Tiivistelmä
ODA (Omat digiajan hyvinvointipalvelut) on yksi Sosiaali- ja terveysministeriön Sote-tieto hyötykäyttöön -strategiaa toteuttava kärkihanke. ODA-palvelukokonaisuus valmistui vuonna 2018 ja sitä on kutsuttu sen jälkeen nimellä Omaolo-palvelu. Se sisältää sähköiset Omaolo-oirearviot, hyvinvointitarkastukset ja -valmennukset, palveluarvion ja palveluohjauksen sekä Oma-suunnitelman. Opinnäytetyö keskittyy hengitystietulehduksen Omaolo-oirearvioon. Oirearviointi on tapahtumasarja, jossa asiakas täyttää tietokoneella tiedot omista oireistaan. Tämän perusteella palvelu tekee oirearvion ja asiakas saa sähköisesti itsehoito-ohjeita tai ohjauksen tarkoituksenmukaisten palvelujen piiriin.
Opinnäytetyön tarkoituksena on mitata hengitystietulehduksen diagnoosien ja takaisinsoiton määrien muutoksia, kun sähköinen oirearvio on otettu käyttöön terveysasemilla. Tavoitteena on tuottaa selvitys oirearvioiden vaikutuksista takaisinsoiton ja hengitystietulehdus-diagnoosien määriin.
Opinnäytetyö on kvantitatiivinen rekisteritutkimus, jonka tilastot saatiin Helsingin kaupungin Omaolo-tiimiltä. Opinnäytetyön tutkimusasetelma on kvasikokeellinen ennen-jälkeen -asetelma, jossa interventiona on Omaolo-palvelu. Aineisto sisältää numeraalisen tiedon terveysasemien takaisinsoiton, Omaolon yhteydenottojen ja hengitystietulehduksien määristä. Ajanjaksoksi valikoitui 1.2.-31.5.2018 ja 1.2.-31.5.2019 jolloin hengitystietulehdukset ovat yleisempiä, kuin kesäkuukausina. Lisäksi tarkkailuun otettiin 1.6.-31.8.2019 Omaolon käyttömäärät. Tutkimukseen valittiin mukaan kuusi Helsingin kaupungin terveysasemaa. Vuoden 2018 rekisteritietotoja verrattiin vastaavaan ajankohtaan vuonna 2019 ristiintaulukoinnilla, jolloin nähdään ero siihen, kun oirearvio ei ole ollut vielä käytössä.
Omaolon ja takaisinsoiton määrien vertailussa havaittiin Omaolon hengitystietulehduksen oirearvion käytön (n=666) olevan selvästi alle prosentin luokkaa valittujen terveysasemien kokonaisväestön keskuudessa. Käytön havaittiin lisääntyneen tasaisesti helmikuun 2019 jälkeen. Takaisinsoiton määrät ovat vähentyneet vuodesta 2018, mutta tähän ovat vaikuttaneet tutkimuksen ulkopuoliset tekijät. Myös käytettyjen hengitystietulehdus-diagnoosimäärien luvut ovat laskeneet, mutta tämä selittyisi enemmän vuoden 2019 lievemmällä influenssakaudella. ODA (short for Omat digiajan hyvinvointipalvelut, translated My Digital Wellbeing Services) is a project under one of the flagship projects implementing the Ministry of Social Affairs and Health’s Social Information Utilization Strategy. ODA was later known as Omaolo (translated My Wellbeing) and the service it offers was ready to use in 2018. Omaolo includes electronic symptom assessments, wellbeing checks, coaching, service reviews and guides as well as the client’s own plan. This Master’s thesis is focusing on the symptom assessment of respiratory inflammation. The assessment is a process where the client fills in information about their symptoms online in the Omaolo system. Based on the information given, the system performs an assessment and the client will receive self-care instructions electronically or they will be directed to the appropriate local health care services.
The aim of the research is to measure changes in respiratory tract diagnosis and recurrence rates after the introduction of the electronic symptom assessment in health care centres. The aim is to provide an explanation of the effects of the electronic service to the numbers of respiratory tract infections and the numbers of calls made by clients to the health care centres about these types of symptoms.
This Master’s thesis is based on a quantitative register survey and the statistics obtained by the Omaolo-project in the City of Helsinki. The frame of the research is a quasi-experimental before-after setting, with the Omaolo service acting as an intervention. The data includes numerical information about the number of calls to the health care centres, contacts to the Omaolo service and the amount of respiratory tract infections. The data was collected during a spring (February 1 - May 31, 2018 and 2019) when respiratory tract infections are common. In addition for the period June 1 – August 31 2019 of Omaolo assesments were included for the analysis. Six (6) health care centres in the city of Helsinki were selected for the study. The 2018 registry data was compared to the corresponding dates in 2019 by cross-tabulation which shows a difference when the symptom assessment was not yet in use. In addition, the Omaolo utilization rates were reviewed in June-August 2019.
When comparing the Omaolo assessments and callbacks to the health care centres it was revealed that the use of Omaolo’s respiratory tract symptom assessment (n=666) was well below 1 % at the selected health care centres. The numbers of callbacks have declined since 2018 but this is also due to factors outside the study. The number of respiratory infection diagnosis has also fallen. This may also be explained by the less severe influenza season in 2019.
