Tiivistelmä
Turun yliopistollisen keskussairaalan tekonivelkirurgian vastuualueella muodostui tarve tehokkaamman tekonivelpotilaiden leikkaukseen valmistavan prosessin kehittämisestä ja sen kuvauksesta. Terveydenhuollon yhdeksi laadun ulottuvuudeksi määritellään prosessit ja niiden sujuvuus. Potilaan näkökulmasta tämä tarkoittaa muun muassa sitä, että hän saa asianmukaisen hoidon ilman tarpeettomia viivästyksiä tai päällekkäisyyksiä. Terveydenhuollon yksikön näkökulmasta tämä taas merkitsee riittävää ammattitaitoista henkilökuntaa sekä potilaan hoidon edellyttämää muuta riittävää resurssia.
Lean on tuttu teollisuuden toimialalla mutta terveydenhuollon palveluprosessien kehittämisessä Lean on ilmiönä vielä melko uusi. Usein Leania on hyödynnetty terveydenhuollossa yksittäisten hankkeiden toteuttamisessa, prosessien kuvaamisessa sekä kehittämisessä. Kuitenkin juurtuakseen organisaation toimintatavaksi on Leanin oltava osa organisaatiokulttuuria, jossa Lean-arvot ja periaatteet näkyvät jatkuvan parantamisen ajatuksena.
Tämän kehittämisprojektin tavoitteena oli kehittää tekonivelleikkaukseen tulevan potilaan polikliinistä palveluketjua niin, että potilaan preoperatiivinen valmistelu voidaan toteuttaa prosessin aikaisemmassa vaiheessa sekä hyödyntää enemmän etähoitoa siihen soveltuvilla potilailla. Tarkoitus oli selvittää Tyks Orton poliklinikan tekonivelprosessin nykytilaa Lean-menetelmiä hyödyntäen. Toisena tarkoituksena oli sujuvoittaa tekonivelpotilaan polikliininen palveluketju ensikäynnistä leikkauspäivän aamuun.
Kehittämisprojektin lähestymistapa oli toiminnallinen, jossa näkyi kehittämisprosessin lineaarisuus ja syklisyys. Menetelmien valinnan taustalla vaikutti Lean, henkilökunnan osallistaminen sekä fasilitointi. Keskeistä työssä oli löytää menetelmiä, joissa keskitytään ongelmien ratkaisuun ja prosessin toiminnan kehittämiseen jatkuvan parantamisen ajatuksella.
Kehittämisprojektin tuotoksena syntyi tekonivelpotilaan sujuvampi preoperatiivinen palveluketju, jossa rutiininomaiset käynnit vähenivät, työtä vakioitiin sekä potilas sai asianmukaisen leikkaukseen valmistavan hoidon ilman tarpeetonta odottelua. There was a need to develop and describe a more efficient process for preparing for artificial joint surgery at Turku University Hospital’s arthroplasty surgery unit. One of the dimensions of health care quality is defined as processes and their smoothness. From the patient's point of view, this means, among other things, that he or she receives appropriate treatment without unnecessary delays or duplication. From the point of view of the health care unit, this, in turn, means sufficient professional staff as well as other adequate resources required for the care of the patient.
Lean is familiar to the industry, but Lean is still a fairly new phenomenon in the development of healthcare service processes. Lean has often been used in healthcare to implement individual projects, describe processes and develop them. However, to take root in the way an organization operates, Lean must be part of an organizational culture where Lean values and principles are reflected in the idea of continuous improvement.
The aim of this development project was to develop an outpatient service chain for a patient undergoing arthroplasty surgery so that the patient's preoperative preparation can be carried out at an earlier stage of the process and more remote care can be utilized for suitable patients. The purpose was to find out the current state of the arthroplasty surgery process in the Tyks Orto outpatient clinic using Lean methods. The other purpose was to streamline the arthroplasty surgery patient’s outpatient service chain from the first visit to the morning of the day of surgery.
The approach of the development project was functional, reflecting the linearity and cyclicality of the development process. The choice of methods was influenced by Lean, staff involvement and facilitation. Central to the work was finding methods that focus on solving problems and developing the operation of the process with the idea of continuous improvement.
The result of the development project was a smoother preoperative service chain for the arthroplasty surgery patient, where routine visits were reduced, work was standardized, and the patient received appropriate pre-operative treatment without unnecessary waiting.
