Tiivistelmä
Lapsi kehittyy ensimmäisen kahden elinvuotensa aikana nopeasti, ja vanhempien huoli lapsen hyvinvoinnista on suuri. Kun lapsen motorinen kehitys ei etene odotetusti, tulee lasta arvioida huolellisesti soveltuvin menetelmin. Lasten fysioterapeuttien käytössä on useita kehityksen arvioinnin menetelmiä, joista Hammersmithin menetelmää kliinisessä työssä käyttää Suomessa vain harva. Hammersmithin menetelmä on systemaattinen neurologinen arviointimenetelmä 2–24 kuukauden ikäiselle lapselle, jolla arvioidaan neurologista ja motorista kehitystä strukturoidun tutkimuslomakkeen avulla. Menetelmän pisteytettävää ensimmäistä osaa voidaan käyttää apuna CP-vamman varhaisessa diagnosoinnissa.
Opinnäytetyön tarkoitus oli lisätä lasten kanssa työskentelevien fysioterapeuttien ja muiden ammattilaisten tietoutta Hammersmithin menetelmän käytöstä. Tavoitteena opinnäytetyössä oli tutkia kuka Hammersmithin menetelmää voi käyttää, ja millaisia käyttötarkoituksia sillä on. Ensisijaisesti opinnäytetyössä tarkasteltiin Hammersmithin menetelmän käyttöä aineistossa, ei niinkään aineistosta nousseita tuloksia. Opinnäytetyö toteutettiin integroivana kirjallisuuskatsauksena, jossa kolmesta tietokannasta aineistoksi valittiin sisäänotto- ja poissulkukriteerien mukaan 12 tutkimusta. Aineistoa analysoitiin aineistolähtöisellä sisällönanalyysilla käyttäen luokittelua ja teemoittelua.
Aineistossa esille nousseita luokkia ja teemoja olivat käyttötarkoitus, tutkijan ammatti, tutkittavan ikä sekä luotettavuus. Aineiston perusteella Hammersmithin menetelmää käytettiin monipuolisesti erilaisilla kohderyhmillä kehityksen arviointimittarina. Lisäksi sen korrelaatiota kuvantamiseen ja sen ennustavuutta tutkittiin. Aineistossa menetelmää käyttivät lääkärit sekä fysioterapeutit ja tutkittavien ikä vaihteli 6 kuukaudesta 24 kuukauteen. Hammersmithin menetelmän luotettavuuden pohtiminen aineistossa oli puutteellista. Children develop rapidly during the first two years of their life, and parents are greatly concerned about the well-being of their child. In instances where the motor development of the child does not proceed as expected, the child should be assessed accordingly. Pediatric physiotherapists have a variety of assessment methods available to them, of which the Hammersmith Infant Neurological Examination (HINE) is not widely in clinical use in Finland. The HINE is a systematic neurological assessment method for children of 2—24 months of age. It evaluates the neuromotor development using a specific proforma. The first scorable section of the HINE can be used in early diagnosis of Cerebral Palsy.
The purpose of the thesis was to increase the knowledge of pediatric physiotherapists and other professionals about the usage of the HINE. The aim of the thesis was to recognize who can use the HINE and what purposes of use it has. The use of the HINE in the chosen literature was examined, rather than discussing the results of the studies. The thesis was conducted as an integrative literature review. 12 studies were chosen from three databases according to inclusion and exclusion criteria. The data was analyzed by a material-driven content analysis using categorization and thematization.
The categories and themes identified from the material were purpose of use, the profession of the examiner, the age of the examined child and the reliability of the HINE. According to the material, the HINE was used in assessing neuromotor development in various study groups. Furthermore, the studies examined the correlation of the HINE to ultrasonography as well as the prognostic value of the method. The HINE was used by doctors and physiotherapists. The age of examined children varied from 6 to 24 months of age. The evaluation of the reliability of the assessment method in the studies was lacking.
