Tiivistelmä
Sosiaali- ja terveydenhuollon palvelurakenteen uudistus (SOTE) asettaa haasteita säästötavoitteillaan myös kuntoutukselle. Näkökulman tulee olla oikea- aikaisen ja tavoitteellisen kuntoutumisen varmistavan prosessin saumattomassa toteutumisessa. (Autti-Rämö ym. 2016, 16.)
Tämä kehittämisprojekti toteutettiin laadullista toimintatutkimuksen strategiaa seuraten. Tavoitteeksi asetettiin kohteena olevan organisaation toiminnan kehittäminen. Tavoitteena oli saavuttaa moniammatillisen yhteistyön kehittymisen tuloksena sujuvammat, tehokkaammat, asiakaslähtöisemmät toimintatavat kuntoutujan lyhytaikaiselle tutkimus- ja kuntoutusjaksolle. Kehitysvamma- alan tuki- ja osaamiskeskuksen yksiköistä Lääketieteellinen tutkimus- ja kuntoutusyksikkö (LAKU) toimi pilottiyksikkönä muutosprosessissa.
Kehittämisprojektin aineistonkeruu menetelmänä käytettiin ensisijaisesti moniammatillisen tiimin yhteistyöpalavereita sekä hoitohenkilökunnan kanssa käytyä avointa dialogia. Kehittämisprojektin myötä saatiin käyttöön yhteinen toimintamalli tutkimusasiakkaan palveluprosessille. Toimintamallia testattiin case pilottijaksoilla elokuussa 2018. Minkä jälkeen kehitettyä toimintamallia arvioitiin case jaksojen vastuutiimiä teemahaastattelemalla. Aineistoa analysoitiin laadullista sisällönanalyysiä apuna käyttäen.
Tulokset osoittivat, että kehitetty toimintamalli sujuvoitti lyhytaikaisen kuntoutumisen palveluprosessia. Tiedonkulku ja yhteistyö tehostuivat sekä vastuukysymykset selkeytyivät. Lopputuotokset linkitettiin sisäisen intran IMS- toimintajärjestelmän päivitettyyn prosessikaavioon kronologiseen järjestykseen. Kehitetty toimintamalli jalkautetaan vuoden 2019 aikana KTO:n neljään muuhun tutkimus- ja kuntoutusyksikköön. The social welfare and healthcare reform (SOTE), with its aim to be more economical, presents challenges for rehabilitation services as well. The viewpoint must be on seamlessly accomplishing the secured process of well-timed and goal-oriented rehabilitation (Autti-Rämö et al. 2016, 16.).
This development project was carried out by following the qualitative strategy of action research. The development of the operations of the organization studied was set as the purpose. The objective was to achieve, as the result of the development of multi-professional cooperation, more streamlined, efficient and client-oriented courses of action for the rehabilitation client’s short-term research and rehabilitation period. The Medical Research and Rehabilitation Department (LAKU) of the Support and Expert Center for persons with Intellectual Disability was the pilot department in the process of change.
The primary method of the data collection for the development project were the cooperation meetings of the multi-professional team and also the open dialogue with the nursing staff. With the development project, a common model was established for the service process of the research client. The operational model was tested with case pilot period in August 2018. After that, the developed operational model was evaluated by arranging theme interviews of the team responsible for the case period. The data was analyzed with the help of qualitative content analysis.
The results showed that the developed operational model streamlined the short-term rehabilitation service process. The passing of information and the cooperation were more efficient and the questions on responsibility were clarified. The final output was linked to the process chart of the intranet of the IMS operating system in chronological order. The developed operational model is to be implemented to the other four research and rehabilitation units of KTO during the year 2019.
