Tiivistelmä
Sydän- ja verisuonisairaudet ovat maailman yleisin kuolemaan johtava syy (WHO 2017). Tahdistin on invasiivisesti potilaalle asennettava tekninen laite, jolla pystytään ehkäisemään sydänperäisiä äkkikuolemia. Tahdistimia on asennettu jo yli 60 vuoden ajan ja tänä aikana laitteet sekä tahdistinhoidon aiheet ovat kehittyneet valtavasti. Nykyään tahdistimia asennetaan hoitamaan oireista bradykardiaa, vaikeaa sydämen vajaatoimintaa, henkeä uhkaavia kammioperäisiä rytmihäiriöitä ja näistä johtuvia äkkikuolemia. Vaikka tahdistinhoito onkin kehittynyttä, tutkimusten mukaan tahdistininfektiot lisääntyvät nopeammin suhteessa tahdistinasennuksiin.
Tämän opinnäytetyön tarkoituksena oli systemaattista kirjallisuuskatsausta soveltaen (n=31) kartoittaa tahdistininfektioiden syntyyn vaikuttavia tekijöitä sekä niiden ehkäisyä. Työn tavoitteena on edistää tahdistininfektioiden ehkäisyä. Opinnäytetyön toimeksiantajana on Turun yliopistollisen keskussairaalan (TYKS) Sydänkeskus.
Tahdistininfektion syntyyn vaikuttavia tekijöitä on tunnistettu monia.Tekijät voivat olla potilaasta johtuvia, kuten miessukupuoli, nuorempi ikä, perussairaudet tai tietyt lääkitykset, esimerkiksi antikoagulantit. Toisaalta tekijät voivat olla myös potilaasta riippumattomia, kuten tahdistinlaitteesta tai sen asennuksesta johtuvia. Esimerkiksi useamman tahdistinjohdon käyttö tai toimenpiteen pidempi kesto lisäävät infektioriskiä. Merkittävin yksittäinen tahdistininfektion riskitekijä on uusintatoimenpide. Tunnistamalla tekijät tahdistininfektioiden taustalla, pystytään infektioriskiä lisäävät tekijät minimoimaan ja näin ehkäisemään infektioita.
Maailmalla tahdistininfektioita ja niiden ehkäisyä on tutkittu ja kehitetty laajasti. Infektioiden ehkäisyssä on tärkeää potilaan huolellinen valmistelu; perussairauksien ja olemassa olevien infektioiden hoito, lääkitysten tauotus tarvittaessa sekä hyvä hygienia ja leikkausalueen valmistelu aseptisesti. Hyvän aseptiikan nuodattaminen ja näyttöön perustuvien leikkaustekniikoiden käyttö on tärkeää. Antibioottiprofylaksian annostelu tahdistinpotilaille on tehokkaasti vähentänyt infektioita. Yhdistämällä erilaisia infektion ehkäisytekniikoita, infektioiden syntymistä pystytään ehkäisemään tehokkaasti. Cardiovascular diseases are the most common cause of death in the world (WHO 2017). The pacemaker is an invasive installed technical device, which is able to prevent sudden cardiac deaths. Pacemakers have been installed for over 60 years and during this time of period the technological advances and indications for pacemakers treatments has developed tremendously. Currently pacemakers can be installed to treat the symptomatic bradycardia, severe heart failure, life threatening ventricular arrhythmias and sudden deaths caused by these. Although the pacemaker therapy is advanced, infection rates have increased faster relative to the installed pacemakers, according to the researchers.
The purpose of this thesis was to find out factors affecting pacemaker infections and prevention of those by using applied systematic literature review (n=31). The aim was to advance the prevention of the pacemaker infections. The thesis is commissioned by The Hospital District of Southwest Finland.
Many factors affecting pacemaker infections have been identified. Factors can be patientrelated, such as male gender, younger age, comorbidities or certain medications, for example anticoagulants. In other hand factors also can be independent of the patient, for example device or operation related. For instance the use of multiple pacemaker leads or longer duration of the operation increases the risk of infection. Replacement operation has been identified as the most significant independent risk factor of infection. By identifying the factors affecting pacemaker infection, the risks of infection can be minimized and thus prevent the infections.
Pacemaker infections and prevention of infections has been widely researched and developed in the world. Meticulous preparation of the patient is important in the prevention of infections; treatment of comorbidities and underlying infections, interruption of medication when necessary, good hygiene and aseptic preparation of the skin over the operative site. Following good aseptic practices and evidence-based surgical techniques are important. Administration of antibiotic prophylaxis to the pacemaker patients has effectively decreased infections. By combining different infection prevention techniques, infections can be efficiently prevented.
