Tiivistelmä
Nyky-yhteiskunnan kiire, stressi, tavoitteet ja ympäristön luoma paine ovat merkittäviä tekijöitä unettomuuden syntyyn. Unettomuus on yleistä Suomessa ja maailmalla. Jokainen kokee jossain vaiheessa elämäänsä tilapäistä unettomuutta mm. stressin vuoksi. Viime vuosina mediassa on puhuttu paljon unettomuuden hoidosta unilääkkein ja sielläkin on tullut esille niiden sivu- ja haittavaikutukset. Lisäksi tv:ssä näytetään usein mainoksia reseptivapaista melatoniini -tuotteista, joissa luvataan unettomuudesta paranemista.
Opinnäytetyö sai alkunsa mielenterveys- ja päihdetyön harjoittelun aikana alkuvuodesta 2018, kun tekijät huomasivat, minkälaisia ongelmia eri-ikäisillä asiakkailla oli unensaannissa. Sairaanhoitajaopinnoissa unta käsitellään vain pintapuolisesti, jolloin unettomuuden vaikutukset terveyteen jäävät opiskelematta. Opinnäytetyön tekijöiden mielenkiinto heräsi, kun he huomasivat, kuinka suuresta ongelmasta on kyse. Huomio kiinnittyi siihen, kuinka asiakkaille tarjottiin ensimmäisenä vaihtoehtona unilääkkeitä. Lisäksi aikaisemman työkokemuksen myötä tekijät ovat nähneet kuinka esim. vanhukset syövät unilääkkeitä jopa monia vuosia. Syntyi tarve tarjota asiakkaille lääkkeetön vaihtoehto lääkkeiden tilalle tai rinnalle.
Toimeksiantajana opinnäytetyössä oli Varkauden kaupungin Mielenterveys- ja päihdepalvelut, jotka huomasivat oppaasta saatavan hyödyn asiakkaan itsehoitoon ja ohjaustilanteisiin.
Uni koostuu monesta eri tekijästä ja sen tarve on yksilöllistä. Tilapäistä unettomuutta aiheuttavat stressin ja mieltä painavien asioiden lisäksi uni- ja valverytmin häiriöt. Käypä hoito -suosituksen mukaan unettomuuden ensisijainen hoitomuoto tulisi keskittyä lääkkeettömiin menetelmiin. Kognitiivisesta ja behavioraalisesta terapiasta on saatu hyviä ja kestäviä tuloksia unen laadun paranemisessa. Unihygienia, ravinto ja nautintoaineet, liikunta ja rentoutumismenetelmät vaikuttavat unensaantiin ja unen laatuun.
Jatkotutkimuksissa voitaisiin selvittää, kuinka lääkkeettömien unettomuuden hoitokeinojen toteutus onnistuu kotona ja kaipaavatko asiakkaat lisää tietoa tai tukea. Lisäksi voitaisiin tehdä kyselytutkimus työntekijöille, millaisena he kokevat lääkkeettömien unensaantikeinojen ohjauksen. The rush, stress and pressure of modern society and the individual ambitions are significant causes of the development of insomnia. Insomnia is very common in Finland and around the world. Anyone may experience temporary insomnia at some point of their live, for example because of stress. There has been a lot of discussion about the usage of medication as treatment for insomnia in the media. The discussion has dealt with the side- and injurious effects of sleeping pills. The number of TV advertisements for melatonin products have increased and the advertisements promise that the consumers will get rid of insomnia.
This thesis was first started in January 2018 during the authors´ mental health and substance abuse clinical training. During the training period the authors paid attention to how many patients from different age groups suffered from insomnia. Nursing studies cover the necessity of sleep on the face of it, but a lot of information must be acquired on one´s own. The authors noticed how big a problem insomnia is and how the patients were offered medicines as a primary treatment for insomnia. The former work experience of the authors had showed that for example elderly people had been using large amounts of sleeping pills for years. For this reason the thesis authors wanted to offer a non-pharmacological alternative to medication.
The client organization of the thesis was Mental health and substance abuse services of the city of Varkaus. This study was assigned by the client organization as they saw how much a guide book would benefit the self-care of patients and the guidance given by nurses.
Sleep consists of many different factors and the need for sleep is individual. Temporary insomnia is caused by stress, concerning thoughts and disturbances in the cycle of sleep and waking state. The Käypä hoito -recommendations state that the non-pharmacological treatment of insomnia should be the primary cure. Ac-cording to clinical tests, cognitive and behavioristic therapy bring good results as they improve the quality of sleep in the long term. Sleep hygiene, nutrition, stimulants, exercise and different relaxation techniques can affect the onset and quality of sleep.
A topic for further study could be doing research on how patients experience the usage of non-pharmacological treatment for insomnia at home and whether they need more information or support. Furthermore, a survey meant for nurses about how they feel giving guidance on non-pharmacological sleep methods could also be useful.
