Tiivistelmä
Organisaatiossa otettiin käyttöön hiljainen raportti vuonna 2014. Hiljaista raportointia käytettäessä potilasta hoitava henkilökunta perehtyy potilaan hoitoon liittyviin tietoihin potilastietojärjestelmästä jo ennakkoon. Hiljainen raportti ei poissulje suullisen raportoinnin mahdollisuutta, vaan tietoja pitää pystyä tarvittaessa täydentämään suullisella raportilla. Tämä korostuu erityisesti päivystysajan leikkauksissa, jolloin potilaan alkuvaiheen hoito saattaa perustua ainoastaan suullisen raportin tietoihin. Ongelmana on että, hiljaisen raportin käyttöönoton jälkeen suullisen raportin antaminen potilaan hoitoon liittyvistä asioista on vähentynyt ja suullisen raportoinnin laatu on heikentynyt. Päivystystilanteissa ei osata raportoida selkeästi potilaan hoitoon liittyviä olennaisia asioita ja siksi potilasturvallisuus saattaa vaarantua. Tavoitteena oli kehittää potilaan hoitoon liittyvää tiedonkulkua ja suullista raportointimenetelmää hoito-osaston henkilökunnan ja leikkausosaston henkilökunnan välillä. Tiedonsiirto hoitovastuun siirtyessä pitää olla selkeää ja potilaan hoidon kannalta oleelliset asiat pitää raportoida niin, että potilasturvallisuus ei vaarannu.
Opinnäytetyön viitekehyksenä käytettiin toimintatutkimuksen metodologiaa. Lisäksi tutkimusta ohjasi lean-ajattelu. Ensimmäisen syklin tarkoituksena oli kirjallisuuskatsauksen avulla kuvata suullisen raportoinnin merkitystä tiedonkulun välineenä potilaan hoitovastuun siirtyessä hoito-osastolta leikkausosastolle. Tutkimuskysymyksenä oli, mikä merkitys suullisella raportoinnilla on tiedonkulun välineenä potilaan hoitovastuun siirtyessä hoito-osastolta leikkausosastolle. Kirjallisuuskatsaukseen valituille artikkeleille (n=9) suoritettiin laadunarviointi, ja aineisto analysoitiin induktiivisella sisällönanalyysillä. Tuloksissa korostui potilasturvallisuusnäkökulma. Tiedon välittämisellä, vastavuoroisella kommunikoinnilla ja organisaation potilasturvallisuuskulttuurin tuntemisella potilasturvallisuutta voidaan kehittää.
Toisen syklin tarkoituksena oli kartoittaa potilasturvallisuuden näkökulmasta päivystyspotilaan tiedonkulkua ja suullisia raportointimenetelmiä potilaan hoitovastuun siirtyessä hoito-osaston henkilökunnalta leikkausosastolle potilasta hoitavalle henkilökunnalle. Kehittämiskysymyksenä oli, miten potilaan hoitoon liittyvän oleellisen tiedon välittämistä suullisesti raportoimalla voidaan kehittää, kun potilaan hoitovastuu siirtyy hoito-osastolta leikkausosastolle. Aineiston keruu toteutettiin havainnoinnin ja kommunikaation avulla lean-menetelmiä hyödyntäen. Gemba walk:n ja arvovirtakuvauksen menetelmien avulla päivystyspotilaan tiedonkulun prosessi visualisoitiin näkyväksi työyhteisön toimijoille, sekä kommunikaation ja avoimen argumentaation avulla toimintaa kehitettiin. Benchmarking-menetelmällä havainnoitiin parhaita käytänteitä päivystyspotilaan tiedonkulun prosessista ja raportointikäytännöistä organisaation toisessa työyksikössä. Kehittämistyön tuloksena kuvattiin päivystyspotilaan tiedonkulun prosessikuvaus työyhteisöön prosessin selkiyttämiseksi. Lisäksi kehitettiin tarkistuslista suullisen raportoinnin tueksi tiedonsiirtoon.
Kolmannen syklin tarkoituksena oli kuvata johtamisen näkökulmasta työyhteisön työntekijöiden osaamisen kehittämistä potilaan hoitoon liittyvän tiedonkulun ja suullisen raportoinnin kehittämisessä. Hypoteesina on, tarkistuslistan käyttäminen suullisen raportoinnin tukena hoitovastuun siirtyessä osaston henkilökunnalta leikkausosastolle potilasta hoitavalle henkilökunnalle edistää potilasturvallisuutta ja potilaan hoidon laatua. Pilotointi toteutetaan PDSA-syklien mukaisesti. Osaamista kehitetään toiminnan ja avoimen argumentaation avulla valmentavasti johtamalla. Hypoteesin todennuksen ja pilotoinnin tuloksien arvioinnin avulla voidaan jatkossa päättää, otetaanko tarkistuslista käyttöön suullisen raportoinnin tueksi potilaan turvallisen tiedonsiirron tueksi. The organization introduced a silent reporting in 2014. When tacit reporting is used, the staff studies patient information in advance using the patient information system. Tacit reporting does not exclude oral reporting; oral reporting can be used to complement information reported tacitly if necessary. This is particularly important in emergency surgeries where the initial treatment of a patient may be based only on orally reported information. The problem is that, since the introduction of tacit, oral reporting is being used less frequently and the quality of oral reporting has deteriorated. In emergency situations, essential issues related to patient care are not clearly reported, and therefore patient safety may be jeopardized. The aim of this thesis was to develop the information flow and reporting methods related to patient care, so that the flow of information in patient handovers is clear and relevant treatment issues are reported in such a way that patient safety is not jeopardized.
The reference framework for this thesis was the methodology of action research. In addition, this thesis was guided by lean thinking. The purpose of the first cycle was to describe the importance of oral reporting to the flow of information when patients are handed over from an inpatient ward to the surgery department through a literature review. The research issue was to study the importance of oral reporting as a means of transferring information when patients are handed over to the surgery department. First the articles selected for the literature review (n=9) were assessed qualitatively and later analyzed using inductive content analysis. The results highlighted the patient safety perspective: transfer of information, reciprocal communication and knowledge of the organization's patient safety culture contributed to the development of patient safety.
The purpose of the second cycle was to explore, from the point of view of patient safety, the transfer of emergency patient information and the use of oral reporting methods when the inpatient ward staff handed emergency patients over to the surgery department staff. The development issue was to study how the oral reporting of essential information related to patient care could be developed when patients were handed over from an inpatient ward to the surgery department. The data was collected through observation and communication using lean methods. The emergency patient information transfer process was visualized for the work community staff using the Gemba Walk and the value stream mapping, and activities were developed using communication and open argumentation. Benchmarking was used to observe the best practices related to emergency patient information transfer processes and reporting practices in another unit of the same organization. As a result of the development work, the emergency patient information transfer process was described for the work community. In addition, a checklist was developed to support oral reporting in patient information transfers.
The third cycle is designed to describe, from the management perspective, the development of staff competence in relation to the development of information transfer and oral reporting. The hypothesis is, that the use of the checklist to support oral reporting in patient handovers from the inpatient ward to the surgery department promotes patient safety and the quality of patient care. The piloting is carried out in accordance with the PDSA cycles. Competence is developed through action and open argumentation by coaching leadership. The evaluation of hypothesis authentication and pilot results can be used to decide whether the checklist should be implemented in the working community.
