Tiivistelmä
Fetaalihemoglobiinivärjäys on sytokemiallinen tutkimusmenetelmä, jota käytetään pääosin epäiltäessä fetomaternaalista eli sikiöstä äitiin tapahtuvaa verenvuotoa. Fetomaternaalivuoto on vakava raskauskomplikaatio, joka voi johtaa sikiön vammautumiseen tai jopa kuolemaan. Fetaalihemoglobiini on sikiön ja vastasyntyneen pääasiallinen hemoglobiinimuoto, kun taas aikuisella pääasiallinen hemoglobiinimuoto on hemoglobiini A.
Tämä opinnäytetyö tehtiin toimeksiantona Tykslabin (Turun yliopistollisen keskussairaalan laboratorio) hematologian laboratoriolle. Opinnäytetyötä tehdessä kyseisen laboratorion menetelmätyöohjeen mukaan fetaalihemoglobiinivärjäykset tehtiin siten, että sivelyvalmisteet ilmakuivattiin työvaiheiden välissä. Tämän opinnäytetyön tarkoituksena oli selvittää, vaikuttaako kuivaustapa värjäyksen onnistumiseen ja voiko fetaalihemoglobiinivärjäystä nopeuttaa käyttämällä ilmapuhallinta eli fööniä valmisteiden kuivaukseen työvaiheiden välissä. Opinnäytetyön tavoitteena oli tuottaa toimeksiantajalle uutta tietoa mahdollista menetelmätyöohjeen päivitystä varten.
Tätä opinnäytetyötä varten hankittiin kymmenen näyteparia, joista kukin muodostui vastasyntyneen lapsen ja aikuisen miehen verinäytteestä. Jokaisesta näyteparista tehtiin verisivelyvalmisteet. Puolet valmisteista värjättiin Tykslabin hematologian laboratorion nykyisen fetaalihemoglobiinivärjäysohjeen mukaan, eli ilmakuivaamalla valmisteet työvaiheiden välissä ja puolet valmisteista kuivattiin föönillä. Värjätyt sivelyvalmisteet tarkasteltiin mikroskoopilla opinnäytetyön tekijän ja kahden opinnäytetyön ohjaajan toimesta.
Värjäysten mikroskooppisen tarkastelun jälkeen voitiin todeta, ettei ilmakuivattujen ja föönillä kuivattujen sivelyvalmisteiden välillä ollut laadullisia eroja. Tämän johdosta Tykslabin hematologian laboratorio harkitsee fetaalihemoglobiinivärjäyksen menetelmätyöohjeen päivittämistä siten, että jatkossa valmisteiden kuivaamiseen käytettäisiin fööniä. Kleihauer–Betke test is a cytochemical research method, which is mainly used in the suspicion of fetomaternal hemorrhage, in other words bleeding of fetus into maternal circulation. Fetomaternal hemorrhage is a severe pregnancy complication which can cause injuries or even death of the fetus. Fetal hemoglobin is the major hemoglobin component in fetuses and newborns while adult’s major hemoglobin component is hemoglobin A.
This bachelor’s thesis was commissioned by hematology laboratory of Tykslab (Turku University Hospital Laboratory). While working on this thesis, the aforementioned laboratory’s Kleihauer-Betke tests were performed in a way, where blood smears were air-dried between the stages of the test. The purpose of this thesis was to investigate if drying method impacts the quality of Kleihauer–Betke test and investigating the possibility to speed up performing of Kleihauer–Betke test by drying blood smears with a blow-dryer between different stages of the test. The goal of this thesis was to produce new information for the potential updating of the Kleihauer-Betke test’s work instructions.
Ten blood specimen pairs were gathered for the research of this thesis. Each specimen pair contained a blood sample from a newborn and an adult male. Blood smears were drawn from each specimen pair. Half of the blood smears were stained with Tykslab hematology laboratory’s current Kleihauer–Betke test work instructions, simply put by air-drying the blood smears between the stages of the test and other half were dried with a blow-dryer. The Kleihauer–Betke stained blood smears were examined with microscope by the writer of this thesis and by two supervisors of the thesis.
After microscopy examination the writer of the thesis and the supersivors discovered that there weren’t any quality differences between air-dried and blow-dried blood smears. Due to this thesis, the hematology laboratory of Tykslab will consider updating Kleihauer–Betke test work instructions, so that in the future the blow-dryer would be used to dry the blood smears.
