Tiivistelmä
Tekoäly on kiinnostanut tutkijoita ja tieteilijöitä jo vuosikymmenten ajan. Lähtökohtaisesti kysymys on siitä, miten teknologia voidaan valjastaa suorittamaan tehtäviä, joissa on perinteisesti vaadittu joustavaa ajattelua ja inhimillistä päätöksentekoa eli älykkyyttä. Näihin päiviin saakka koneet ovat noudattaneet niille eksplisiittisesti ohjelmoituja toimintaohjeita eli algoritmejä. Tekoälyn aikakaudella keskeiseksi tekijäksi on noussut koneiden ja ohjelmistojen kyky oppia. Oppiminen ei kuitenkaan tapahdu itsestään, vaan se perustuu koneoppimiseksi kutsuttuun prosessiin. Koneoppimisessa algoritmille ohjelmoidaan toimintalogiikka, jonka avulla se käsittelee valtavan määrän dataa ja tuottaa päätelmiä havaitsemiensa säännönmukaisuuksien pohjalta. Mitä enemmän kor-kealaatuista dataa algoritmille syötetään, sitä paremmaksi se oppii. Koneiden laskentatehon kehittyminen sekä digitaalisen datan määrän kasvu ja saatavuus ovat tekijöitä, jotka ovat viime aikoina mahdollistaneet tekoälyn räjähdysmäisen kehittymisen.
Digitalisaatioon ja teknologian kehittymiseen liittyy odotus toiminnan tehostumisesta, tuottavuuden paranemisesta ja kustannusten kasvun hillitsemisestä. Yksi merkittävimmistä julkisen talouden kustannustekijöistä on sosiaali- ja terveydenhuoltopalvelujen tuottaminen. Huomionarvoista sosiaali- ja terveydenhuollon kulurakenteessa on se, että useiden eri tutkimusten mukaan kustannukset eivät kerry tasaisesti koko väestöstä, vaan valtaosa kuluista aiheutuu pienestä osasta palveluja käyttävistä asiakkaista. Näiden kustannuksia kerryttävien asiakas- ja potilasryhmien parempi tunnistaminen ja painopisteen siirtäminen ennaltaehkäisevien palvelujen järjestämiseen voisivat olla mahdollinen ratkaisu sosiaali- ja terveydenhuollon kustannusten hallintaan.
Tässä opinnäytetyössä tekoälyllä on keskeinen rooli, mutta pääasiallinen kiinnostus kohdistuu tekoälyn mahdollisuuksien hyödyntämiseen sosiaali- ja terveydenhuollossa, esimerkiksi edellä kuvattujen kohderyhmien tunnistamisessa. Tarvittavan ymmärryksen muodostamiseksi työssä käydään aluksi läpi, mitä tekoälyllä tarkoitetaan, miten se on kehittynyt ja mihin koneiden älykkyys tämänhetkisen käsityksen mukaan perustuu. Lisäksi käsitellään kysymyksiä tekoälyyn liittyvistä rajoituksista ja uhkakuvista. Tämän jälkeen tutustutaan siihen, millaisia tekoälykokeiluja sosiaali- ja terveydenhuollon toimijat ovat Suomessa tehneet ja millaisin tuloksin. Tarkastelussa ovat Pirkanmaan sairaanhoitopiirin kokeilu monisairaiden potilaiden tunnistamiseksi, Espoon kaupungin kokeilu asiakkaiden palvelutarpeiden tunnistamiseksi ja Helsingin ja Uudenmaan sairaanhoitopiirin kokeilut teko-älyn hyödyntämisestä erikoissairaanhoidossa diagnosoinnin ja hoitopäätösten tukena. Kokeilut toteutettiin vuosina 2016-2017.
Työn empiirinen osuus perustuu sairaanhoitopiireille tehtyyn kyselyyn, jonka avulla haluttiin selvittää sairaan-hoitopiirien yleistä suhtautumista, tahtotilaa ja odotuksia tekoälyä hyödyntäviin teknologioihin liittyen. Kyselyllä kartoitettiin myös sitä, onko vastaajaorganisaatioissa toteutettu tekoälyteknologiaan liittyviä kokeiluja tai onko heillä käytössä joitain robotiikan tai ohjelmistorobotiikan ratkaisuja. Lisäksi haluttiin selvittää, miten sairaanhoitopiirit ovat varautuneet tekoälypohjaisten ratkaisujen käyttöönottoon tulevaisuudessa. Kyselyyn vastasi yhdeksän sairaanhoitopiiriä kahdestakymmenestä. Tulosten mukaan sairaanhoitopiireillä on ollut tai on parhaillaan käynnissä erilaisia tekoälyä hyödyntäviä kokeiluja, mutta vakiintunut käyttö tai suunnitelmat tekoälyn käyttöönottamiseksi ja asiaan liittyvän oman osaamisen kehittämiseksi ovat vielä vähäisiä. Artificial intelligence has attracted researchers and scientists for decades. The question is in principle how tech-nology can be harnessed to perform tasks that traditionally require flexible thinking and human decision making, that is, intelligence. Until now, the machines have followed explicitly programmed operating instructions, ie. algorithms. In the era of artificial intelligence, the ability to learn about machines and software has become a key factor. However, learning does not happen by itself, but is based on a process known as machine learning, in which an algorithm has been programmed with a certain operational logic to handle a huge amount of data and to draw conclusions based on the regularities observed. The more high-quality data is fed to the algorithm, the bet-ter it learns. The development of computing power, coupled with the growth and availability of digital data, is the most important factor in recent explosive evolution of artificial intelligence.
Digitalisation and technology development are expected to result in improved operational efficiency, improved productivity and curbing cost growth. One of the most important cost factors in public finances is the provision of social and health care services. Noteworthy in the cost structure of social and health care is that, according to several studies, costs do not accrue steadily across the population, but most of the costs are borne by a small proportion of the customers using the services. Better identification of these cost-generating customer and pa-tient groups and shifting the focus to the provision of preventive services could be a possible solution for manag-ing social and health care costs.
In this thesis, artificial intelligence plays a key role, but the main interest is in utilizing the potential of artificial intelligence in social and health care, for example in identifying the target groups described above. In order to create the necessary understanding of the work, we first start by examining what artificial intelligence means, how it has evolved and what the intelligence of the machines is based on. In addition, issues and limitations of artificial intelligence are discussed. After that, we will get acquainted with what kind of artificial intelligence exper-iments have been done by social and health care providers in Finland and with what results. The review includes an experiment by the Pirkanmaa Hospital District to identify patients with multiple illnesses, an experiment by the City of Espoo to identify customers with an increased need for social care, and experiments by the Helsinki and Uusimaa Hospital District to utilize artificial intelligence in specialized medical care to support diagnosis and treatment decisions. The experiments were carried out in 2016-2017.
The empirical part of the work is based on a questionnaire on hospital districts to find out about the general atti-tudes, intent and expectations of medical care districts in the use of artificial intelligence technologies. The ques-tionnaire also surveyed whether experimentation on artificial intelligence was carried out by respondent organiza-tions or whether they have some robotic or software robots’ solutions. It was also wanted to find out how the hospital districts have been prepared for the introduction of artificial intelligence solutions in the future. The ques-tionnaire was answered by nine hospital districts in twenty. According to the results, hospital districts have had or have been experimenting with different artificial intelligence, but the established use or plans for the deployment of artificial intelligence and the development of relevant know-how are still limited.
