Tiivistelmä
Opinnäytetyön tarkoituksena oli kuvata lastensuojelulaitoksen henkilöstön lääkehoito-osaamista. Tarkoituksena oli kuvata myös lastensuojelulaitoksen lääkehoidon toteutus ja luoda uusia käytäntöjä lääkehoidon toteuttamiseksi turvallisesti. Tavoitteena opinnäytetyössä oli toteuttaa lääkehoitoa turvallisesti lastensuojelulaitoksessa ja varmistaa henkilöstön lääkehoidon osaaminen. Tavoitteena oli myös selkeyttää lääkehoidon kokonaisuus jokaiselle henkilöstön jäsenelle ajantasaisen lääkehoitosuunnitelman avulla. Opinnäytetyö toteutettiin tiiviissä yhteistyössä lastensuojelulaitoksen henkilökunnan kanssa.
Opinnäytetyö toteutettiin toimintatutkimuksena, joka sisälsi kolme kehittävää sykliä. Ensimmäisessä syklissä käytettiin työkonferenssimenetelmää. Toimintatutkimuksen ensimmäisessä syklissä toteutettiin myös alkukartoitus henkilöstön lääkehoito-osaamisesta MiniLop-tenttien avulla. MiniLop-tentti on lääkehoidon osaamisen mittari, joka on suunnattu lääkehoitoon kouluttamattomalle henkilöstölle. MiniLop-kurssi toimi tässä opinnäytetyössä interventiona ja oli käytössä myös toisen syklin aikana. Toimintatutkimuksen toisessa syklissä toteutettiin henkilöstön teemahaastattelut. Haastateltavat valittiin harkinnanvaraisesti. Toimintatutkimuksen kolmannessa syklissä käytettiin myös työkonferenssimenetelmää, jonka avulla arvioitiin päivitettyä lääkehoitosuunnitelmaa. Kolmannessa syklissä myös analysoitiin teemahaastatteluista koottua aineistoa. Tässä opinnäytetyössä syklit eivät olleet tarkkarajaisia ja ajallisesti syklit toteutuivat osin päällekkäin. Lisäksi koko opinnäytetyön ajan arvioitiin ja muokattiin yhdessä lääkehoidosta vastaavien ja lastensuojelulaitoksen johdon kanssa ensimmäisessä työkonferenssissa tuotettua lääkehoitosuunnitelmaa.
Teemahaastattelusta koottu laadullinen aineisto analysoitiin sisällön analyysillä. Aineistoa kertyi myös työkonferensseista. Aineiston tulosten perusteella lääkehoito-osaaminen ja sen arvostus lastensuojelulaitoksessa on puutteellista ja osalla henkilöstöstä ei ole lääkehoidon koulutusta. Henkilöstö koki tarvitsevansa koulutusta yleisesti lääkehoitoon ja erityisesti lastensuojelulaitoksessa yleisesti käytettäviin lääkkeisiin. Opinnäytetyön aikana laadittiin lääkehoitosuunnitelma, jossa on kuvattuna lastensuojelulaitoksen turvallinen lääkehoidon toteutus.
Jatkotutkimuksissa tulisi selvittää useiden lastensuojelulaitosten henkilöstön lääkehoito-osaamista ja lääkehoidon turvallisia käytäntöjä sekä laatia yhtenäinen malli turvalliseen lääkehoidon toteuttamiseen lastensuojelulaitoksissa. The purpose of this thesis was to describe the medication competency of the staff in a child protection institution. In addition, the thesis also aimed at describing the realization of medication in the child protection institution as well as creating new procedures to implement medication safely. The objective of the thesis was to realize medication in the child protection institution safely as well as to ensure the medication competency of the staff. Moreover, the thesis strived at clarifying the entity of medication to every member of the staff by way of up-to-date medication plan. The thesis was implemented in close cooperation with the staff of the child protection institution.
As its method, this thesis was focusing on operational aspects and consisted of three developmental cycles. The method used during the first cycle was the work conference method. The preliminary survey on the medication competency of the staff was also carried out during the first cycle by means of MiniLOP exams. MiniLOP exam is an assessment tool for medication competency designed for staff not trained in medication. MiniLOP course served as an intervention in the thesis and was also used during the second cycle. Thematic interviews of the staff were carried out in the second cycle of this thesis focusing on operational aspects. The individuals for the interview were selected on a discretionary basis. The third cycle used again the work conference method with the help of which it was possible to evaluate the updated medication plan. The third cycle also included the analysis of the material collected from the thematic interviews. The cycles were not clearly defined in this thesis and partially they were also chronologically overlapping. Furthermore, the medication plan created in the first work conference was evaluated and revised continuously together with the people responsible for medication and the management of the child protection institute.
The qualitative material gathered from the thematic interviews was analyzed by using content analysis. The work conferences also provided more material for the study. Based on the results on the material the medication competency and its appreciation in the child protection institution are inadequate and some members of the staff have not been trained in medication. The staff felt that they need training in medication in general and especially in medicines commonly used in child protection institutions. One outcome of this thesis was a medication plan which describes a safe realization of medication in a child protection institution.
Further study should focus on studying the medication competency of a large number of child protection institutions and safe procedures of medication as well as on creating a consistent model for realizing safe medication in child protection institutions.
