Tiivistelmä
Tämä opinnäytetyö on kirjallisuuskatsaukseen pohjautuva toiminnallinen opinnäytetyö, jonka tarkoituksena oli luoda potilasskenaarioita ennakkomateriaaliksi simulaatioharjoitusta varten. Toimeksiantajana toimi Turun ammattikorkeakoulu, jolle opinnäytetyön tuotos tulee käyttöön opetusmateriaalin tueksi.
Suomessa ikärakenteen muutoksen seurauksena muistisairauksia diagnosoidaan yhä enemmän. Muistisairaudet heikentävät yksilön toimintakykyä ja aivotoimintaa. Muistisairaan potilaan kohtaamisessa haasteen luo potilaan vaikeus ilmaista itseään, sekä hänen oikeutensa tulla kohdatuksi tasavertaisesti ja oikeudenmukaisesti. Tämä muodostaa eettisen ristiriidan esimerkiksi tilanteessa, jossa henkilö on nopean hoidon tarpeessa, mutta kykenemätön ilmaisemaan omaa tahtoaan hoidon suhteen. Eettinen päätöksenteko ja sairaiden kohtaaminen korostuu ensihoitajan ammatissa, jossa eettiset ongelmatilanteet ovat arkipäivää.
Opinnäytetyön tuotoksena syntyi neljä potilastapausta, joissa käsitellään keskeisimpiä muistisairaiden kohtaamiseen liittyviä ongelmakohtia, sekä ensihoitajan eettiseen päätöksentekoon vaikuttavia tekijöitä. Potilastapausten yhteyteen luotiin myös avoimia kysymyksiä, sekä mallivastaukset tapauskohtaisiin kysymyksiin.
Kirjallisuuskatsauksen tavoitteena on toimia pohjana eettisesti haastaville tapauksille, jotka käsittelevät muistisairaita potilaita ensihoitotilanteissa. Potilastapausten, sekä kysymysten tavoitteena on tuoda opiskelijoille tietoon miten muistisairas potilas tulisi kohdata, sekä mitkä tekijät vaikuttavat ensihoitajan eettiseen päätöksentekoon.
Potilastapaukset ja mallivastaukset eivät sisälly opinnäytetyön julkiseen versioon. This thesis is a functional thesis based on a literature review, with the intention to produce patient scenarios as advance material for simulation exercises. Our sponsor was the Turku University of Applied Sciences, which will use the output of the thesis as an addition of the teaching material.
As a result of the change in the age structure in Finland, memory disorders are being diagnosed more and more. Memory disorders weaken an individual’s ability to function and their brain function. The patient’s right to be treated equally and fairly creates a challenge in situations where the patient with a memory disorder has difficulty in expressing him-/herself. This creates an ethical discord, for example, in a situation where a person is in need of rapid treatment but is unable to express his or her own will for treatment. Ethical decision-making and encounters with the sick are in a point of emphasis in the profession of an emergency nurse, where ethical problems are encountered daily.
The result of the thesis was four patient cases, which deal with the most focal issues related to the encounter of memory disabilities, as well as the factors influencing the ethical decision-making of emergency nurses. Open questions were created as an addition to the patient cases, as well as model answers to case-specific questions.
The aim of the literature view is to serve as a basis for ethically challenging cases that deal with memory disordered patients in emergency situations. The aim of the patient cases and questions is to inform students about how a patient with memory disorders should be faced, and what factors influence the ethical decision-making of the emergency nurse.
The patient cases and model answers are not included in the public version of the thesis.
