Tiivistelmä
Kun terveydenhuollon yksikössä otetaan käyttöön uutta teknologiaa, täytyy käyttöönotto toteuttaa hallitusti ja arvioida teknologian hyötyjä useasta näkökulmasta. Hoitoteknologioiden arvioinnin periaatteiden mukaisesti voidaan esim. tarkastella teknologian vaikuttavuutta hoitoon, tai kustannuksiin. Teknologian hyväksymismallien ymmärtämisen kautta voidaan myös vaikuttaa ratkaisun sujuvaan käyttöönottoon.
Tässä opinnäytetyössä pilotoitiin Tyks Neurokeskuksessa Monidor nestehoidon etävalvontaratkaisua kahdella vuodeosastolla. Opinnäytetyön tutkimuksessa selvitettiin useita näkökulmia, kuten säästääkö ratkaisu hoitajien työaikaa, onko ratkaisu helppokäyttöinen ja miten se vaikuttaa potilasturvallisuuteen ja hoitajien työnkulkuun.
Tutkimus toteutettiin haastatteluilla sekä sähköisellä palautekyselyllä. Tutkimuksessa osoitettiin, että Monidor nestehoidon etävalvontaratkaisu säästää hoitajien työaikaa. Ajansäästö toteutuu mm. vähenevillä rutiinikäynneillä potilashuoneissa. Ratkaisun todettiin myös vaikuttavan positiivisesti potilaan hoitoon, sillä sen avulla voidaan reagoida oikea-aikaisesti mahdollisiin ongelmiin nestehoidossa.
Ratkaisun hyödyt tulivat erityisesti esille öisin ja eristyspotilaiden hoidossa. Rutiinikäyntien vähentäminen öisin parantaa potilaan kokemusta sairaalassa. Käyntien vähentäminen eristyshuoneessa taas säästää suojamateriaalikustannuksia.
Monidor nestehoidon etävalvontaratkaisu hyväksyttiin hyvin. Tutkimus noudatti tyypillistä pilottia. Alussa ratkaisun käyttöä hieman arasteltiin, mutta käytön kautta saatujen positiivisten kokemusten kautta ratkaisun hyödyt tulivat selväksi ja käyttö lisääntyi.
Tutkimuksen aikana osoitettiin Monidor nestehoidon etävalvontaratkaisun laajennuspotentiaali muille Tyksin toimialueille. Ennen laajennuksia täytyy keskustella muutamista asioista etukäteen, kuten sopia mitä lääkkeitä potilaalle voidaan antaa ratkaisun avulla. Tässä tutkimuksessa käytöstä rajattiin pois tiettyjä lääkkeitä. Osaa noista lääkkeistä kuitenkin annetaan muissa organisaatioissa potilaille Monidor nestehoidon etävalvontaratkaisua käyttäen. Tyksissäkin voidaan siis keskustella mahdollisesta ratkaisulla annettavien lääkkeiden laajennuksesta. When new technology is initialized in a healthcare unit, it must be done with control and the benefits of the technology must be assessed from many sides. With methods of health technology assessment one can assess for example the effectiveness for patient care, or the costs. Through understanding technology acceptance models one can positively impact the initialization.
In this thesis the Monidor intravenous therapy remote monitoring solution was piloted in two wards in Tyks Neurocenter. In the study of this thesis multiple viewpoints were examined, such as the possible time saving of nurses, is the solution easy to use and how it affects patient safety and the nurses’ workflow.
The methods of the study were interviews and a digital questionnaire. In the study it was shown that Monidor intravenous therapy remote monitoring solution saves nurses’ time. The time saving comes true for example with fewer routine visits in patient’s rooms in the ward. The solution was also shown to affect patient’s care positively, because when using it, one can react at the right time for the possible problems that can occur in intravenous therapy.
The benefits of the solution were especially shown during night and in the care of isolated patients. Reducing routine visits to the patient’s room during night enhances the patient’s experience while in the hospital. Reducing routine visits to isolation rooms saves protective material costs.
Monidor intravenous therapy remote monitoring solution was accepted well. The study went through like the usual pilot does. At first the nurses didn’t dare use the solution as much. But through positive experiences received from using the solution, the benefits became clear, and thus the solution was used more.
During the study the potential of expanding Monidor intravenous therapy remote monitoring solution to other service divisions of Tyks was shown. Before expanding the solution, some things must be discussed in advance, such as what medicines are allowed to be given to patients with the solution. In this study some medicines were not allowed to be given to patients with the solution. Some of those medicines are given to patients in other organizations using the solution. This means that in Tyks it’s possible to discuss if more medicines would be allowed to be given to patients using the solution.
