Tiivistelmä
Tämän opinnäytetyön tarkoituksena oli tutkia, kuinka hoitotyön toiminnot ja vuorovaikutus toteutuivat sappirakon poistoleikkauksesta eli kolekystektomiasta tulleiden asiakkaiden hoidossa siirtovaiheessa heräämöstä vuodeosastolle. Tarkoituksena oli tutkia hoitotyön toimintojen ja vuorovaikutuksen eri osioita ja eroavaisuuksia jokaisen asiakkaan kohdalla. Havainnoinnin kohteena olivat sekä hoitajat että asiakkaat. Tavoitteena oli antaa osastolle tietoa siirtovaiheen sujuvuudesta ja yhdenmukaisuudesta. Tutkimuksen toimeksiantaja oli HUS Hyvinkään kirurginen vuodeosasto 4A. Osastolla hoidetaan useita erilaisia kirurgisia asiakkaita ja se on jaettu neljään eri soluun. Osastolle tulee usein monia asiakkaita samanaikaisesti ja sen vuoksi asiakkaan vastaanottotilanne voi olla kiireinen.
Opinnäytetyö oli laadullinen tutkimus, jonka aineistokeruumenetelmänä oli havainnointi ilman osallistumista. Havainnointi toteutettiin, kun asiakas siirrettiin heräämöstä kirurgiselle vuodeosastolle. Havainnoinnin aikana kirjoitettiin muistiinpanoja valmiille kaavakkeelle, jotta havainnointi kohdistui ennalta suunniteltuihin asioihin. Havainnoitavat asiat on käyty kirjallisessa työssä läpi tutkittua ja tuoretta teoriatietoa käyttäen. Aineisto analysoitiin sisällönanalyysillä.
Tutkimuksessa selvisi, että vuorovaikutus asiakkaan kanssa toteutui rutiininomaisesti osaston tapojen mukaan. Hoitotyön toiminnoissa, kuten asiakkaan leikkauksen jälkeisessä tarkkailussa ja ohjauksessa havaittiin muutamia puutteita ja vaihtelevuutta hoitajien kesken. Tulosten perusteella suunniteltiin ohjelista vuodeosaston hoitajille, jotta kaikki vaaditut hoitotyön toiminnot tulisivat toteutettua asiakkaan vastaanottotilanteessa.
The purpose of this thesis was to find out how nursing functions and interactions were implemented in the treatment of cholecystectomia clients during the transfer phase from the recovery room to the surgical ward. The aim was to observe the different parts and differences in nursing functions and interactions for each client. Both nurses and clients were observed. The purpose was to provide the department with information on the fluency and consistency of the transfer phase. The principal of this thesis was HUS Hyvinkää Surgical Ward 4A. Many customers come to the ward simultaneously and therefore the customer's reception situation can be busy. Several different surgical clients are treated in the ward and the ward is divided into four different sections.
This thesis was a qualitative research and used a method of observation without participation. The observations were carried out when the customer was transferred from the recovery room to the surgical ward. During the observations, notes were written on the completed form in order to target the pre-planned matters. The matters that were discovered have been studied in the thesis using researched and recent theoretical literature.
The research revealed that the interaction with the client was routinely handled according to the ward´s habits. In nursing functions such as post-operative monitoring and guidance there were found few shortcomings and variations among nurses. Based on the results, a set of instructions were designed for the nurses in order to implement all the required nursing functions in the client's reception situation.
