Tiivistelmä
Kehittämistyön tarkoituksena oli luoda selkeä rakenne erityishuollon palveluja tuottavan moniammatillisen arviointityöryhmän kuntoutussuunnitelmalle ja systematisoida kuntoutussuunnitelman kirjaamista. Uudistamisella tavoiteltiin sitä, että käyttäjätahot löytävät kuntoutussuunnitelmasta tarvitsemansa tiedon helpommin ja että kuntoutussuunnitelma sisältää mahdollisimman vähän saman tiedon kirjaamista useampaan kertaan.
Laadukas kuntoutussuunnitelma takaa tiedon siirtymisen eri käyttäjätahojen välillä ja se takaa asiakkaille oikeudenmukaisen ja yhdenvertaisen mahdollisuuden saada kuntoutusta. Aiemmissa tutkimuksissa on todettu, että kuntoutussuunnitelman kirjaamista tulisi kehittää. On todettu, että Toimintakyvyn, toimintarajoitteiden ja terveyden kansainvälinen luokitus (ICF) voi parantaa kuntoutussuunnitelmien sisältöä ja laatua. ICF-luokitus sopii viitekehykseksi moniammatillisen työryhmän kirjaamiseen laaja-alaisuutensa ja toimintakykytietoa jäsentävän rakenteensa vuoksi.
Kehittämistyö toteutettiin työpajoissa, joissa hyödynnettiin osallistavia kehittämistoiminnan menetelmiä. Tuotoksena syntyi kuntoutussuunnitelman uusi rakenne, jossa toimintakykytieto rakenteistettiin ICF-luokituksen otsikoin asiakas- ja toimintakeskeisesti. Uudesta rakenteesta kerättiin palautetta asiakasraadilta, joka koostui kuntoutussuunnitelman käyttäjätahojen edustajista. Asiakasraadista saatu palaute huomioitiin kehittämistyössä.
Kuntoutussuunnitelmaa voisi jatkossa kehittää systematisoimalla kirjaamisen sisältöä ja ottamalla käyttöön ICF-luokituksen koodeja kuvattaessa toimintakykyä. Jatkossa ICF-luokitusta voisi hyödyntää laajemminkin kirjaamisessa sekä kehittämistyön toteuttaneessa organisaatiossa että sen ulkopuolella. ICF-luokitus auttoi rakenteistamaan moniammatillisen työryhmän kuntoutussuunnitelman kirjaamista.
The purpose of the development work was to create a clear structure for rehabilitation plan of a multidisciplinary team that produces special services for persons with intellectual and developmental disabilities. Another purpose was to systematize the documentation of the plan. The aim was that the users of the rehabilitation plan would find the information they need more easily, and that the same information would not have to be recorded multiple times.
A high-quality rehabilitation plan will ensure that information is transferred between the users of rehabilitation plan and that the clients have fair and equal possibilities for rehabilitation. Previous research has highlighted that the documentation of rehabilitation plans should be developed. It has also been emphasized that International Classification of Functioning, Disability and Health (ICF) could help to develop the content and quality of rehabilitation plans. ICF is a good framework for documenting in multidisciplinary team because of its wide perspective and structure that classifies the information about functioning.
The development work was implemented in workshops where participatory methods of development work were used. The outcome was a new structure of the rehabilitation plan where the information about functioning was classified on a client-centred and occupation-centred basis by using the headlines of ICF classification. Feedback was collected about the new structure of the rehabilitation plan from the users. The feedback was taken into account in the development work.
The rehabilitation plan could be developed by organizing the content of the documentation and by using the ICF codes when describing human function. In the future, it could be possible to benefit from the ICF more widely in documentation both in the organization where the development work implemented as well as in other organizations. ICF helped with structuring the documentation of the rehabilitation plan in the multidisciplinary team.
