Tiivistelmä
Sairaalapotilaan unen tukeminen on keskeinen osa hyvää hoitoa. Sairaalassa unta häiritsevät monet tekijät, joihin hoitohenkilökunta voi omalla toiminnallaan vaikuttaa. Opinnäytetyö liittyy Satasairaalassa käynnissä olleeseen perushoidon kehittämishankkeeseen (STEPPI).
Potilaan kokemuksen huomioiminen on tärkeä osa näyttöön perustuvaa toimintaa. Tämän opinnäytetyön tarkoituksena oli kuvata niitä tekijöitä, jotka potilaiden kokemuksen mukaan häiritsevät heidän untaan sairaalassa sekä kuvata potilaiden kokemuksia unen tukemisesta sairaalassa. Integroivalla kirjallisuuskatsauksella koottiin aiempi tutkimusnäyttö potilaan unta häiritsevistä tekijöistä. Sen lisäksi analysoitiin deduktiivisella sisällön analyysilla aiemmin kerätty haastatteluaineisto, jonka avulla saatiin tietoa potilaiden kokemuksista unen tukemisessa.
Kirjallisuuskatsauksen perusteella potilaan unta sairaalan vuodeosastolla häiritsevät potilaan kokemat oireet ja tunnetilat, yksilöllisyyden puute, sairaala ympäristönä sekä sairaalan ympärivuorokautinen toiminta. Potilaat kuvasivat hoitajien toimintaa unen tukemiseksi liittyen kivun ja sairauden oireiden hoitamiseen, hoidon toteutukseen liittyvän epämukavuuden huomiointiin, osaston ympärivuorokautisesta toiminnasta aiheutuvien häiriöiden ehkäisemiseen, potilashuoneen huolehtimiseen hyväksi nukkumisympäristöksi sekä vuorokausirytmin sekoittumisen ehkäisemiseen. Selkeä puute hoitajien toiminnassa oli potilaiden tukeminen voimakkaissa tunnetiloissa.
Potilaiden henkisen hyvinvoinnin tukeminen vuodeosastolla on tärkeä osa unen tukemista sekä kokonaisvaltaista hoitoa ja jatkossa siihen on syytä kiinnittää enemmän huomiota. Tarvittaessa hoitajien osaamista voidaan lisätä esimerkiksi vuorovaikutuskoulutuksella. Jatkotutkimusaiheena kvantitatiivinen lähestyminen potilaiden unen tukemiseen vuodeosastolla voisi syventää ymmärrystä aiheesta. Sleep promotion in an essential part of patient care. Several things can disturb sleep in hospital and nursing staff can promote the patients’ sleep in many ways. This study was made as a part of STEPPI-project which aimed to improve evidence-based nursing and basic nursing care, running in Satasairaala.
The purpose of this study was to describe the factors disturbing sleep in hospital and the patients’ experiences on sleep promotion. An integrative review was conducted to determine the factors disturbing the patients’ sleep in hospital. Also, data from previously made patient interviews was analyzed by deductive content analysis. The data from the analysis gave information how the nursing staff had promoted the patients’ sleep.
According to the integrative review the patients’ sleep was disturbed by the patients’ symptoms and emotions, lack of individuality, hospital environment and round-the-clock activity in hospital. According to the data from the interviews the nursing staff had promoted the patients’ sleep in many ways and identified the factors disturbing the patients’ sleep mostly well. The patients described nursing interventions for sleep promotion related to treating pain and other symptoms, minimizing disturbance related to nursing activity and the round-the-clock activity in the ward, arranging the room for a good sleeping environment and an effort to avoid the disturbance of the circadian rhythm. However, the nursing staff had a deficiency in supporting the patients with their emotional conditions.
Supporting the patients’ emotional well-being at the hospital is important for sleep promotion and also an essential part of holistic care. In the future it should be paid more attention to. If needed, the nursing staff’s communication skills could be improved by further education. For future studies it is recommended to take a quantitative approach to patients’ experiences on sleep promotion which would give a better understanding on the subject.
