Tiivistelmä
Terveyden ja hyvinvoinnin laitoksen mukaan psykiatristen avohoitokäyntien määrä on kasvanut vuosittain (THL2017.) Avopalveluiden lisääntyminen osastohoidon sijaan näkyy myös suoraan ensihoidossa, sillä mielenterveys häiriöistä kärsiviä potilaita on kotonaan avopalveluiden turvin ja näin ollen tulee yhä useammin tilanteita, jossa akuutisti pahentuneeseen tilanteeseen tarvitaan ensihoitoa. Tästä syystä ensihoitajat kohtaavat mielenterveys- ja päihdepotilaita nykyään lähes päivittäin ja tilanteet saattavat olla hyvin haastavia mm. potilaiden sairaudentunnottomuuden, päihtymyksen tai aggressiivisuuden vuoksi. (Kuisma ym. 2018, 688.) Ensihoidossa tehtäväkoodi 785 tarkoittaa mielenterveydenhäiriötä, mutta mielenterveysongelmista kärsiviä potilaita kohdataan myös monilla muilla ensihoidon tehtävillä ja se luo omat haasteensa ensihoitoon. Avopalveluiden lisääntyminen osastohoidon sijaan näkyy myös suoraan ensihoidossa lisääntyneinä 785 koodin tehtävämäärinä. Usein mielenterveyspotilailla on moninaisia ongelmia sekä runsasta päihteiden käyttöä.
Tämä opinnäytetyö toteutetaan kuvailevana kirjallisuuskatsauksena ja sen narratiivisena kirjallisuuskatsaus muotona, jossa kootaan yhteen kirjallisuudesta, tutkimuksista sekä aiemmista opinnäytetöistä saatuja tietoja mielenterveys potilaan kohtaamisesta ensihoidossa. Tutkimusaineistoa on kerätty kirjallisuuden lisäksi mm. seuraavista tietokannoista: Medic, Melinda, CINAHL ja PubMed. Työn tilaajana toimii Turun AMK.
Työn tavoitteena on koota saatavilla olevan tiedon pohjalta selkeä yhteenveto mielenterveyspotilaan kohtaamisesta, haastattelusta, tutkimisesta sekä auttaa tunnistamaan yleisimmät mielenterveyshäiriöt ensihoidossa. According to National Institute for Health and Welfare the number of outpatient psychiatric visits has increased every year (THL 2017). The increase in outpatient care has influence straight on the emergency care because the patients suffering from mental disorders are staying at home under cover of outpatient services and, therefore, there will be even more situations where emergency care is required for an acutely worsened situation. For this reason paramedics meet patients with mental disorders and substance abuse nearly every day and those situations may be very challenging due to for example patients anosognosia, intoxication or aggressiveness. (Kuisma etc. 2018, 688.) In emergency care the code 785 refers to mental disorders but patients suffering from mental disorders are confronted with many emergency care tasks which creates its own difficulties in emergency care. The increase of an outpatient care has also influence on the increasing number of code 785 tasks in emergency care. Usually mental health patients have various problems as well a high level of substance abuse.
This thesis is implemented as a descriptive literature review and its narrative literature review which gathers information from literature, researches and previous theses about encountering a mental health patient in emergency care. In addition to literature, research material has been collected from for example the following databases: Medic, Melinda, CINAHL and PubMed. The work is commissioned by Turku University of Applied Sciences.
The purpose of this thesis is through the information available to gather a clear summary of encountering, interviewing and examining a mental health patient and also identifying the most common disorders in emergency care.
