollut lähinnä avustavassa roolissa. Kerätyn aineiston pohjalta todettiin, että terveydenhuollon toimintaa ohjaavat lait ja säädökset tulee saada ajan tasalle, vaatimus- ja vastuukysymykset tulee määritellä selkeästi. Tältä pohjalta voidaan suunnitella yhdenmukaiset ohjeistukset ja toimintamallit sekä pätevyysvaatimukset, joiden perusteella voidaan suunnitella riittävä koulutus. Tulevaisuudessa sairaanhoitaja voi päivystysvastaanotolla toimia potilaan ensikontaktina arvioiden hoidon kiireellisyyden joko puhelimessa tai henkilökohtaisen kontaktin kautta. Hän voi vastaanottaa potilaan, määrätä tarvittavat tutkimukset ja ohjata soveltuvat potilasryhmät hoitajavastaanotolle. Siellä hän voi tehdä tarvittavat hoitotoimenpiteet, määrätä sopivan lääkityksen ja ohjelmoida tarvittavan jatkohoidon ja seurannan. Edellytyksenä on lääkärikonsultoinnin mahdollisuus prosessin kaikissa vaiheissa. Tutkimusten ja selvitysten mukaan sairaanhoitajien toteuttama vastaanottotoiminta on sekä laadukasta että potilaiden hyväksymää. Se vapauttaa lääkäriresursseja vaativampaan potilastyöhön ja on näin ollen myös kustannustehokasta. The purpose of this study was to examine how the model of nurses' reception of patients is suited for the common emergency duty above all in transfer of duties from doctors to nurses. The objective was to draw up a preliminary study, which could help planning in NOVA-project of Central Finland health care district. From the basis of the collected material was drawn up two cases, which procedure of treatment in common emergency duty was processed to form of figures. There has been a lot of transfer of duties between the doctors and nurses aboveall in the receptions of public health service, but harmonized, nation-wide rules and regulations have been lacking. Concerning the special health service the reception is mostly conducted by doctors, transfer of duties has been carried out quite few and nurses have mostly been in assisting role. From the basis of the collected material was found out that the rules and regulations, which direct the actions of health care, should bring up to date and questions concerning demands and responsibilities should define to become clearer. Directions, operations modelsand qualifications required could be planned from the basis of the collected material. On grounds of them, adequate training could be planned. In the future nurse in emergency duty could be the first contact for patient byestimating the urgency of the treatment either in telephone or by personal contact. She/he could receive a patient, order examinations needed and direct suitable groups of patients to nurse's reception. In the nurses' reception of patients she/he could do necessary medical treatment, assign suitable medication and organize extension care and follow-up needed. Preconditions would be the possibility to consultation of the doctor in every stage of the process The research and settlements show that the reception work, which is done by nurses is both of a high quality and well accepted by patients. It releases the resources of doctors to more demanding work with patients and is such being the case also effective in costs.
