Tiivistelmä
Opinnäytetyön tarkoituksena oli selvittää asiakasnäkökulmasta eri tekijöiden vaikutuksia imetyskokemukseen. Tavoitteena oli, että terveydenhoitajat tuntevat entistä paremmin asiakkaiden imetyskokemuksia ja pystyvät hyödyntämään tutkimustuloksia kehittäessään imetysohjausta. Kohderyhmänä toimivat Porin perusturvakeskuksessa asioivien vuosien 2015-2017 aikana syntyneiden imetettävien lasten vanhemmat. Opinnäytetyössä käytettiin kvantitatiivista tutkimusotetta. Aineisto kerättiin strukturoidulla kyselylomakkeella, joka laadittiin yhteistyössä samaa aihealuetta tutkivan opinnäytetyöparin kanssa. Verkkokyselylomakkeet jaettiin äideille terveydenhoitajien välityksellä ja vastaaminen oli täysin vapaaehtoista (n=23).
Suurin osa äideistä piti imetyskokemustaan myönteisenä. Puolisolta saatu tuki ja myönteinen asenne imetystä kohtaan kuvattiin merkittäväksi tekijäksi imetyksen onnistumisessa. Lisäksi vertaistukiryhmistä saatua tukea pidettiin tärkeänä. Äideistä viidennes koki terveydenhoitajalta saadun imetystuen merkittävimmäksi ja lähes 80% äideistä kuvasi terveydenhoitajalta saadun imetystuen merkitsevän paljon tai melko paljon. Kuitenkin yli kaksi kolmesta äidistä kertoi terveydenhoitajalta saadun imetystuen jääneen puutteelliseksi lastenneuvolassa.
Imetykseen liittyvää tietoa haettiin eniten netistä, perheeltä ja ystäviltä sekä terveydenhoitajalta. Äidit kokivat terveydenhoitajan kertovan neuvolakäynneillä runsaasti imetyksen eduista, mutta imetyksen ongelmatilanteista saatu tiedon määrä kuvattiin vähäiseksi. Terveydenhoitajalta kaivattiin tietoa ja tukea erityisesti imetyksen pulmatilanteista selviämiseen sekä vauvan ravitsemustilaan ja kehitysvaiheisiin liittyen.
Imetysohjaus erityisesti esikoislapsien kohdalla koettiin riittämättömäksi. Äidit kaipasivat lisää ohjausta imetysasentoihin ja -tekniikkaan. Suosituimmaksi ohjausmuodoksi koettiin yksilöohjaus. Säännöllinen koulutus ja selkeä imetyksen toimintasuunnitelma tukee imetysohjauksen yhtenäisiä toimintatapoja, ja parantaa terveydenhoitajien mahdollisuuksia tarjota asiakkaiden tarpeiden mukaista palvelua neuvoloissa. Jatkotutkimuksessa voisi selvittää kehittämistoimien jälkeen uudelleen sekä perheiden että terveydenhoitajien näkemyksiä imetyksen tuesta ja ohjauksesta. The purpose of this thesis was to find out the effects of different factors of breastfeeding based upon customers experience. The goal was that public health nurses to become more aware of their clients breastfeeding habits, and to utilize the research finding better when it comes to breastfeeding guidance. The target group are the parents whose children were born between 2015 and 2017 and are clients of “Porin Perusturvakeskus”. The thesis was done by using a quantitative research approach. The research material was gathered using a structured questionnaire that was made by collaborating with another workgroup who are also working on a thesis on the same topic. Nurses handed out the link of the online questionnaire to the mothers and answering was completely voluntary (n=23).
The majority of mothers considered their breastfeeding experience in a positive way. Positive attitude towards breastfeeding and the support of partners were described as a significant factor for success in breastfeeding. Also, the support gained from peer support groups was considered important. From all the mothers, 20% found the nurses support significant, and 80% found it very more or less meaningful. However, two out of three mothers answered the questionnaire that the information received from the children’s clinic nurses was insufficient when it came to breastfeeding.
Information considering breastfeeding was gathered from family, friends, healthcare professionals and sought from the Internet. Mothers felt that nurses gave a lot of information about the positives of breastfeeding but did not tell a lot about the problems that might occur while breastfeeding. More information from the following areas from the nurses was desired: problems occurring during breastfeeding, nutritional information on the child’s different stages of development.
Breastfeeding guidance, especially for new-borns was found to be insufficient. Mothers needed more information when it came to breastfeeding positions and techniques. The answers showed that the most popular form of guidance was one on one. Regular training for nurses and a clear breastfeeding plan will support a clear united breastfeeding plan. The clearer these plans are the easier it will be for nurses to offer counselling for all types of families at different clinics. After the improvements have been made, future studies could re-examine the views of families and nurses when it comes to breastfeeding guidance and support.
