Tiivistelmä
Turun yliopistollisen keskussairaalan keuhko-osastolle 1 valmistui loppuvuodesta 2017 LUOVA-tila sairaalan ja taiteilijoiden yhteistyönä. LUOVA-tilan tavoitteena on innostaa potilasta oman terveytensä edistämiseen sekä edistää paranemista. Lähtökohtana on parempi potilaskokemus. LUOVA-tilassa on mahdollisuus toteuttaa omaa luovaa tekemistä taiteen, kuten musiikin ja valokuvan sekä ohjelmasovellusten avulla. Kaikki LUOVA-tilaan pääsevät potilaat eivät välttämättä pysty itse tekemään taidetta; tilassa koettu aistinen kokemus on heille taidetta.
Kehittämisprojektin tarkoituksena oli kehittää ohjausmalli ja ohjeet LUOVA-tilaa varten sekä kerätä hoitajien ja potilaiden kokemuksia LUOVA-tilan käytöstä. Tutkimusaineisto kerättiin teemahaastattelun avulla sairaanhoitajilta sekä välittömänä palautteena potilailta LUOVA-tilassa käynnin yhteydessä kyselylomakkeen avulla. Tutkimus rajattiin koskemaan COPD- ja pneumoniapotilaita. Lisäksi hoitajille annettiin mahdollisuus kertoa kokemuksensa sähköisen kyselylomakkeen avulla.
Sähköisen kyselylomakkeen kautta saatiin yksi vastaus. Potilaille suunnattuun kyselylomakkeeseen vastasi 5 potilasta ja haastatteluun osallistuneita hoitajia oli 3. Ipadin käyttö sekä sovellukset olivat vastaajille vieraita. Potilaiden LUOVA-tilassa käymistä hankaloittivat terveydelliset syyt (esim. sairaus, uupumus, eristys- tai vuodepotilas), tiedonpuute LUOVA-tilasta sekä harrastuskaverin puute. Myös haastatteluissa mainittiin, että potilailla ei ole riittävästi tietoa LUOVA-tilasta ja sen mahdollisuuksista. LUOVA-tilan kuvattiin antavan jaksamista, voimaa, positiivista energiaa, rauhaa, uudenlaisia kokemuksia, yhteisöllisyyttä, sosiaalisia kontakteja ja tekemistä. Samat syyt olivat ne, jotka saivat palaamaan tilaan takaisin. Lisäksi luontokuvia ja äänimaisemaa kiiteltiin. LUOVA-tilassa käyminen koettiin luontevana osana hoitoa. Enemmistö haastatelluista oli innostuneita lähtemään LUOVA-tilaan yhdessä potilaan kanssa. LUOVA-tilan kaltainen toimintatapa koettiin erittäin tärkeänä tai tärkeänä. Hoitajat kuitenkin kokivat, että tilan käyttöaste on vielä matala. Sekä potilaat että hoitajat toivoivat tilaan lisää istumapaikkoja ja tilaa seurustelulle. Molemmat ryhmät myös pohtivat, voisiko tilassa olla lisäksi ohjattua toimintaa. Potilaiden palautteen perusteella iPadin ja sovellusten ohjeistusta lisättiin. Käytettävyyden ongelmakohdat ja käyttäjien toiveet konkretisoituivat kokemusten myötä.
Osastolla on nyt välineet LUOVA-tilan käyttämiseen, mutta se, että tilan käyttöastetta saataisiin nostettua, edellyttää uudenlaista ajattelu- ja toimintatapaa, tilasta kertomista potilaille ja omaisille sekä ohjeistukseen ja ohjausmalliin perehtymistä. CREATIVE-space, developed in co-operation with the hospital and artists, has been introduced in the pulmonary diseases inpatient ward 1. In CREATIVE-space patients have the opportunity to realize their own creativity trough music and art program applications. The wall is projected as a landscaping video with nature sounds. The objective of the study was to design guidance model for the CREATIVE-space and to collect experiences. The aim of CREATIVE-space is to create a new kind of environment that would inspire the patient to promote their own health, help patient to endure the illness and promote healing. The development and research work carried out trough action research. The collection of the data carried out with two different methods: semi-structured survey for patients and theme interview with nurses. In addition, nurses were given the opportunity to share their experience whit an electronic survey. Patients were hospitalized because of pneumonia or obstructive pulmonary disease.
Five patients responded to the patient survey and three nurses were interviewed. One response was received through the electronic survey for nurses. It was found that the use of the devices and applications in the CREATIVE-space was not familiar to the respondents. Visits to the CREATIVE-space were complicated by health reasons, lack of knowledge and a friend to visit with the CREATIVE-space. Nurses also reported that patients do not know enough about the CREATIVE-space. The CREATIVE-space helped patients cope with their disease and gave positive energy, strength, peace, new experiences, community, social contacts and something to do. The same reasons were mentioned as reasons to return to the CREATIVE-space. Nature photographs and the sound landscape were praised. Visiting the CREATIVE-space was seen as a natural part of the treatment and care. Most of the nurses were enthusiastic to visit in the CREATIVE-space with patients. The new approach was considered either very important or important. However, the utilization as the CREATIVE-space was still considered low by the nurses. Both patients and nurses hoped for places to sit on and a space for socializing. Both groups also wondered if there could be guided activities in the space. Based on the feedback provided by the patients, instructions for iPads and applications were added. The experiences gathered made the wishes of the users more concrete.
The ward has the tools to use the CREATIVE-space but a new way of thinking and acting is required to increase the utilization. Nurses need to tell the patients and relatives about the CREATIVE-space and they also need to familiarize themselves with the instructions and the guidance model.
