Tiivistelmä
Tasa-arvoinen hoitotyö edellyttää hoitoammattilaisilta kulttuurikompetenttia hoitotyön hallintaa. Tämän opinnäytetyön tarkoituksena on kuvata pakolaistaustaisen maahanmuuttaja-asiakkaan omahoidon ohjaukseen vaikuttavia tekijöitä hoitajien kokemana. Tavoitteena on tuottaa tietoa ja lisätä tietoisuutta kulttuurikompetentista hoito-otteesta omahoidon ohjauksessa sekä myötävaikuttaa hoitajan ja maahanmuuttaja-asiakkaan kohtaamisiin kartoittamalla kokemuksia edistämään kulttuurisen osaamisen kehittymistä terveydenhoitotyössä.
Teoreettisena viitekehyksenä oli Papadopouloksen, Tilkin ja Taylorin (2006) malli prosessinomaisesta kulttuurisen kompetenssin kehittymisestä. Empiirinen aineisto hankittiin teemahaastattelun avulla haastattelemalla sairaanhoitajia (n=6) terveysasemilla huhtikuussa 2021. Haastattelun kysymykset muodostettiin mallin osa-alueiden tietoisuus, tieto, sensitiivisyys ja kompetenssi avulla. Aineisto analysoitiin teemoittelemalla.
Etniset kohtaamiset koettiin haastavaksi. Hoitajilla oli kulttuurista herkkyyttä ja tietoisuutta, mutta tietoa hoitotyön tueksi puuttui. Negatiivisia ajatuksia ja epävarmuutta esiintyi, yleistä negatiivista suhtautumista ei kuitenkaan tullut esiin. Positiivisemmin maahanmuuttajista ajattelevat omasivat korkeamman kompetenssin tason, arvioivat onnistuvansa ohjaustilanteissa ja kohtasivat vähemmän kulttuurisia ongelmatilanteita vastaanotoillaan. Kulttuurisidonnaiset sairaus- ja terveyskäsitykset ilmenivät lääkkeettömien hoitojen toiveena, syy-seuraussuhteiden selityksinä, vaikeutena ymmärtää biologista sairauskäsitettä ja pientenkin vaivojen hoidontarpeena.
Tutkimuksen tuloksiin ja aiempaan tietoon perustuvien johtopäätösten mukaan sairaanhoitajan kulttuurisen kompetenssin tasolla on vaikutusta omahoidon ohjaukseen maahanmuuttajatyössä. Kulttuurisesti kompetentti hoitaja voi antaa hyvää hoidon ohjausta sekä vastata paremmin kulttuurisidonnaisiin terveys- ja sairauskäsityksiin. Avoin asenne vaikuttaa myönteisesti monikulttuurisen hoidon sujuvuuteen. Tutkimus onnistui kartoittamaan monikulttuuristen asiakkaiden ohjaamiseen tarvittavia taitoja. Tietoa voidaan hyödyntää kehitettäessä hoitajien ja eri kulttuureista tulevien asiakkaiden vuorovaikutusta, hoitoa ja koulutusta. The objective of this thesis is to describe the experiences of nurses on taking care and giving guidance to immigrant patients with refugee background. The aim is to produce
knowledge and descriptive information on how nurses carried out and personally experienced multicultural encounters, as well as to describe how they were able to interact with the ethnically diverse patients in a more culturally competent way. One aspect of the study was the patient-nurse contacts and their contribution to the level of professional learning.
The reference frame of this thesis was the developing model for health care professionals’
cultural competence by Papadopoulos, Tilki and Taylor. This process-based model incorporates a four-dimensional view on cultural competence: awareness, knowledge, sensitivity and competence. The questions for the theme interviews were constructed on the basis of the said model. The research data was collected in spring 2021 through theme interviews. Health center nurses were interviewed (n=6). The research data was analyzed through sectioned themes. Based on the results of the interviews it is possible to assess the cultural competence of the nurses and what effect it has on the selfcare instructions given to immigrant refugee patients.
Being in professional contact with ethnic patients was deemed quite demanding by the
nursing staff. The nurses were culturally sensitive and aware, but some lacked the necessary cultural knowledge to perform professionally in care and guidance. Only few of the interviewed nurses were competent specialists in this field. Some nurses considered themselves insecure with immigrant refugee patients and had negative expectations of the care situations. However, no general negative outlook towards the immigrant refugee patients was present. The nurses who were more positive about the patients were also equipped with higher cultural competence. They estimated themselves competent in giving care instructions. The culture related views of immigrant refugee patients concerning illness and health presented themselves in requests of nonmedicinal care, vivid cause-and-effect -relations and in difficulties of understanding the biological aspect of diseases.
This study supports the results from earlier information-based research according to which the level of cultural competence of nurses has an effect on the reception of instructions on self-management of illness and health in immigrant patients. A nurse with cultural competence can give positive and responsible guidance and is able to respond better to the culture related views on health and illness, which affect the self-management of immigrant refugee patients. Open approach of the caretaker contributes to the success of health care appointments of multicultural clients. This thesis was able to increase the knowledge of the skills required to guide and instruct multicultural clients in health care. The knowledge can be applied to the development of interaction, care and training of nurses, who meet clients with different cultural backgrounds.
