Tiivistelmä
Kansainvälisiltä terveysorganisaatioilta, hallituksilta ja terveyspalveluiden järjestäjiltä tuleva paine ottaa potilaat mukaan laadunkehittämistyöhön, luo haasteita kehitystyötä tekeville. Jotta potilaiden tarkoituksenmukainen osallistaminen olisi mahdollista, on tietoa pystyttävä keräämään ja mittaamaan järkevästi. Kehittämistyöhön validoituja työkaluja, strategioita sekä menetelmiä potilaiden osallistamiseen on kuitenkin vasta rajallisesti.
Potilaskokemustiedon käyttö kehittämistyössä on nähty vaikuttavan hoidon laatuun ja näin ollen osallistavan potilaat kehittämistyöhön vaikuttavasti. WHO kannustaa potilaskokemuksen hyödyntämiseen, jotta tietoa terveydenhuollon palveluiden saatavuudesta ja laadusta olisi käytettävissä niiden seurantaan, vertailuun ja arviointiin.
Tämän kehittämisprojektin tarkoituksena oli tuottaa tietoa siitä, miten toteutunut hoito näyttäytyy asiakkaan näkökulmasta ja tavoitteena hyödyntää tuloksia laadunkehittämistyössä. Tutkimuksellisen osion tarkoituksena oli kartoittaa potilaskokemustietoa strukturoidun, valmiin Hyvä Perioperatiivinen Hoito- mittarin (Liite 1.) avulla, jonka tulosten pohjalta hoitotyön laadun nykytila arvioitiin. Kyselyyn saatiin 154 vastausta, minkä pohjalta tulokset analysoitiin.
Potilaan kokemus hoidosta oli kokonaisuudessaan hyvä, mutta selkeitä kehittämiskohteita löytyi. Henkilökunnan ominaisuudet arvioitiin kokemuksen kannalta parhaaksi osatekijäksi ja sitä seurasivat kunnioitus sekä ilmapiiri. Fyysisen hoidon alueelta parannettavaa löytyi kivunhoidosta sekä lämpötalouden hoidosta. Suurimmaksi kehitystä vaativaksi osa-alueeksi tutkimuksessa nousi omatoimisuuden tukeminen. Potilaat kokivat etteivät pystyneet vaikuttamaan hoitoonsa sekä rohkaisu, henkinen tuki ja jännityksen lievennys eivät vastanneet potilaiden tarpeita. Hoito ei näyttäytynyt myöskään tasalaatuisena, vaan kokemukseen vaikuttivat hoidon ajankohta, toimenpiteen ennakoitavuus, anestesiamuoto sekä potilaan ikä.
Tulosten valossa osastolle tehtiin hoidon nykytilan kartoitus, mikä toimii pohjana tulevan laadunkehittämistyön suunnittelulle. Suunnittelua jatketaan osastolla, missä yhdessä henkilökunnan kanssa mietitään menetelmiä kehitettävien osa-alueiden parantamiseksi. The pressure from the international healthorganisations, goverments and healthservice producers to involve patients in qualityimprovement (QI), makes challenges for qualityimprovementworkers. To involve the patient effectively we should be able to collect and measure the knowledge properly and the tools, strategys and methods to do that are still limited.
Usage of patient experience in the field of QI has been seen to improve the quality of care and to involve patients in QI effectively. WHO is encouraging to utilize patient experience, so that we can have knowledge of healthcareservices availability and quality so they can be evaluated, compared and monitored.
Purpose of this developementprojects was to provide knowledge about patients perceptions of the quality of perioperative care, and the possibilities to utilize received results in our QI work. Reseach part of this project we charted patient experience with Good Perioperative Care-tool (Appendices 1.) and based on the results we mapped the state of the quality of care in our department. reseach included 154 forms and the analysis was made based on those answers.
The general patient experience was good, but there was clear development areas to be found. The staff was rated the best part of the experience and not near behind comes the environment and the processes. From the physical care most QI needed areas were painmanagement and the chillnes of the patients. Areas that needed the most GI in the hole study was encouragement and mental support of patient, alleviation of fear and anxiety and opportunity to influence to given care.
Care itself did´t show very uniform, and factors like timig and predictability of the operation, form of anesthesia and the age of the patient influenced the patient experience of care. Based on this results, the chart of quality of perioperative care was made on our department. That chart will be used as basis for future qualityimprovementwork, that will be continued in the department with the whole staff after this study.
