Tiivistelmä
Noin 25 000 suomalaista sairastuu vuosittain aivoverenkiertohäiriöön, joista 2500 henkilöllä aivoverenkiertohäiriötä uusiutuu vuoden kuluessa. Aivoinfarkti tai aivoverenvuoto on sairastuneelle traumaattinen kokemus. Sairastumisen jälkeen henkilö tarvitsee eri ammattilaisten tukea, kuten fysioterapeutin, kotisairaanhoitajan ja sosiaalityöntekijän, selviytyäkseen elämässään ja arjessaan. Opinnäytetyön tavoitteena oli kehittää aivoverenkiertohäiriön sairastaneen henkilön selviytymistä hänen kotiutuessaan sairaalasta. Opinnäytetyön metodologiana oli osallistava toimintatutkimus. Ensimmäisen syklin tarkoitus oli kartoittaa, mitä tukea aivoverenkiertohäiriöön sairastunut kotiutuja tarvitsee sairaalasta kotiutuessaan. Kehityskysymys oli, minkälaista tukea aivoverenkiertohäiriöön sairastunut kotiutuja tarvitsee. Vastausta haettiin aivoverenkiertohäiriön sairastuneilta itseltään (n=6) 6-3-5 -menetelmän avulla. Aineisto analysoitiin induktiivisen sisällönanalyysin avulla. Tulokseksi muodostuivat luokat yhteiskuntaan sopeutuminen, tasavertaisuuden mahdollistaminen, tukea elämäntilanteeseen sopeutumiseen sekä osallisuus ja yhteenkuuluvuus. Reflektiossa todettiin, että aivoverenkiertohäiriöön sairastunut kotiutuja tarvitsee vastuuhoitajamallin mukaisen yksilöllisen tuen, sosiaalityön tuen ja kolmannen sektorin palveluja.
Toisen syklin tarkoitus oli kuvata, miten aivoverenkiertohäiriön aiheuttamien neuropsykologisten oireiden kanssa kotiutuvan henkilön voimaantumista voidaan edistää. Tutkimuskysymys oli, miten aivoverenkiertohäiriöön sairastunutta neuropsykologisia häiriötä omaavaa henkilöä voidaan tukea voimaantumisessa. Vastaus haettiin kirjallisuuskatsauksen avulla Medic, Ebsco, Cinahl, Academic Search Elite, Arto ja Ovid tietokannoista. Tutkimuksia saatiin yhteensä kuusi. Aineisto analysoitiin deduktiivisen sisällön analyysin avulla. Analyysissa sovellettiin voimaantumisen osaprosesseja kuvaavaa luokitusta: päämäärät, kykyuskomukset, kontekstiuskomukset ja emootiot. Tuloksina saatiin, että voimaantumisen päämäärän osaprosessia tukivat monialainen potilaan kuntoutumisen seuranta, ohjauksen suunnitelmallisuus ja menetelmät sekä terveellisten elämäntapojen tuki. Emootioiden tukeminen myös tuki voimaantumisprosessissa. Aivoverenkiertohäiriöön sairastuneen kykyuskomuksien tukemista auttoivat osallistuminen omaan kuntoutumiseen, tarpeen mukainen ohjaus ja monialainen kuntoutumisen tukiverkosto. Kontekstiuskomusta tuki toimintaympäristön mukauttaminen ja sosiaalisten suhteiden tukeminen. Reflektiossa korostui henkilön tukiverkoston merkitys, hoitohenkilökunnan asenne ja sosiaalinen kuntoutumisen merkitys voimaantumisen tukena.
Sivusyklin tarkoitus oli tunnistaa afasiaa sairastavan kotiutujan asiakas- ja potilasturvallisuutta edistävää kotiutumisprosessia. Kehittämiskysymys oli, miten afasiaa sairastavan kotiutujan asiakas- ja potilasturvallisuutta edistävää kotiutumisprosessia voidaan kehittää. Vastausta haettiin benchmarkingin avulla vertailukohteesta, jossa oli aikaisemmin kehitetty KYTKE-hankkeessa aivoverenkiertohäiriön sairastaneen kotiutusprosessia. Bencmarkingin tulokset osittivat, että kolmannen sektorin toimijoiden kanssa tehtävä tiivis yhteistyö kotiutumisvaiheessa, sosiaalisen kuntoutumisen kehittäminen, yhteisöllisyys ja vertaistuki edistivät afasiaa sairastavan kotiutujan kuntoutumista ja asiakasturvallisuutta. Kolmannen syklin tarkoitus oli kuvata, miten aivoverenkiertohäiriöön sairastuneen henkilön sosiaalista kuntoutumista voidaan tukea sairaalasta kotiutumisen jälkeen. Kehittämiskysymys oli, miten sosiaalista kuntoutumista edistetään aivoverenkiertohäiriöön sairastuneen henkilön kotiutuessa sairaalasta. Kehittämiskysymykseen haettiin ratkaisua suunnittelemalla hankehakemus, ESTONO. Hankehakemus sisälsi viisi eri toimenpidekokonaisuutta, joilla vastattiin aivoverenkiertohäiriöön sairastuneen henkilön sosiaalisen kuntoutumisen haasteisiin yhteistyössä eri sidosryhmätoimijoiden kanssa paikallisella ja alueellisella tasolla. Toimenpidekokonaisuutta kuvaavat tavoitteet ja sisällöt muotoutuivat toisen ja sen sivusyklin tulosten reflektioiden mukaisesti. Out of the approximately 25,000 persons diagnosed with cerebrovascular disease (CVD) each year, 2,500 experience recurrent CVD within one year. Brain stroke or cerebral hemorrhage is a traumatic experience for a person. After having CVD, they need the support of different professionals, such as a physiotherapist, home nurse and social worker, to cope with their daily lives. The aim of this thesis was to develop means that would help CVD patients to cope after discharge from hospital.
The methodology of this thesis was participatory action research. The purpose of the first cycle was to identify what support CVD patients needed after discharge from hospital. The development question was to explore what kind of support CVD patients needed after discharge from hospital. The material was collected from CVD patients (n = 6) with the 6-3-5 method and analyzed by inductive content analysis. The analysis resulted in the following categories: social adaptation, equal opportunities, and support for adapting to a new life situation, and participation and cohesion. In the reflection it was stated that a CVD patient would need individual support as well as support from social and third sector services in accordance with the primary nurse model.
The purpose of the second cycle was to describe how the empowerment of a person with neuropsychological symptoms caused by CVD could be promoted. The research question was to study how a person with neuropsychological disorders induced by CVD could be supported in empowerment. Information was sought through a literature review from Medic, Ebsco, Cinahl, Academic Search Elite, Arto and Ovid. A total of six studies were obtained. The material was analyzed by deductive content analysis. The analysis used a classification describing the partial processes of empowerment: goals, skills, contextual commands, and emotions. As a result, the partial process of empowerment was supported by multidisciplinary patient rehabilitation monitoring, systematic planning and methods, and support for healthy lifestyles. Supporting emotions also supported the empowerment process. Participation in rehabilitation, need-based guidance and a multidisciplinary rehabilitation support network supported the skills to cope with CVD. Contextual commands were enhanced by adapting the operating environment and supporting social relationships. The reflection emphasized the importance of the support network, the attitude of nursing staff and the importance of social rehabilitation to support empowerment.
The second cycle generated a secondary cycle the purpose of which was to identify the factors contributing to the rehabilitation and client and patient safety of aphasia patients. The discharge process for CVD patients earlier developed in the KYTKE-project was benchmarked. The results showed that close cooperation with third-sector actors in the discharge phase, development of social rehabilitation, communality and peer support contributed to the rehabilitation and client safety of aphasia patients.
The purpose of the third cycle was to describe how the social rehabilitation of CVD patients could be supported from discharge from hospital. The development task was to explore how social rehabilitation was promoted when a CVD patient was discharged from the hospital. A solution was sought by drawing a project application, ESTONO. The project application included five different action packages that respond to CVD patients’ social rehabilitation challenges in cooperation with various stakeholder actors at the local and regional level. Objectives and contents describing the measures to be taken were formulated on the basis of the reflections of the results of the second and its secondary cycle.
