Tiivistelmä
Uudistettu tartuntatautilaki (1227/2016) astui voimaan 1.3.2018. Lain velvoittamana terveydenhuollon ja sosiaalihuollon toimintayksiköiden on suunnitelmallisesti torjuttava hoitoon liittyviä infektioita. Tämän kehittämisprojektin tarve havaittiin Uudenkaupungin sosiaali- ja terveyspalvelujen ulkoisessa auditoinnissa. Terveyspalveluissa ei ole ollut systemaattista seurantaa hoitoon liittyvissä infektioissa. Kehittämisprojektin aihe tuli yhtenä lisänäyttöpyyntönä, jotta voidaan laaduntunnustus myöntää.
Kehittämisprojektilla oli kaksi tavoitetta. Ensimmäisenä oli muokata potilastietojärjestelmää siten, että se mahdollistaa systemaattisen hoitoon liittyvien infektioiden seurannan. Toisena tavoitteena oli suunnitella systemaattisen kirjaamisen kirjaamisohje, sekä viedä se käytäntöön Uudenkaupungin sosiaali- ja terveyspalveluiden eri toimintayksiköihin.
Kehittämisprojektin tiedonkeruumenetelmänä käytettiin teemahaastattelua. Teemahaastattelun aiheet kohdistuivat muokatun potilastietojärjestelmän sekä kirjaamismallin toimivuuteen sekä systemaattisen kirjaamisen vaikutukseen potilasturvallisuuteen sekä laatuun. Kehittämisprojektin aineisto analysoitiin aineistolähtöisesti sisällönanalyysiä hyödyntäen.
Kehittämisprojektin tuotoksena saatiin luotua yhtenäinen systemaattinen kirjaamismoduuli hoitoon liittyvistä infektioista potilastietojärjestelmään. Yhtenäisellä kirjaamisen toimintamallilla halutaan parantaa potilasturvallisuutta sekä parantaa hoitotyön laatua. The reformed Communicable Diseases Act (1227/2016) entered into force on 1 March 2018. Under the Act, healthcare and social welfare units are required to take systematic action to control healthcare-associated infections. The need for this development project was detected in external auditing performed in the social welfare and healthcare service of the City of Uusikaupunki, Finland. No systematic monitoring was used by the healthcare services in the area of healthcare-associated infections. The topic of this development project emerged as a request for further evidence required for obtaining quality certification.
The development project had two objectives. The first involved modifying a patient records system to facilitate the systematic monitoring of healthcare-associated infections. The second was to design guidelines for systematic data entry in the records system, and to implement these in the different operating units of the social welfare and healthcare services of Uusikaupunki.
The theme interview was used as the data collection method in this development project. The topics of the theme interview were focused on the operability of the modified patient records systems and guidelines for recording information in the system as well as the effect of this systematic record keeping on both patient safety and quality. The data of the development project was analysed with a data-driven approach utilizing the content analysis method.
The development project produced a cohesive, systematic module on recording data about healthcare-associated infections in the patient records system. The uniform model for recording data in the patient records system aims at improving patient safety and enhancing the quality of nursing.
