Tiivistelmä
Tutkimuksen tarkoituksena oli arvioida, onko automaattisen peritoneaalidialyysihoidon ohjaus (APD-hoidon ohjaus) riittävää ja potilaslähtöistä ja miten potilaat ovat selviytyneet kotona annetun ohjauksen turvin. Lisäksi tutkimuksessa verrattiin, onko yökoneiden tuote-edustajan ja hoitajan antamissa ohjauksissa eroja. Tutkimustehtävänä oli selvittää potilaiden kokemuksia APDhoidon ohjauksesta ja sen merkityksestä kotona selviytymiselle. Tutkimuksen tilaajana oli Keski- Suomen keskussairaalan sisätautiosasto 11. Tutkimukseen osallistui neljä potilasta, jotka olivat aloittaneet APD-hoidon vuoden 2006 aikana. Laadullinen tutkimus toteutettiin kyselyn ja teemahaastattelun avulla. Kyselyistä saatu aineisto teemoiteltiin ja teemahaastattelujen nauhoitettu aineisto analysoitiin sisällönanalyysillä. Tutkimustulosten mukaan ohjausta saatiin riittävästi ja se oli pääasiassa voimavarakeskeistä. Ohjauksessa toteutui hyvin emotionaalinen tunneilmaston luominen sekä tiedon antaminen. Potilaiden kompetenssin kunnioittaminen ja assertiivisuus jäivät kuitenkin puutteellisiksi. Kotona potilaat tunsivat aluksi epävarmuutta, mutta kokivat selviävänsä annetun tiedonturvin. Sillä, oliko ohjaajana ollut kone-edustaja vai hoitaja, ei näyttänyt olevan suurta merkitystä potilaiden ohjauskokemuksiin tai kotona selviytymiseen. Tulokset osoittavat APD-hoidon ohjauksen vahvuudet ja kehittämistä tarvitsevat alueet. Tuloksia voidaan käyttää APD-hoidon ohjauksen ja PD-hoidon ohjausmallin kehittämisessä Keski- Suomen keskussairaalan osastolla 11. Kehittämällä APD-hoidon ohjausta voidaan potilaan voimavaroja ja kotona selviytymistä tukea entistä paremmin. The aim of this study was to rate if the counselling of Automated Peritoneal Dialysis(APD) treatment is sufficient and patient-centered. And also to rate how patients have coped at home by means of the counselling they got. In addition the study compared if there are any differentials between counseling given by demonstrator of APD machine or a nurse. The purpose of this study was found out from the patients' experience of counselling of APD treatment and its meaning to cope at home. Subscriber of the study was the Medical Ward 11 of Central Hospital of Central Finland. Four patients took part in the study. They had begun the APD treatment during the year 2006. The qualitative study was carried out by the enquiry and the focused interview.The data from enquiries was sorted out by the themes and the data from the focused interviewswas analyzed by the method of content analysis. According to the findings of this study, counselling was got sufficiently and it was mainly felt as empowerment. The creation of emotional atmosphere and informing came true on counseling. However, the respect of patients' competence and the assertion of the patients came not completely true. In the beginning the patients felt ambivalence at home, but they still felt coping by means of the information. There were not significant effects to counseling experience and coping at home whether the instructor was the demonstrator or a nurse. The findings of this study show in which sectors of APD treatment counseling succeeded and which sectors need to be developed. The results can be used for the developing of counseling of APD treatment. In Medical Ward 11 in Central Hospital of Central Finland, patient's resources and coping at home can be supported better than before by developing the counseling of APD treatment.
