Tiivistelmä
Laadukkaalla astmaselvityksen teolla on mahdollista parantaa astman hoitotasapainoa oikein suoritetun PEF-mittauksen ja lääkkeenoton kautta. Hoitotasapainon ollessa hyvä, siitä johtuvat aikuisten työ- ja lasten koulu- sekä hoitopäiväpoissaolot vähenevät ja syntyy kustannussäästöjä niin työnantajille kuin yhteiskunnalle. Opinnäytetyön tarkoituksena oli tutkia, kuinka mobiilisovellus Astmatti vaikuttaa lasten astmaselvityksen tekoon ja millaisena apuvälineenä sovellus koetaan astmaselvityksen teossa vanhempien näkökulmasta. Opinnäytetyön tavoitteena oli selvittää, kuinka mobiilisovellus Astmatti vaikuttaa lasten kykyyn suorittaa PEF-mittaus, PEF-kirjaus sekä keuhkoputkia avaavan lääkkeenotto jauheinhalaattoria käyttäen oikealla tekniikalla. Työn toimeksiantaja oli Ylä-Savon SOTE kuntayhtymä. Opinnäytetyön tutkimus tehtiin yhdessä Ylä-Savon SOTE kuntayhtymän lastentautien vastaanoton kanssa. Toimeksiantajan pyynnöstä tutkimuksen tulokset salattiin ja osaa opinnäytetyöstä ei julkaista.
Opinnäytetyö toteutettiin kvantitatiivisena tutkimuksena. Tutkimuksen aineisto koostui Ylä-Savon SOTE kuntayhtymän lastentautien vastaanoton kymmenestä lapsipotilaasta, joista viisi käytti mobiilisovellus Astmattia ja viisi ei käyttänyt. Tutkimusryhmän PEF-puhallustekniikkaa sekä lääkkeenottotekniikkaa arvioitiin sairaanhoitajan vastaanotolla kaksi kertaa. Ensimmäisen havainnointikerran jälkeen viisi tutkimusryhmän jäsentä otti käyttöön Astmatti- sovelluksen ja viisi jäsentä sai perinteisen kirjallisen ohjeistuksen astmaselvityksen tekoon. Toisella sairaanhoitajan havainnointikerralla tarkasteltiin PEF-puhallustekniikkaa, lääkkeenottotekniikkaa sekä kahden viikon seurantajakson PEF-kirjauksia. Lisäksi Astmatti -sovellusta käyttäneiden lapsien huoltajat täyttivät kyselylomakkeen mobiilisovelluksen käytöstä. Lapsille oli oma lomake Astmatti-sovelluksen käyttöön ja käytön miellyttävyyteen liittyen.
Astmatti -sovellusta käyttäneiden ikäjakauma oli 8 – 10 vuotta ja ei Astmattia käytössä olevien ikäjakauma 11 – 13 vuotta. Ensimmäisellä käynnillä yhdellä osallistujalla oli virhe PEF-puhalluksissa, toisella käynnillä kukaan ei tehnyt siinä virhettä. Kyselylomakkeen tulosten perusteella huoltajat kokivat, että voivat vastaanottaa astmaohjausta tarvittaessa mobiilisovelluksen avulla. Lasten mielestä sovellusta oli helppo käyttää, mutta kaikkien mielestä ohjevideot eivät olleet hauskoja.
Mobiilisovelluksen käyttö ei korvaa vanhempien ohjausta. Sovelluksesta saatavat ohjeet ovat hyvää pohjustusta lapselle oikeanlaisen PEF-puhallus- ja lääkkeenottotekniikan oppimiseen, mutta lisäksi tarvitaan reaaliaikaista ohjausta ja neuvontaa. Vanhempien valvonta ei saa vähentyä, vaikka ohjauksessa otettaisiin mobiilisovellus apuvälineenä käyttöön.
Tämän tutkimuksen toteutusta ja tuloksia voi hyödyntää mahdollisessa Astmatin jatkotutkimuksessa. Jatkotutkimuksen teoriapohja voisi koostua esimerkiksi potilasohjauksesta ja siitä, millaiset ohjeet vanhemmille ja eri ikäisille lapsille tulisi laatia, jotta se tukisi parhaalla mahdollisella tavalla laadukkaan astmaselvityksen tekoa. A high asthma examination quality has the potential to improve the balance in asthma treatment with correctly performed PEF-measuring and self-medication practices. A balanced treatment reduces absences from work and school or daycare for adults and children, respectively, creating savings to both employers and society. The purpose of this thesis was to study how the effects of the mobile application “Astmatti” on asthma examination and parents’ perceptions of the use of this application in the examination. The aim of this thesis was to find out how the mobile application Astmatti affects children's ability to perform PEF -measurement, PEF -noting and bronchodilator medication using a powder inhaler with the right technology.The client organisation of the thesis was Ylä-Savon SOTE and collaborated with the pediatric reception of Ylä-Savon SOTE. At the request of the client, the results of the investigation are encrypted and part of the thesis is not published.
This thesis was conducted as a qualitative research. The study group was comprised of 10 patients of the pediatric reception of Ylä-Savon SOTE, where five children used the mobile application Astmatti and five children did not use the application. The PEF- blow technique and drug intake technique of the study group were observed two times at a nurse’s appointment. After the first observation event, five group members started the use of the Astmatti application and five group members received conventional written instructions on asthma examination. The PEF-blow technique, drug intake technique and PEF-notes from a two-week following period were observed at the second appointment with a nurse. Additionally, the parents of the children that used the Astmatti application filled in a questionnaire on their experiences of the application, and the children using Astmatti filled in a separate questionnaire on the comfortability of the application.
The ages of the study group members using Astmatti were 8 – 10 years and the ages of the members that were not using the application were 11 – 13 years. At the first observation event, one of the group members had an incorrect PEF -blow technique, while on the second observation event, all group members had a correct technique. The questionnaires showed that parents were willing to receive directions for asthma treatment through a mobile application. All children perceived the use of the application as effortless, yet not all of the children felt that the instruction videos were funny.
The use of the mobile application does not replace the instructions of parents. The instructions from the application serve as a good foundation for the children to learn the correct PEF-blow technique and drug intake technique but simultaneous instructions and advice are also required. Thus, the importance of parents’ supervision should not be overlooked even if a mobile application is used to aid the guidance.
The implementation and results of this study may be utilized in potential further studies on Astmatti. Future research could potentially focus on the guidance of the patient, for example what type of instructions should be provided for the parents and children of various ages in order to result in a high quality asthma examination.
