Tiivistelmä
Rakenteinen kirjaaminen tarkoittaa tiedon kirjaamista ja tallettamista yhteisten, ennakkoon sovittujen rakenteiden mukaisesti potilas- ja asiakastietojärjestelmiin. Vapaamuotoisena tekstinä kirjattavia potilasta koskevia asioita voidaan muokata rakenteiseen muotoon jakamalla niitä eri näkymien ja otsikoiden alle. Potilaan hoidossa käytettävät keskeiset tiedot on määritetty rakenteisina tietosisältöinä. Jokainen potilasta koskeva tieto kirjataan sille erikseen määriteltyyn tietokenttään, jossa käytetään sille valittua koodistoa tai luokitusta.
Tämän opinnäytetyön tarkoituksena oli tuottaa ohje rakenteisen kirjaamisen tueksi palvelutalo Pääskylään Kittilään. Ohjeen tarkoituksena oli kehittää hoitohenkilöstön rakenteista kirjaamista Effica-potilastietojärjestelmän yleisimmin käytettäviä komponentteja hyödyntäen. Tarkoituksena oli luoda apuväline kirjaamisen tueksi, jotta hoitotyön kirjaaminen yhtenäistyisi Pääskylässä. Yhdenmukaiset käsitteet antaisivat mahdollisimman ajantasaisen ja luotettavan kuvauksen asiakkaan voinnista.
Projekti eteni suunnitteluseminaarista joulukuussa 2018 aloitusvaiheeseen, joka tapahtui heinäkuussa 2019. Ohje koottiin ja otettiin koekäyttöön syyskuussa 2019. Ohje oli koekäytössä joulukuuhun 2019 saakka. Tulokset ohjeen koekäytöstä koottiin alkuvuodesta 2020. Opinnäytetyö valmistui keväällä 2020.
Kirjaamisohjeen komponentit valittiin palvelutalossa yleisimmin hyödyksi olevista komponenteista. Yhtenäiset potilastiedot mahdollistavat potilastietojen käytön ja siirrettävyyden eri järjestelmien välillä. Projektin käynnistämiseksi laadittiin syksyllä 2019 ohje koekäyttöön, jotta työntekijöillä oli mahdollisuus harjoitella komponenttien entistä parempaa hyödyntämistä päivittäisissä kirjauksissaan.
Kirjaamisen kehittyminen tulee jatkumaan Pääskylässä. Kirjaamisen laadun jatkuvuuden varmistamiseksi tulee palvelutalon henkilöstö sitouttaa opettamaan uusia työntekijöitä työn perehdytyksen yhteydessä rakenteiseen kirjaamiseen. Uuden Lifecare-kirjaamisalustan käyttöönoton jälkeen, tulee rakenteisen kirjaamisen ohjetta muuttaa suomalaisen hoitotyön luokituskokonaisuuden, FinCC:n, uuden version 4.0 mukaiseksi. Uusi arviointi kirjaamiskäytännöstä on ajankohtaista ehkä noin vuoden päästä opinnäytetyön valmistumisesta. Kirjoittajien mielestä projekti oli erittäin onnistunut. Structured recording means the recording and storage of information in patient and customer information systems according to common, pre-agreed structures. Patient issues that are recorded as free-form text can be edited in a structured form by dividing them under different views and headings. The key data used to treat the patient have been defined as structured data contents. Each patient data is recorded in a data field specifically defined for it, using the code or classification chosen for it.
The purpose of this thesis was to produce a guide to support structured registration in the service house Pääskylä in Kittilä. The purpose of the guide was to develop the structured recording of nursing staff using the most commonly used components of the Effica patient information system. The purpose was to create an aid to support registration so that the registration of nursing work would be harmonized in Pääskylä. Uniform concepts would provide the most up-to-date and reliable description of customer well-being.
The project progressed from the design seminar in December 2018 to the start-up phase, which took place in July 2019. The guide was compiled and put into trial use in September 2019. The guide was in trial use until December 2019. The results of the trial use of the guide were compiled in early 2020. The thesis was completed in spring 2020.
The components of the registration guide were selected from the most commonly used components in the service building. Unified patient data enables the use and transferability of patient data between different systems. In order to start the project, instructions for trial use were prepared in the autumn of 2019, so that employees had the opportunity to practice better utilization of the components in their daily entries.
The development of recording will continue in Pääskylä. In order to ensure the continuity of the quality of recording, the staff of the service house must commit to teaching new employees in connection with structured recording in connection with job orientation. After the introduction of the new Lifecare recording platform, the instructions for structured recording will have to be changed in accordance with the new version 4.0 of the Finnish nursing classification entity, FinCC. A new assessment of the recording practice is relevant perhaps about a year after the completion of the thesis. The authors thought the project was very successful.
