Tiivistelmä
Opinnäytetyön toimeksiantajana oli TYKS:n lasten klinikan lasten ja nuorten päivystyspoliklinikka. Tavoitteena on tuoda uusia näkökulmia lasten ja nuorten päivystyspoliklinikalla työskentelevien sairaanhoitajien ammattitaitoon lapsen kohtaamisessa. Tarkoituksena oli kartoittaa lapsen kohtaamista päivystyksessä ja tulosten pohjalta luotiin strukturoitu kyselylomake yli 9-vuotiaille lapsille. Tulevaisuudessa kyselylomakkeen avulla voidaan tulevaisuudessa kerätä lisätietoa lapsen kohtaamisesta päivystyksessä lapsen näkökulmasta. Opinnäytetyö tehtiin kuvailevana kirjallisuuskatsauksena (n=24). Aineisto analysoitiin sisällönanalyysiä hyödyntäen.
Lapsen kohtaamisen onnistuminen päivystyksessä on monien tekijöiden summa. Siihen vaikuttavat lapsen aikaisemmat sairaalakokemukset, mielentila, huoltajat, hoitohenkilökunta sekä sairaalaympäristö. Terveydenhuollon ammattilaisten käytöksen tulisi myötäillä tunnepuolen tuen antamista lapselle ja rehellisen tiedon jakamista. Sairaanhoitajalle lapsen kohtaaminen on tietynlainen haaste, koska kohtaaminen ja toteutettava hoito on oltava sopusoinnussa lapsen kehitystason ja iän mukaan. Sairaanhoitajan on kohdattava lapsi täysivertaisena potilaana, huomioida lapsen mielipiteet, kuunnella lasta ja vastata tämän kysymyksiin. Sairaalaympäristön luotettavuutta lisäävät rauhallisuus, ystävällisyys, lapsen kehitystasoon sopivat lelut ja lapsen mukaan ottaminen hoitoon. Päivystyksen toiminta koostuu hoidon tarpeen kiireellisyyden arvioon, perusterveydenhuollosta ja erikoissairaanhoidosta sekä puhelinneuvonnasta. Lapsen päivystyskäynti saattaa olla myös huoltajalle vaikea ja huolettava kokemus. Huoltajien mukaanotto lapsensa hoitotoimenpiteisiin ja rehellinen informointi lisäävät heidän luottamustaan hoitohenkilökuntaan.
Lasten hoitotyön ammattilaisen on tärkeää tunnistaa ja kehittää erilaisia toimintatapoja lasten kanssa, koska lapset reagoivat sairaalaan eri tavoin. Hoidossa on tärkeää mennä lapsen ehdoilla. Välillä tulee kuitenkin tilanteita, joissa hoitajan on vain tehtävä päätös ja mentävä määrätietoisesti eteenpäin. Jatkossa olisikin hyvä tutkia enemmän lasten omia kokemuksia sairaalasta. The thesis was made for TYKS paediatric clinic’s emergency department. The aim of this thesis is to bring new aspects to the Paediatric Emergency Clinic nurses’ professional skills in encoun-tering with children. The purpose of thesis was to find out about a child’s encounter in emergency department and to create a structured questionnaire in the basis of this thesis’ results. In the future with this questionnaire it’s possible to get more information about confronting over a 9- year-old children in the Paediatric Emergency Clinic. The questions were chosen by analyzing many of publications used in this thesis and by finding similarities in them. This thesis is done as a descriptive literature review (n=24). Content analysis was used to analyze the material.
The health care professional’s behaviour should conform giving emotional support to the child and distribution of honest information. Encountering a child is a challenge for a nurse because encounter and treatment needs to be in harmony with child’s age and development grade. A nurse must confront a child as an equal patient: listening to the child’s opinions, taking them into account and answer to the child’s questions. Calmness, friendliness, suitable toys to child’s developmental level and including the child to the treatments increase the reliability of the hospital environment. Function of the emergency department consist on the evaluation of treatment urgency, public and the special health care and telephone services. The child’s visit to the Paediatric Emergency Clinic can be difficult and worrying to the child’s caregivers. Informing caregivers and letting them to take part in their children’s care increases the trust for hospital personal.
It’s important that nurses working in the paediatric field recognize and create different approaches on how to act with a child because each child reacts differently to hospitals. Nurses should follow the child’s terms in treatments as much as possible but there’s situations when a nurse needs to take over to ensure that the necessary treatments are done. In the future it would be good to research more about childrens’ own hospital experiences.
