Tiivistelmä
Opinnäytetyön tarkoituksena oli kartoittaa tyytymättömän potilaan kohtaamista puhelimessa ja kasvotusten. Tavoitteena oli kehittää tyytymättömiä potilaita kohtaavien sairaanhoitajien toimintaa yhtenäisten ohjeiden avulla, jotka lisäävät sairaanhoitajien varmuutta omasta toiminnasta ja edistävät työhyvinvointia. Opinnäytetyön tuloksista koottiin posterimuotoiset ohjeet, jotka on suunnattu hoitohenkilökunnalle. Opinnäytetyön toimeksiantajana toimi Varsinais-Suomen sairaanhoitopiiri.
Opinnäytetyö toteutettiin narratiivisena kirjallisuuskatsauksena (n=20). Lähdeaineisto etsittiin tutkittuun tietoon perustuvista hoito- ja lääketieteen tietokannoista, hoitoalan tieteellisistä lehdistä ja kirjoista ja luotettavilta internetsivustoilta.
Tuloksista on nähtävissä, että potilas ilmaisee tyytymättömyyden tunteensa pääasiassa sanallisesti ja kehonkielen välityksellä. Potilaan tyytymättömyyteen on monia eri syitä, jotka ovat yhteydessä hoitoon, organisaatioon ja kohteluun. Yleisimmät syyt ovat hoitoon pääsemisen vaikeus, hoidosta aiheutuneet komplikaatiot, hoitohenkilökunnan epäammattimainen käytös ja riittämätön vuorovaikutus hoitohenkilökunnan ja potilaan kesken. Tyytymättömät potilaat kokevat myös, että he eivät saa riittävästi tietoa sairaudestaan, lääkityksestään tai hoidostaan eivätkä pääse osallistumaan hoitoonsa ja siihen liittyviin päätöksiin haluamallaan tavalla.
Kohdatessaan tyytymättömän potilaan sairaanhoitajan tulee pysyä rauhallisena ja kuunnella potilasta. On tärkeää, että potilas kokee tulevansa kuulluksi ja että hänen asiansa otetaan vakavasti. Oikeanlainen äänensävy ja avoin kehonkieli lisäävät potilaan luottamusta sairaanhoitajaa kohtaan.
Potilaat tekevät yhä enemmän valituksia terveydenhuollon palveluista. Tästä syystä on tärkeää luoda hoitohenkilökunnalle yhtenäiset ohjeet siitä, miten kohdata tyytymätön potilas ja miten ehkäistä potilaiden tyytymättömyyttä omalla käytöksellään. The purpose of this thesis was to find out how to confront a dissatisfied patient on the phone and face to face. The aim was to improve nurse performance when facing dissatisfied patients with the help of guidelines that help nurses to feel more confident of what they do and to promote wellbeing in the workplace. Guidelines for the care staff were made based on the results of this thesis. This thesis was commissioned by the Hospital District of Southwest Finland.
The thesis was carried out as a narrative literature review (n=20). The sources of information included articles from different medical databases, medical journals and books, and reliable websites.
The results show that the patient mainly expresses his dissatisfaction verbally or by using body language. There are many reasons for the dissatisfaction of the patient considering care, organization and treatment. The most common reasons for dissatisfaction are difficulties in getting health care, complications after the treatment, unprofessional behaviour of the care staff or inadequate communication between the patient and the care staff. The dissatisfied patients may also feel that they do not get enough information about their illness, medication or treatment, and they are not able to take part in the decisions concerning their own care in a way they wanted to.
When meeting a dissatisfied patient, the nurse should remain calm and listen to the patient. It is important that the patient feels he is being heard and his problem is being taken seriously. Using a right tone of voice and an open body language helps the patient to increase trust towards the nurse.
The patients make more complaints about the healthcare services. For that reason, it is important to create guidelines for the care staff for how to meet a dissatisfied patient and how to prevent patient dissatisfaction with one’s own performance.
