Tiivistelmä
Eturauhasten magneettiohjatut korkeaintensiteettiset ultraäänihoidot ovat varsin uusi hoitomuoto eturauhassyövälle ja eturauhasen liikakasvulle. Kuvantamisalan jatkuva kehittyminen korreloi röntgenhoitajan roolin kehittymistä ja työtehtävien lisääntymistä, ja näin myös osaamisen lisääntymistä.
Tämän opinnäytetyön tavoitteena on tehdä näkyväksi magneettikuvantamisohjattujen eturauhasten korkeaintensiteettisten ultraäänihoitojen röntgenhoitajalta edellytettävää erityistä osaamista. Tämän opinnäytetyön tarkoituksena on kuvata röntgenhoitajan toimintaa näissä hoidoissa.
Opinnäytetyö sisältää teoreettisen viitekehyksen, jossa käsitellään eturauhasen anatomiaa, eturauhassyöpää ja eturauhasen liikakasvua, sekä näiden hoitomenetelmiä, ablaatiohoitoja korkeaintensiteettisellä ultraäänellä ja jo tutkittua tietoa röntgenhoitajan osaamisesta ja sen eri ulottuvuuksista.
Aineisto kerättiin fyysisillä tiedonkeruulomakkeilla Turun yliopistollisen keskussairaalan osastolta, jossa näitä hoitoja tehdään. Tiedonkeruulomakkeiden kysymykset perustuivat tutkimuskysymykseen ja kysymykset esitettiin hoitoprosessin eri vaiheista mahdollistaen hoitoprosessin kokonaiskuvan luomisen röntgenhoitajan näkökulmasta. Tiedonkeruulomakkeisiin vastasi viisi (5) röntgenhoitajaa, joilla kaikilla oli sama työkokemuksen määrä näistä hoidoista.
Eturauhasen magneettiohjatuissa korkeaintensiteettisissä ultraäänitoimenpiteissä erityinen osaaminen, mitä röntgenhoitajalta edellytetään, on osata käyttää hoidolle spesifistä hoitolaitteistoa, toimia osana moniammatillista tiimiä koko hoitoprosessin aikana, sekä ymmärtää kystofix-katetrin toiminnan ja avustaa sen, ja hoitolaitteiston asennuksessa potilaaseen. Opinnäytetyö tuo ilmi vain yhden esimerkin siitä, miten laaja röntgenhoitajan todellinen osaaminen on verraten siihen, mitä röntgenhoitajan ammatista yleisesti ajatellaan. The MRI-guided high-intensity ultrasound treatments of the prostate are a relatively new form of treatment for prostate cancer and prostatic hyperplasia. The constant progress made in medical imaging correlates in the development of a radiographer’s role and new work tasks, thus increasing radiographer’s competence in their profession.
The objective of this thesis is to make visible the specific competence required from radiographers in the MRI-guided high-intensity ultrasound treatments of the prostate. The purpose of this thesis is to document radiographers’ actions during these treatments.
This thesis contains a theoretical frame that includes information about the anatomy of a prostate, prostate cancer and prostatic hyperplasia, and the treatment methods used for them, information of ablation treatments using high-intensity ultrasound, and the studied information of radiographer’s competence and its dimensions.
The data for this thesis was collected using physical questionnaires that were distributed to the department where the treatments were executed in Turku university hospital. The questions in the questionnaires where based on the research question and presented in the steps of the treatment process to create an overall picture from the point of view of the radiographers. The information was collected from five (5) radiographers, who all had the equal amount of experience in these treatments.
The data shows that the specific competence required in MRI-guided high-intensity ultrasound treatments of the prostate are knowledge of how to use and work the specific treatment system used in these treatments, how to work in a multi-professional team during the whole treatment process, and to understand how a cystofix catheter works and how to assist doctors during the placement of the catheter and the treatment system in the patient. This thesis brings forth only one example of the extensiveness of competence that radiographers possess compared to what the general knowledge of the profession might indicate.
