Tiivistelmä
Tutkimuksessa selvitettiin Dräger Oxylog -ventilaattorilla annettavan NIV (noninvasiivisen) hoidon vaikutusta hengitysvaikeuspotilaan kokemaan hengitysvaikeuteen, hengitystaajuuteen ja saturaatioon sekä ensihoitajien käyttökokemuksia ventilaattorin käytöstä Oulu-Koillismaan (OK Pela) ja Jokilaaksojen (Jopela) pelastuslaitosten alueella.
Dräger Oxylog -ventilaattorit otettiin uutena laitteena käyttöön Pohjois-Pohjanmaan sairaanhoitopiiriin (PPSHP) kuuluvissa OK Pela:n ja Jopela:n ambulansseissa keväällä 2018. Ventilaattorien hyötyjen suhteuttaminen alueiden ensihoitoon on tarpeen, sillä tällä hetkellä laitetta ei löydy lähellekään kaikista alueiden ambulansseista. Toimeksiantajien toiveesta sekä aineiston vuoksi tutkimus rajattiin koskemaan vain NIV-hoitoa.
Tutkimuksen tarkoituksena oli selvittää, hyötyivätkö potilaat Oxylog -ventilaattorilla annettavasta NIV-hoidosta ensihoidossa, miten hoito vaikutti potilaan vitaaliparametreihin ja miten ventilaattorin käyttö vaikutti potilaan kokemaan hengitysvaikeuteen ensihoitajan arvioimana. Tutkimuksen tavoitteena oli saada tietoa NIV-hoidon hyödyistä ja vaikutuksista hengitysvaikeuspotilaan ensihoidossa näiden Pelastuslaitosten alueilla.
Tämän lisäksi kartoitettiin ensihoitajien käyttökokemuksia NIVhoidosta Oxylog -käyttötaulukon avulla. Tutkimusaineisto koottiin OK Pela:n ja Jopela:n alueella NIV -hoitoa saaneiden potilaiden Oxylog -käyttötaulukoista ja näihin tehtäviin liittyvistä ensihoitokertomuksista. Tutkimus on kvantitatiivinen ja siihen kerättiin aineistoa yhteensä 77 ensihoitotehtävältä. Tutkimuksessa ei otettu huomioon ennen NIV-hoitoa annettuja hoitoja, kuten happihoito tai lääkehoito.
Potilaan kokema hengitysvaikeus oli helpottanut 92 %:lla NIV-hoitoa saaneista. Suurimmalla osalla tutkimuksen potilaista oli lähtötilanteessa vaikea hengitysvaikeus (ka. HT 34 krt/min) ja NIV-hoidon jälkeen suurimmalla osalla oli lievä hengitysvaikeus (ka. HT 25 krt/min). Suurimmalla osalla tutkimuksen potilaista oli lähtötilanteessa kohtalainen hypoksemia (ka. SpO2 83%), jota ei suurimmalla osalla potilaista ollut enää NIV-hoidon jälkeen (ka.SpO2 95%).
Ensihoitajien Oxylogin käyttökokemukset olivat pääasiassa positiivisia. Ventilaattori todettiin helppokäyttöiseksi ja sen vaste nopeaksi. Negatiivisiksi asioiksi oli kirjattu potilaan sopeutumattomuus NIV-hoitoon, maskin huono istuvuus ja sen ilmavuotoihin liittyviä asioita sekä ventilaattorin alhaisten happiprosenttien puuttuminen. Tuloksista voidaan päätellä, että ventilaattorin käyttäjien koulutus on onnistunut, Oxylog soveltuu hyvin alueiden ensihoidon käyttöön ja suurin osa potilaista on hyötynyt annetusta NIV-hoidosta. The study was set to examine the effects of NIV (non-invasive) treatment given by Dräger Oxylog ventilator for patients suffering from respiratory distress in Oulu-Koillismaa (OK Pela) and Jokilaakso (Jopela) rescue Departments area. Effects of NIV treatment were measured by respiratory distress of patient, respiratory rate and oxygen saturation of blood. In addition, we report the paramedics experiences with the ventilator in use.
Dräger Oxylog -ventilators were introduced as new divices at the OK Pela and Jopela on spring 2018. Both departments are part of Northern Ostrobothnia Hospital District (PPSHP). Balancing the benefits of the ventilators for emergency care in the regions is necessary because the ventilators are not yet found in every ambulance. The study was limited to noninvasive treatment because of the composition of the material and request of the client.
The purpose of the study was to find out whether the patients of the emergency care benefited from NIV treatment given with Oxylog ventilator, how the treatment worked for the patient’s vitals and how the use of the ventilator influence patient experiencing respiratory distress as assessed by a paramedic. The aim of the study was to obtain information from benefits and effects of the NIV treatment given to respiratory distress patient in emergency care in the area of these rescue Departments. In addition, an Oxylog usage chart was used to map the NIV treatment experience of paramedics. The research data was gathered from Oxylog usage chart of patients treated with NIV
in the OK Pela and Jopela area and from emergency care reports realated to these tasks. The research is quantitative, and the material was collected from a total of 77 paramedic tasks. In this study the treatments given before the NIV treatment, such as oxygen therapy or drug therapy, was
not taken into account.
Patients experiencing respiratory distress had relieved 92% of patients given NIV treatment. Most of the patients in the study had severe breathing difficulties at the beginning (ka respiratory rate 34 times per minute) and after NIV treatment majority of patients had mild breathing difficulties (ka respiratory rate 25 times per minute). Most of the patients in the study had moderate hypoxemia at the beginning (ka SpO2 83%) which most patients no longer had after NIV treatment (ka SpO2 95%).
Paramedics’ experiences of Oxylog were mainly positive. The ventilator was found to be easy to operate and its response was quick. Negative issues included patient nonadaptation to NIV treatment, poor fit of the mask and issues with air leakage and lack of low ventilator oxygen levels. The results suggest that ventilator user training has been successful, Oxylog is well suited for use in emergency care of the area and most patients have benefited from NIV treatment.
